On 01/21/2022, Sara Meier, program director contacted and left a message for Ms. Gibby, licensing consultant, regarding a child being left unsupervised on 01/20/2022. On 01/21/2022, Ms. Meier, submitted a written incident report to the department. According to the incident report, the "preschool prep was lined up to go to another classroom. Cate Williams and Anna Raptoulis, child care staff members, counted the children before leaving the room. They had 11 children total. One child said they had to go to the bathroom. Ms. Williams took the three children that typically have accidents to the bathroom/changing area adjacent to the classroom. Ms. Raptoulis walked with what was thought to be the eight other children to the ECHO room. Ms. William planned to meet her there once she finished in the bathroom. When Ms. William arrived at the ECHO room, a child asked to go to the bathroom. She took three children to the hallway bathroom. 2 When she came back Ms. Raptoulis left to get something from the classroom. Ms. Williams did a head count and counted 10 children. She thought she may have counted wrong, so she counted again and at that time Ms. Raptoulis walked in holding Child A. She had found him in the classroom crying on the couch with a book." On 01/25/2022, I spoke to Child A's Mother regarding the allegation. She stated that Child A is in the preschool prep class and has been attending since November 2021. The child care staff members are Betina Davis and Ms. Williams. Ms. Davis was not present during the incident. On 01/20/2022, Child A's Mother received a call from Ms. Meier informing her that Child A was left in the classroom when the class left one side of the building to go to the other side of the building. They went to take the children to change clothes and they realized they were missing a child. They went back to the starting room and Child A was found there, curled up on couch with a book. She was told that he was not left for long. Child A returned to the facility the following day. Child A's Mother stated that she has no concerns as the child care center reported the incident. On 01/26/2022, I completed and unannounced inspection at Trombly Kids Club & Kinder Club. I spoke to child care staff members, Ms. Williams, Ms. Raptoulis and program director, Ms. Meier. I observed the preschool prep classroom. There were 12 children, ages 2 to 3 years, with two child care staff members. The attendance 1/2 sheet from 01/20/2022 and Child A's child information card was reviewed. On 1/20/2022, there were 11 children present with two child care staff members. According to Ms. Williams, she is the assistant caregiver in the preschool prep classroom since 2020. Ms. Davis is the lead caregiver, but she was not present on 01/20/2022 and 01/21/2022. On 01/20/2022, there were 11 children present with Ms. Williams and Ms. Raptoulis. They were in the preschool prep room and they started to clean up the room at 11:00 a.m. They were scheduled to go into the ECHO room at 11:00 a.m. and running late. The ECHO room is a classroom set up with different manipulatives for the children to explore. The ECHO room is down the hallway from the preschool prep classroom. Two children usually have bathroom accidents when they leave their classroom. They had the children line up at the classroom door and they counted the children. Ms. Williams handed the Ipad to Ms. Raptoulis and told her it is up to her to wait for them or take the children to the ECHO room. Ms. Williams took three children to the bathroom adjacent in the classroom. When they were finishing at the bathroom, she seen Ms. Raptoulis leaving with the children heading to the ECHO room. As Ms. Williams and the three children left the preschool prep classroom, she could see Ms. Raptoulis and the other children entering the ECHO room, down the hallway. Ms. Williams stated that the preschool prep classroom appeared closed, lights off and undisturbed when she left the room. She did not hear an
