On 3/29/2021 1 received a complaint statin that Ms. Williams took seven children to a local hotel pool. Child P , age ) left Ms. Williams' group to swim in a separate pool with other children who were not in the care of Ms. Williams. Child P was observed standing one moment and a cou le minutes later I was floating on ■ back, with head under the water, lips were blue, and was not moving or breathing. ~i d P was pulled out of the pool, CPR was performed for three minutes, and ■ began breathinq n. 911 was subsequentolyn Red and the child was transported to On 3/29/21 I received an additional complaint ame allegation. CI 5 On 3/30/2021 and I made a face-to-face contact with aRness at the Holiday Inn where the incident occurred. We inspected the pool area. The pool area consists of two pools and a splash pad. The pool in the Southeast corner was a "kiddie" pool with water measurements reading 8 inches. In the middle of the kiddie pool, there is a small climber and slide and small buckets that dump water into the pool below. This pool was next to the splash pad which had no standing water. In front of the kiddie pool and the splash pad was a larger pool with water measurements of 3 feet 6 inches, which is the depth for that entire pool. This larger pool was Northwest and had a connected smaller round shaped "swirly" pool connected that was also 3 feet 6 inches high. The "swirly' pool had water that was actively moving in a circular motion. This part of the pool is where Child P was found unresponsive by witness and I took photos of the pool area. took a photo from thear end of the pool facing the kiddy pool and sent a photo o witness , asking ■ to explain where the child was found in the pool and where caregiver was standing by the kiddie pool. noted that at the end of thepicture w ere it states "no diving" is where the child was found floating in the pool. noted in the photo where the mini purple bucket is hanging is where the caregiver was standing by the kiddie pool. It should be noted that from where Ms. Williams was reported to be standing, on the Southeast side of the room near the kiddie pool, vision to the larger pool was partial obstructed by wooden slats on a fence that is around the splash pad. On 3/30/2021 and I interviewed Holiday Inn hotel who p resent at the hotel on 3/27/21. SheMsastated that s. Williams rented one room at the hotel for one night, 3/27121, and reported to have one adult and no children. However, she indicated it was clear there were multiple children with Ms. Williams, as children were `popping up everywhere." She indicated she was told it was an Easter or birthday party, though she did not observe this party taking place as she did not enter the pool room before the incident occurred. She indicated that Ms. Williams was observed to be caring for a least 5 children. stated that after the incident occurred, a or -year-old child approached the front desk by and asked her wha oor mother was at because was scared to go by the pool after what had taken p ace. indiea ed that she opened one of the hotel doors and a woman, who seemed to be the child's Mother came into the hotel with food from McDonalds and went up to a hotel room with the child. This woman was later observed leaving the hotel by herself. She stated that approximately three different sets of arents arrived after the incident occurred asking what had happened. stated she was told by other _ hotel staff that she could only tell the parents that "an incident occurred, and everyone is ok", stated that all of parents left the hotel to her knowledge and did not leave their children. On 3/30/21 and I completed an unannounced on-site inspection at the child care home. Six children were reported to be present at the time of the inspection. Child I age,), Child D a e , Child M age , Child F age ), Child 0 age ), and Child H ag gowere ,t he children present at the time of the inspection. Ms. Williams stated that on 3/27/2021 she was not doing "daycare"
On 3/27/2021, Melanie Williams went to a hotel pool with 7 children. While in Ms. Williams' care, Child P , ages) was found in the hotel pool floating on ■ head was beneath the water, was not moving, and l lips were bbalucek.,Wild P was given CPR by two wi nesses who were at the poo and was successfully revived. Ms. Williams was made aware of this by an adult at the pool. NOTE: Throughout this report, the incident involving Child P is referred to as a near drowning. Pubmed.ndbi.nlm.nih.gov defines `near drowning" as immediate survival after asphyxia due to submersion.

When ■ was pulled out of the water Child P was not breathing and did not have a pulse. • Ms. Williams brow ht seven children, five who were younger than age years to a pool without any floatation devices or other safety precautions in place. • Ms. Williams did not notice Child P was missing for approximately 8 minutes and not 30 seconds as she estimated. • Ms. Williams took no precautions to adequately supervise seven children on 3/27/21; she did not ensure children could swim, she did not mandate floatation devices to be worn, nor did she have additional adults present to supervise.

N Child P was able to enter a pool that was deeper than was tall unknown to Ms. Williams, which led to Child P becoming totally submerged in the water. was found unresponsive, not breathing and with no pulse by witnesses, who happen to be professional medical staff. According to the hotel video there were four minutes between the time Child P went into the pool and when was pulled out unresponsive. ■ • Ms. Williams stated that Child P was still wearing one arm floatie when was receiving CPR and that prior to the near drowning, was wearing two arm floaties. According to the hotel video, Child P was not wearing an 0 arm floatie when was pulled from the pool and CPR was administered. • Ms. Williams stated that prior to the near drowning, she had not observed any of the seven children, including Child P, go over to the big pool which was "off-limits"; however, according to the , Ms. Williams told the several of the children recognized other children in the northwest pool area and kept trying to leave the kiddie pool to go to the other pool. She had to keep bringing them back. • Ms. Williams stated she informed the other parents of children who were at the hotel over text or phone; however, Child I's Guardian indicated she did not know about the near drowning incident. • Ms. Williams stated her CPR certification was current: however, upon reviewing the CPR card, it was discovered that her certification expired in November 2020. On 2124/21 1 received a complaint stating that on 2/22/2021, while Child A's Mother was picking Child A up from Ms. Williams child care home around 7:10 PM, she got into a verbal argument with Ms. Williams for being 10 minutes late. As Child A's Mother and Child A were exiting the child care home, the Ms. Williams followed them out to their vehicle with a in her hand while stating she will Child A's Mother. Child A was reported to be sitting on mother's hip during this altercation. When Child A's Mother reached her vehicle, s e grabbed her phone and recorded Ms. Williams walking away and into the home with a in hand. The ~ were then called by Child A's Mother. Both On 2/25/2021, 1 received the video of the alle r On 02/25.2021, . and l completed an unannounced on sst a tnspec ion a .~~e Fc c"arecme at approximately 2pm. No one answered the door. Children could be heard in the home, talking. playing, and running around. I called to speak with Ms. Williams to let her know we were at the C] residence as there was no answer when knocking. Ms. Williams stated that she was not currently at the home and that she was in the process of picking up a child from school. Ms. Williams explained that her "helper" was at the home, and that she would call her and tell her to answer the door. Her "helper" let us into the home to wait for Ms. Williams to arrive. Ms. Williams didn't arrive until 3:15pm. When asked about the incident with Child A's Mother, Ms. Williams stated that Child A's Mother was late picking up her child by about 45 minutes. She indicated that Child A's Mother did not communicate with her to tell her that she was going to be late or that she was on the way to pick up her child. She stated that when Child A's Mother got to the home, Child A's Mother was irritated and short with her. Ms. Williams stated she asked Child A's Mother to talk to her if she were having a bad day and they could work something out with payment. Ms. Williams stated that Child A's Mother got upset and told her she wasn't going to pay extra for being late. She reported that Child A's Mother "got in her face." Ms. Williams asked her to leave and to not "disrespect her in her house." Ms. Williams indicated that Child A's Mother then started throwing paper, tearing items off the wall, turned over a coat rack while trying to get her child's coat, while yelling and using profane language toward her. Ms. Williams explained that Child A's Mother "snatched" and "tore up" the attendance record the day of the incident, and she does not have it to show who attended the child care for that day. She also indicated that Child A's Mother put her finger in Ms. Williams face and then left the house. Ms. Williams explained that as she closed the door to lock it behind her, Child A's Mother then kicked her glass storm door and walked to her car. She noted that Child A's Mother had Child A in her arms while she was leaving the house and while she was kicking the door. Ms. Williams stated she then followed Child A's Mother outside to tell her that she was Ms. Williams denied that she was the aggressor of the incident and at she had a R Ms. Williams stated that Child A's Mother threatened to make up "lies about er aycare" so the police would come and that people would not use her services. Also, she stated that Child A's Mother threatened to come back to the home with her family and beat her up. Ms. Williams reported that Child A's Mother left the home and the showed up about 10 minutes later. She reported the= got her statement and left. Ms. Williams explained that Child B's Mother was present at the time of the incident as she was there to pick up Child B (age ), in addition to two other child care children, Child C , age ) and Child , age ). She informed us that when Child A's Mother starte getting loud" she asked Chi d B's Mother to take the children to another room and indicated they did not see anything else. During my on-site inspection on 2/25/21, I observed change and wipe the bottoms of two children using potty seats and pull-ups. This was done in two places in the living room while the children were standing up and then when was sitting down on the couch, as she pulled them on her lap. There was not a designed area for changing soiled diapers, it was not used exclusively for soiled diapers, there was not a nonabsorbent, easily sanitized surface between the child and the surface, and the surface could not be cleaned and sanitized after each use.
