On 08/09/2023, the complainant reported that Child A continues to struggle with toilet training. Part of that problem is the child care staff members are inconsistent with her and allow her to wear Pull-Ups. Child A does not want to stop playing long enough to go to the bathroom and so she urinates in her Pull-Up. On 08/11/2023, I interviewed licensee designee/program director Candace Outman. She stated that Child A’s child care provider, who drops Child A off at the center, was concerned because she thought Child A was wet one day due to urinating, but instead she was actually wet from having played in the water that day. “Child A goes [to the bathroom] all the time. We are working with her parents. We keep reminding her and her Pull-Up stays dry.” On 08/14/2023, I interviewed Child A’s Mother. She reported that she has no concerns with Child A’s toilet training. “We are extremely grateful to the center.” She further reported that Child A has had some problems with toilet training, but they have worked with the center on creating a toilet training plan and she has only been wet on two occasions and both times were from playing in the water.

On 08/09/2023, the complainant reported that the summer camp, which is being run by licensee designee/program director Candace Outman, is not adequately staffed. Some days there are only a few children who are in attendance and other days it is packed with children. The complainant is concerned that the child care staff members appear to be volunteer’s and do not have the required qualifications to be child care staff members. On 08/10/2023, the complainant stated that the summer program ended on 08/09/2023, but had run throughout the month of 4 July on Monday’s, Tuesday’s and Wednesday’s. Child A has been attending the program for a while, and there is a fee for attending. On 08/11/2023, licensee designee/program director Candace Outman reported that she had spoken to her licensing consultant, Don Vita, last year about running a summer program. “When I mentioned it to him, he told me to go ahead and try it.” She said she had sent him a modification request before he went on a leave of absence. When asked to produce a copy of the modification request, she stated, “I don’t have a copy of it”. She reported the summer program was in operation from the middle of June until the end of July; children were dropped off at the center, parents did not remain on-site and there was a fee to attend the program. On 08/14/2023, licensing consultant Don Vita reported that he never received a modification request from Candace Outman to provide summer programming. A search of the department’s records does not show that a modification request was ever received and the terms of the license states “school-year” only programming. On 08/14/2023, Child A’s Mother reported that Child A had attended the summer program at the center. She was dropped off at the center Monday through Wednesday from 9:00 a.m. to 11:30 a.m. As a parent, she was not on-site during this time, and she paid $20 a day for her to attend. The program ran from mid-June until the end of July. There was one week where Child A went from the summer program in the morning to the Bible school program at the church for the afternoon. On 09/15/2023, licensee designee/program director Candace Outman reported the summer program operated and provided care on the following days: June 12, 13, 14, 19, 20, 21, 26, 27 and 28 and July 10, 11, 12, 17, 18, 19, 24, 25, 26. The hours of operation were 9:00 a.m. to 11:30 a.m. Parents were charged $22 a day for one child and $20 a day for children who had a sibling/s attending. The children brought their own water bottle, and a prepackaged snack was provided for them. During the week of June 19th, 20th and 21st, the children in attendance were walked over to the church sanctuary to attend Bible school from 11:30 a.m. to 2:00 p.m. Once the children arrived in the area of the church that was being used for Bible school, they were released to the church personnel. They did not return to the center, and they were picked up by the parents directly from Bible school. She did not record the daily arrival and departure times of the children who attended the summer program. Instead, parents completed a form at the beginning of the summer program indicating the days their child would be in attendance. Not all of the children who signed up for the program were in attendance every day the program was offered. Licensee designee/program director Candace Outman reported that there were 19 children who signed up to attend the summer program. Out of the 19 children the following paperwork was missing or incomplete: three children did not have a child information card on file, 15 cards had missing or incomplete information on their child information card, eight child information cards had not been updated within the last year, four preschool age children did not have a copy of their immunizations or a wavier on file, three preschool children and one 34-month-old child did not have a 5 health appraisal on file, two school-age children did not have a signed and dated statem
