    Appropriate care and supervision were not provided for Child A on 12/11/2025. Child A fell from an indoor climbing structure which caused his arm to be broken. Neither Teacher 1 or Program Administrator 1 observed Child A fall. They were made aware of the injury when Child A reported it to them.
