Program director/licensee designee Angel Bates stated that infants diapers are changed on the diaper changing table in the infant room. She stated that diapers are not changed outside on the grass or on strollers. Child Care Staff Member 1 has been employed by the center for four years. She stated that she has never changed diapers outside on the grass or on a stroller and she has never seen it done. Infants diapers are changed on the diaper changing table. Child Care Staff Member 2 has been employed by the center for two years. She stated that children’s diapers are never changed on the grass or on a dirty stroller. She stated that diapers are changed on the changing table or on a changing mat in the bathroom. Child Care Staff Member 3 has been employed by the center since November 2022. She stated that she would never change a child’s diaper on the grass or on a stroller. She stated that she changes diapers on the changing table. Child Care Staff Member 4 has been employed by the center for two and half years. She stated that children are never changed outside of the center or in a stroller. Infants diapers are changed on the diaper changing table. I attempted to contact Child A’s Mother but did not receive a return call. I interviewed Child B and Child C’s Mothers. Both stated that they are happy with the care that their children receive at the center. They have only seen children diapered on the diaper changing table.

Program director Angel Bates Child A was in the infant room while Child A’s Mother was in the preschool room. The center is small and there are no doors to any of the rooms so Child A’s Mother could look around the corner into the infant room while standing in the preschool room. Ms. Bates stated that there had been an ongoing problem with Child A’s Mother taking Child A to the preschool room. Ms. Bates explained to Child A’s Mother that Child A had to remain in the infant room because she would cause the preschool room to be out of ratio due to Child A’s age of 5 months. Ms. Bates stated that on Monday, August 7, 2023, Child Care Staff Member 1 and Child Care Staff Member 4 were working together in the infant room. Ms. Bates again had to speak to Child A’s Mother about taking Child A into the preschool room. At 12:15 PM Child A’s Mother told Ms. Bates that she wasn’t feeling well and that she needed to go home early. Ms. Bates told Child A’s Mother that it was fine for her to leave. At 12:43 PM, Ms. Bates received a text message from Child A’s Mother stating that she heard Child A crying in the infant room so she peaked in and saw Child Care Staff Member 1 leaned over Child A’s crib telling Child A to “stop crying right now” in a “very stern voice”. Child A’s Mother stated in the text that she asked Child Care Staff Member 1 if she yelled at Child A. Child Care Staff Member 1 told Child A’s Mother no. Child Care Staff Member 1 stated that she was talking to another child in the room. Child A’s Mother also stated in the text that she wasn’t just saying this because of being told not to take Child A into the preschool room. Ms. Bates stated that she was in and out of all of the classrooms on Monday, August 7, 2023, because she was conducting tours that morning. She stated that the child care staff members and the children all seemed to be fine that day. Ms. Bates stated that she pulled the camera footage from earlier that day. She saw Child A laying in the crib. The video footage has no sound so she could not tell if Child A was crying but she saw Child Care Staff Member 1 lean over the crib and give Child A a pacifier. The video then showed Child Care Staff Member 1 turning around and talking to Child A’s Mother but again the video has no audio so Ms. Bates does not know what was said. Ms. Bates stated spoke with Child Care Staff Member 1 who denied yelling at Child A. Child Care Staff Member 1 stated that Child A’s Mother asked her if she yelled at Child A and she told her that she did not. Child Care Staff Member 1 stated that she told one of the other two children in the room that were laying on cots to lay down. Child Care Staff Member 1 stated that she did not yell at 4 the other children but Child A’s Mother could easily hear her because the rooms are all open and close together. Ms. Bates stated that Child Care Staff Member 1 has worked at the center for four years and she has never witnessed her yelling at children or received any complaints about her. Ms. Bates stated that Child A’s Mother had been upset about other things that did not relate to Child A so she felt it best to “go their separate ways”. Ms. Bates let Child A’s Mother go and told her that she could return to the center to pick up her belongings. Child Care Staff Member 1 stated that on Monday, August 7, 2023, she was working in the infant room with Child Care Staff Member 4. Child Care Staff Member 1 stated that after feeding Child A lunch she put in her the crib at around 11:45 PM. Child A was a little fussy so she reached into the crib and gave Child A a pacifier. Child A’s Mother was in the preschool room near the cubbies looking into the infant room when she asked Child Care Staff Member 1 if she yelled at Child A. Child Care Staff Member 1 told Child A’s Mother that she was talking to another child who was laying on a cot. Child Care Staff Member 1 stated that she talks loudly, but she was not speaking to the child in a “mean way”. Child Care Staff

Program director Angel Bates stated that Child A was just beginning to be fed food instead of just bottles. On Monday, August 7, 2023, Child A was in care from 8:00 AM until 12:30 PM. During that time Child A was fed food twice, one bottle and was offered a second bottle. Ms. Bates stated that she never received any complaints about Child A not being fed on demand. Child Care Staff Member 1 stated that on Monday, August 7, 2023, at around 8:30 AM Child A was fed oatmeal and a bottle. At approximately 10:45 AM Child Care Staff Member 4 put Child A in a high chair and fed Child A and offered Child A a bottle that she did not want. Ms. Bates stated that Child A is always fed on demand and she have never had a previous complaint that Child A was not being fed. Child Care Staff Member 4 stated that Child A and all other children are fed usually before they demand to be fed. Child Care Staff Member 4 stated that Child A is always fed or offered a bottle at least every couple of hours. On Monday, August 7, 2023, Child Care Staff Member 4 stated that she fed Child A at lunch time baby food and offered a bottle but she did not want it. Child A was also fed earlier by Child Care Staff Member 1. I attempted to contact Child A’s Mother but did not receive a return call. I interviewed Child B and Child C’s Mothers. Child B and Child C’s Mothers stated that they have no concerns about the care that their children receive at the center. Child B’s Mother stated that the child care staff members are all caring and communicate well. Child C’s Mother stated that she is very happy with all of the child care staff members at the center. Child B and Child C’s Mother stated that they receive messages about what Child B and Child C are fed throughout the day on the ProCare application. There are no concerns about Child B or Child C being fed appropriately.

Program director Angel Bates stated that in March 2022 the water was routinely tested by Michigan Department of Environment Great Lakes & Energy Drinking Water Laboratory and the results showed that the water was positive for bacteria. Ms. Bates informed the local health department and hired a company to come out to clean the water system. Ms. Bates was advised by the health department that the water was safe for handwashing and washing dishes but she needed to use only bottled water for drinking until the bacteria was no longer present in the water. Ms. Bates stated that she purchased and used only bottled water for two or three months at the center. During that time the water was retested consistently until it was cleared and no longer contained bacteria. Child Care Staff Member 1 stated that over a year ago the water at the center was tested and there was some type of problem so only bottled water was used until it was cleared up. Child Care Staff Member 2 stated that there was a “bad” test result with the water once and Ms. Bates said it was necessary to use bottled water. Everyone at the center used jugs of water until the water was “good” again. Child Care Staff Member 3 stated that the water at the center was tested and it was advised that the water keep running to flush out the problem. Bottled water had to be used during this time. Child Care Staff Member 4 stated that she is not sure what happened but at some point, they were not able to use the water at the center. Ms. Bates bought gallons of water which they used until the water could be used again. I attempted to contact Child A’s but did not receive a return call. I reviewed the report completed by George McAttee from Freshwater Testing Services LLC which indicated that on August 17, 2022, the drinking water system at the center tested positive for Coliform Bacteria. The contamination was caused by an outside influence at the well head. No contamination was detected inside the center. Kristine Spencer from the Jackson County Health Department was immediately advised of the test result and provided written documentation indicating that the water at the center was safe only for handwashing but not for drinking. Ms. Spencer required signs to be posted above all sinks at the center indicating that the water was only to be used for handwashing. According to the reports submitted by Freshwater Testing Services LLC and the health department, all safety protocols stayed in force and were overseen by Mr. McAttee and Ms. Spencer along with program director Angel Bates. During this time bottled water was put in place for the staff and children’s personal consumption. The contamination was finally cured on March 3, 2023, after 3 chlorination’s performed by a licensed well driller, structural 8 improvements around the well head and gutter and roof cleaning to lessen the impact of potential outside influences at the well head. Both Mr. McAttee and Ms. Spencer reported that Ms. Bates was completely cooperative during the entire process and followed instructions “to the letter”.

Program director Angel Bates stated that earlier in the year a tree outside of the center was cut down because the roots were growing into the septic tank however that never caused a problem with the plumbing in the center. On May 23, 2023, water from a toilet overflowed from the bathroom into the heater room which is next door to the bathroom. Ms. Bates called Mr. Rooter Plumbing and they came out the same day to snake out the drain. Mr. Rooter returned a second time to use a hydro jet to clean the drain. Ms. Bates stated that she was advised that staff were flushing tampons down the toilet which was clogging the drain. Ms. Bates stated that she instructed the staff to discontinue flushing tampons and there has not been an issue with the toilet flooding since. Ms. Bates stated that the water from the toilet never went into the kitchen or the classrooms. Child Care Staff Member 1 stated that the water from the toilet in one of the bathrooms overflowed into the furnace room next to the bathroom after children flushed paper towel down the toilet. Child Care Staff Member 1 stated that she bleached and sanitized the floor near the furnace room and in the bathroom. A plumber came in and snaked out the drain and there have not been any problems with flooding since. Child Care Staff Member 2 stated that there was an incident where the toilet backed up. A plumber found that wipes or something like that was found stuck in the drain. The problem was fixed as soon as it happened. Child Care Staff Member 2 stated 9 that the water from the overflow only went into the hallway right outside of the bathroom and did not reach the classrooms or the kitchen. Child Care Staff Member 3 stated that water backed up twice in the bathroom because of paper towels being flushed down the toilet. Child Care Staff Member 3 stated that she did not see the water flood into the hallway but she knows that Ms. Bates had plumbers come out to fix the problem and there have been no issues since the beginning of the spring. Child Care Staff Member 4 stated there were a couple of times when the toilet overflowed because excessive toilet paper or paper towel was flushed down the toilet. Child Care Staff Member 4 stated that the flooding did not reach the classroom or kitchen. Ms. Bates had the plumber come out to the center and there has been no issues with the toilet overflowing since. I attempted to contact Child A’s Mother but I did not receive a return call.

Program director Angel Bates stated that there are no bugs in the center. Ms. Bates stated that she has the building sprayed three times a year for moths, stink bugs, ants, spiders, wasps and box elder bugs. Ms. Bates stated that the exterminator also looked for roaches, fleas and bed bugs but found none. The exterminator only sprays over the weekend when children are not present. Ms. Bates stated that stink bugs began coming into the building in the morning when 10 parents arrived with their children so she began having pest treatment services done in March 2022. Child Care Staff Member 1 stated that she has seen dead beetles on the shelf where cubbies are stored near the front door when she cleaned. Child Care Staff Member 1 stated that she informed Ms. Bates about the beetles and Ms. Bates began having pest treatment done every two to three months which eliminated the problem. Child Care Staff Member 1 stated that she has never seen bugs in the drawers in the kitchen. Child Care Staff Member 2 stated that she has never seen a living bug at the center in the two years that she has been employed. There have been dead bugs near the front door that come in from outside to drop off. Child Care Staff Member 2 stated that she and other child care staff members sweep multiple times a day getting rid of the bugs. Child Care Staff Member 2 stated that Ms. Bates has a pest service come out regularly to get rid of the bugs. Child Care Staff Member 2 stated that she wipes down the cubbies daily and she has never seen any bugs. Child Care Staff Member 2 also stated that she has never seen bugs anywhere in the kitchen. Child Care Staff Member 3 stated that when summer first started there were dead beetles on the floor by the front door. Child Care Staff Member 3 saw the beetles and swept them up. She has never seen bugs in the cubbies where diapers are stored or in the drawers in the kitchen. Child Care Staff Member 3 stated that someone let Ms. Bates know about the beetles and she has not seen them since. Child Care Staff Member 4 stated that last year she saw two beetles come in the door as children came in during drop off. Child Care Staff Member 4 got those beetles out immediately. She also saw a dead beetle on the floor as she was sweeping the floor. Pest control comes out on the weekend throughout the year. Child Care Staff Member 4 has never seen bugs in the cubbies or in the drawers in the kitchen. I attempted to contact Child A’s Mother but did not receive a return call. During the onsite inspection on August 16, 2023, I observed the center to be a clean and safe condition with no bugs. I looked closely at the cubbies and the drawers and cabinets in the kitchen and did not see any bugs. I interviewed Child B and Child C’s Mothers. Child B and Child C’s Mothers stated that the center is always clean. Child B and Child C’s Mothers stated that they have never seen any bugs at the center. 11
