    R 400.8125 · R 400.8125 1 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 10/8/20 I received a voicemail from program director Katie Avink stating that 911 was called due to an injury sustained to a child at the center, resulting in a trip to the ER. On 10/15/20, I received the corresponding incident report. The incident report stated that around 4:30 pm on 10/8/20, Child A was playing a game called pumpkin squash and was running with the ball. He was not looking where he was going and ran into a pole, resulting in a golf ball size bump in between his eyes. On 10/15/20, I contacted program director Katie Avink. She stated that on 10/8/20, all 4 staff members and children were outdoors playing at the center. One group was playing pumpkin squash when Child A ran into playground equipment. Child A hit his face on a "gymnastics" pole and began crying. In between his eyes a very large bump immediately formed. Ms. Avink stated that she called Child A and B's Mother's cell first, but she didn't answer. Then she called Child A and B's Mother's work phone and was eventually directed to her. She believes this call was made around 4:30pm, just after the injury took place. Child A and B's Mother told Ms. Avink that would not be able to get to the facility for approximately 50 minutes because she didn't have a vehicle to get there. Ms. Avink stated she then called Julie Scott, who is the licensee designee of the facility. Ms. Scott was asked by Ms. Avink if she thought the situation warranted a call to 911. Ms. Avink stated that Ms. Scott wanted her to get a second opinion, so she took Child A to the principal's office. Ms. Avink estimated that she and Child A arrived to the principal's office around 4:40 pm, about 10 minutes after the injury occurred. Ms. Avink stated that at this point, Child A's memory "started slipping". She stated that Child A could not remember the day or month and he was beginning to act sleepy. The principal, Ms. Shoup advised Ms. Avink to call 911. When asked what Ms. Avink would have done had the principal not been present, she stated that Ms. Scott would have driven to the center to take a look at the injury. Ms. Avink called 911 and they arrived at approximately 4:50 pm. The EMT's looked Child A over and stated they needed the mother's permission to transport him to the ER. Ms. Avink stated that she called Child A and B's Mother again and she gave permission for him to be transported. Ms. Avink then followed the ambulance with Child A's younger brother, Child B (male, age 8) in her vehicle so that she could meet Child A and B's Mother at the ER. Ms. Avink stated she was given verbal permission to transport Child B in her vehicle from his mother. Ms. Avink estimated that Child A left in an ambulance around 5pm. She noted that when Child A was 3 transported, the ambulance lights and sirens were not on. Ms. Avink then waited in the waiting room with Child B at Helen Devos Children's Hospital as he was not permitted to go back due to the COVID-19 pandemic. She stated she does not know the official diagnosis or treatment as she has not spoken to Child A's Mother since 12th the incident. Child A came back into care with the center on Monday, October On 10/18/2020 I received a one-page copy (page 1 of 11) of Child A's "after visit summary" from his ER visit on 10/8/20. Under "reason for visit", it states, "head injury" and under "diagnoses" it lists "traumatic injury of head, initial encounter" and "Contusion of head, unspecified part of the head, initial encounter". The document indicates that Child A was to follow up with his primary care physician in two days. On 10/20/2020 I completed an unannounced on-site inspection at the center. I interviewed Ms. Avink and asked her about the discharge summary. She stated that Child A and B's Mother only gave that one page and never gave any information on Child A's diagnosis, or after care plan. The only thing Child A's Mother said was that his doctor instructed to keep a close eye on Child A to make sure he didn't have a concussion. Child A onl

    The center program director, Katherine Avink, did not implement the serious accident/injury policy. Under "site supervisor" it states that 911 should be called immediately if warranted, however, Ms. Avink did not contact 911 immediately as was the situation called for. The accident took place at approximately 4:30 pm and 911 was called at approximately 4:45 pm. 1. First, Ms. Avink called Child A's Mother, then she called licensee designee, Julie Scott to get directions on what to do. 2. Second, Ms. Avink took Child A down to the principal's office to further get her opinion on whether to call 911 or not. ........................................................................ _. .......
