Staff; volunteer (1) All staff and volunteers present at the center shall: (b) Act in a manner that is conducive to the welfare of children.

Based on our findings, investigation, and review - which includes considering the investigation and findings of MDHHS -CCLB determines Lead Teacher is conducive to the welfare of children and is eligible to be associated with a child care organization.

On 1/26/2026, Child A left the center unnoticed, without winter clothing. Thirteen minutes after Child A left, a witness notified the Grandville School District office staff that a child was walking down the sidewalk. About the same time ISP 1 realized Child A was not in the classroom. The temperature at the time of Child A’s elopement was 12 degrees. Child A suffered grade 1 frostbite, due to being in the elements for the extended period without proper winter gear.

(2) All program administrators are responsible for the general management of the center, including the following minimum responsibilities: (b) Administering day-to-day operations, including being available to address parent, child, and staff issues.

On 1/26/2026, Child A eloped from the center and was found off the premises. Program Administrator is also a lead teacher. She was unable to leave her classroom to address the issue and contact Child A’s parent. Due to the child behaviors in Program Administrator’s classroom as lead teacher, she is unable to administer the day to day operations including being available to address parent, child, and staff issues. She is unable to provide support to other classrooms when needed for child behaviors in Lead Teacher’s classroom or the other GSRP classrooms if/when needed.

(1) Pursuant to sections 5n and 5q of the act, MCL 722.115n and 722.115q, before an individual has unsupervised contact with children, the department shall determine the individual's eligibility to be any of the following: (d) An unsupervised volunteer.

ISP1 and Therapeutic Professional had unsupervised access to children. They did not complete a comprehensive background clearance for child care licensing and were not determined eligible to have unsupervised access to child care children

(2) An applicant or licensee shall do all of the following: (a) Ensure that each individual who requires an eligibility determination completes, signs, and submits all of the information required in subrule (5) of this rule, and in subrule (6) of this rule if applicable, on a form prescribed by the department.

Licensee Designees Julie Ann Guenther, Rachel Pond and Melissa M Buurstra did not ensure that ISP1 and Therapeutic Professional completed, signed, and submitted all required information on a form prescribed by child care licensing prior to them having unsupervised access to children.

R 400.8201(2)(d) · R 400.8201(2)(d) Comprehensive background check; fingerprinting. (2) An applicant or licensee shall do all of the following: (d) Establish and activate an account and accurately enroll each individual listed in subrule (1) of this rule in the child care background check system.

Licensee Designees Julie Ann Guenther, Rachel Pond, and Melissa M Buurstra did not enroll ISP1 and Therapeutic Professional into the CCBC system to complete a comprehensive background clearance as required prior to unsupervised access to child care children.
