On 8/31/2022, the well that provides water to the child care facility tested positive for E. Coli bacteria. On 9/6/2022 the well tested positive for Total Coliform bacteria. The bacteria and E. Coli levels were in violation of the Safe Water Drinking Act of 1976. The center continued to operate.

On 09/12/2022 I received an email from Environmental Health Sanitarian Jamie Alvesteffer. The email said the water at the child care center had tested positive for bacteria and could not be used until the well was cleaned. On 09/14/2022 I contacted Ms. Alvesteffer regarding the well findings. She indicated that the child care center was in violation of the Safe Drinking Water Act and that they would need to comply with the above information to use the well in the future. She verified the well was taken out of service and is not being utilized. Ms. Alvesteffer has verified the center has posted the necessary requirements for the Health Department and has bottled water on site. On 09/14/2022 I contacted Lisa Fisher who is the Educational Department Specialist for FiveCap. Ms. Fisher stated the well was not being used and they were in the process of completing the requirements of the Health Department. I informed Ms. Fisher that the child care center was in violation of child care rules, and they would need to stop operating until a variance request was approved. The variance request would cover all the necessary rules that were currently in violation because of the contaminated well. Ms. Fisher stated the center would close and would submit the request for a variance. Ms. Fisher stated the child care was operating with only two classrooms at the time with a capacity of 44 enrolled children. She agreed to only operate the two classrooms and not enroll or add any other children until the well had been cleaned, the health department had cleared to resume the water, and this information was sent to the Child Care Licensing Bureau. The child care center stopped operating beginning on 9/15/2022. On 09/16/2022 I received a variance request from Mary Trucks at FiveCap. The variance request indicated that while the well was being cleaned and repaired, the child care center would utilize portable sinks for hand washing and have bottled water on-site for everyone in care. The variance request also indicated the water would be delivered and maintained daily. The portable sinks would be cleaned daily 3 and as needed. This variance request would remain in effect until the well was repaired and the water quality passed testing by the Health Department. On 9/29/2022 The child care center well was tested, and the water was clear from bacteria and E. Coli. On 10/03/2022 I spoke with Environmental Health Sanitarian Jaime Alvesteffer. She verified the well has been repaired and the water was testing within safe limits. Because the water was testing appropriately, the well water could be used, and the portable sinks and bottled water would not be needed anymore. The center could return to normal operation. As a result of the previous contamination, the child care center is required to test the well monthly and provide the results to the Health Department. The child care center would return to normal operation on 10/04/2022. On 10/03/2022 I contacted Lisa Fisher regarding the results of the water testing and my conversation with the health department. I explained the child care could resume regular operation on 10/04/2022. On 10/07/2022 The variance request was processed and approved by the Child Care Licensing Bureau. The child care center's water was found to comply prior to the variance being approved. On 10/12/2022 I completed an on-site inspection at the child care center. I spoke with Program Director Renee Krueger. She showed me the updates to the well and the new water lines. She stated the well water will be tested monthly and the results will be provided to the Child Care Licensing Bureau and the Health Department. On 10/17/2022 I received documentation from Mary Trucks to officially rescind the variance request. On 10/20/2022 I received water testing results from the child care facility. The water test results were clear from bacteria and E. Coli. On 11/03/2022 I completed an exit conference with Mary Trucks. I explained to Ms. Trucks that

The child care center was not able to clean play equipment and furniture because of contaminated water. The well was also not in service while being repaired. The well has since been repaired and sanitized and returned back to normal use.
