During rest time, CCSM 1 would force children would stay on their cot by putting her arm around them so they could not get off. An incident occurred when CCSM 1 wanted a child's attention, she threw a rubber ball at the child hitting them on the back of their head.

I showed Ms. Cossar that photos that I received of Child V. I interviewed Ms. Morgan about Child V's injury. Ms. Morgan said that she thought there were four children in her care when Child V was injured. She said that she was standing by the block shelf, saw Child V running, and called to her to tell her to stop running. She looked away and when she looked back, she saw Child V down on the floor, then jump up and was bleeding from her mouth. Ms. Morgan explained that she was standing with her back to the wall and Child V was to her left. She said that she thinks Child V tripped and hit her mouth on the book shelf when she fell down. Ms. Morgan said she took Child V into the bathroom, had her stand at the sink so the blood dripped into the sink, she put on gloves and then looked in Child V's mouth to see where the blood was coming from. She said that she lifted Child V's lip but could not see any spot where the blood was coming from. She stated that she cleaned Child V up and then her had sit at the table. Ms. Cossar then came in the classroom. Ms. Morgan said that she then called Child V and W's Mother, told her that Child V had an accident and hit her mouth. She said that she told Child V and W's Mother that she did not notice that any teeth were chipped but could not see exactly where the blood was coming from. Ms. Morgan said that she wanted to let Child V and W's Mother know that there were blood drippings on Child V's pants. Ms. Morgan said that Child V and W's Mother told her that she was not surprised because Child V falls a lot. Ms. Morgan explained that she took the children to the gym and Child V did not want to play and just wanted to hold her blanket. When they returned to the classroom, Child V did not want to play and laid on the floor, so she got her cot out for her. Ms. Morgan said that Child V did not eat lunch and did not continue to cry. Ms. Morgan said that she worked until 2 PM that day and CCSM Melissa Swanson worked in the classroom when she left. Ms. Morgan said that she looked at Child V's mouth again when she was leaving and saw that her lips were swollen. I asked Ms. Morgan if she had given Child V any ice or any thing else to help with the pain and swelling; she said that she had not given her anything. Ms. Morgan reported that she looked at Child V's teeth and they looked normal to her. She said that she wondered if the swelling made the teeth change. Ms. Morgan said that she did not see any injury to the skin around Child V's mouth. I showed Ms. Morgan the pictures that Child V and W's Mother had sent me. Ms. Morgan denied that Child V's mouth looked like they did in the photo. Ms. Morgan said that she talked to Ms. Swanson about Child V's injury and wrote up an incident report before she left. On 03/23/2021, I spoke with CCSM Melissa Swanson via telephone. Ms. Swanson reported that she was not present when Child V was injured. She said that Ms. Morgan told her that Child V tripped, fell and hit her mouth on the shelf and that she had contacted Child V and W's Mother. On 02/10/2021, I spoke with Child V and W's Mother about Child W and the allegation. Child V and W's Mother explained that a former employee of the center had a relative who toured the center and saw the incident. Child V and W's Mother said that she was told that her son was seen sucking on Ms. McGovern's fingers. Child V and W's Mother reported that she confronted Ms. McGovern about it when she saw her and Ms. McGovern's response to her was "I can assure you my hands are clean." On 02/19/2021, I interviewed Ms. Cossar about the allegation involving Child W. Ms. Cossar said that she and the licensee designee, Carla Moore, met with Child V and W's Mother and acknowledged that it was not appropriate. Ms. Cossar said that they also met with Ms. McGovern and talked to her about not getting that comfortable with the children, about him not being her own child and that she should have gotten him get a teether instead. 5 I interviewed Ms. McGovern about the incident with Child W. Ms. McGovern explained that Child W was teething that day and she had just washed her hands and sat down on the floor when he crawled over to her. He started chewing on her thumb and she did not stop him. She said that she knows now that she should have stopped him, and she will give him toys or a teether in the future. Ms. McGovern talked about the conversation she had with Ms. Cossar and after they talked about the situation, she understands why Child V and W's Mother was upset.

On 02/10/2021, Child V and W's Mother explained that Child W did not have a diaper rash but did have a "wet rash" that looked like he had been sitting in sweaty clothes. She described that the rash was all over his elbows, chest, thighs and genital area. She said that she took Child W to the doctor and was given a cream for it. I asked her to provide me with documentation from the doctor visit but had not received it at the writing of this report. Child V and W's Mother went on to talk about how her children get sick every couple of weeks. She said that she would drop them off in the morning and they would have no fever at check in and then halfway through the day, her son would have a fever at the center. She said that he would be congested, she would "flush his system" and he would be fine for 48 hours. She would bring him back to the center and he would another runny nose. She explained that she thinks that the center was sending him home when he was not sick because they did not want to take care of him. I explained to Child V and W's Mother that this concern did not allege any rule violations. I spoke with Ms. Cossar about concerns that Child V and W's Mother had shared. Ms. Cossar said that the center changes diapers and clothing when wet and the diaper changes are documented in their parent communication app. Ms. Cossar said that if she had a specific date, she could provide the times Child W was changed on that date. Ms. Cossar said that children are being sent home from care more than normal due to their COVID-19 protocol but denied that they sent Child W home when he did not have any symptoms. Ms. McGovern reported that she was Child W's primary caregiver and that he had rashes in the past. She explained that the children are changed when wet and they are checked every two hours. She went on to describe that if the children's clothing was wet for any reason, they would change them into dry clothes. Ms. McGovern recalled talking to Child V and W's Mother about that rash and that Child V and W's Mother told her that it was eczema heat rash. Child V and W's Mother gave them lotion to put in his skin, but they rarely had to use it.

R 400.8155 · R 400.8155 Child accidents and incidents; child and staff illness. (5) A center shall have a written policy detailing when children, staff, and volunteers will be excluded from the center due to illness.
On 02/03/2021, I completed an unannounced on-site inspection to initiate the special investigation. The facility screened me by asking me COVID-19 questions and my temperature was taken before I could enter the building. I interviewed center director Michelle Cossar about the allegations. Ms. Cossar denied that child care staff members (CCSM) work when they are ill. She explained that CCSMs are sent home if they get sick while working and they do not come in to 2 work if they are sick before work. She has asked CCSMs to get tested for COVID-19 or to obtain a doctor's note if they have any symptoms of COVID-19. The center has a policy that requires that staff stay home or will be sent home if they have any of the following symptoms and children cannot be present if they have: Fever of 100.4 degrees or above, shortness of breath, uncontrolled cough or two of the following: diarrhea, loss of taste or smell, muscle aches without another explanation, severe headache, sore throat, vomiting, chills or if they are in quarantine due to exposure to an individual with a confirmed case of COVID-19. Ms. Cossar explained that two preschool classrooms are currently closed at three CCSMs are quarantined after a child enrolled in one of the classrooms had been present and tested positive for COVID-19. I interviewed CCSM Melissa Swanson who denied that they are expected to work when ill. She reported that she gets checked at the door before she comes in the building and was sent home once because she had a fever when they checked her. I interviewed CCSMs Chelsea Haske, Karissa Mulero, and Ashley VanStee who reported that CCSMs do not work when sick and had no concerns about being required to work when sick. I interviewed CCSM Jasmenn Hudson who reported that she was sick last week (week of January 25) with congestion and a runny nose. She said she sent a text message to Ms. Cossar that she was going to be late and was told to wait a while to come to work until her medication kicked in. She worked on Tuesday and Wednesday and then got a COVID-19 test on Wednesday. I spoke with Ms. Cossar and UMCH Director Carla Moore at the end of my interviews. We discussed Ms. Hudson's claim about having to work when ill. Ms. Cossar reported that Ms. Hudson was off January 19-22 and completed a COVID-19 screen during that time. Ms. Cossar had time sheets verifying when Ms. Hudson worked and an email from Ms. Hudson with her COVID-19 test results that was from 01/23/2021. Ms. Cossar did not have any text messages from Ms. Hudson pertaining to her coming to work sick. On 02/19/2021, I spoke with Ms. Hudson to clarify what she had previously reported. I told her that Ms. Cossar had provided information that contradicted what she had reported. Ms. Hudson said that she must have gotten mixed up. She was not able to find text messages and then said that she had not texted but had spoken on the phone with Ms. Cossar. I interviewed CCSMs Carol Morgan who said she did not know anything about the allegation. She explained that when you are sick, you just call in. She has worked for the center for 19 years and that has been the process. I interviewed CCSM Samantha McGovern who reported that CCSMs do not work when sick and it is the opposite. She explained that she thinks that the center does 3 beyond The Centers for Disease Control and Prevention recommendations regarding COVID-19. I interviewed CCSM Jailene Mateo who reported that she thinks there are "jokes behind you" if you call in sick. She said that they can call in sick. The other CCSMs talk about you if you do. She has not felt that Ms. Cossar is making people work when they are sick. No one had any information about children being present in the center when they should have been quarantining.

Ms. Cossar explained that CCSMs are responsible for cleaning items that children have had in their mouth and for cleaning the classrooms during nap time. They have custodians that clean at night and wipe high contact areas like door knobs and bathrooms during the day. Ms. Swanson, Ms. Haske, Ms. Mulero, Ms. VanStee, Ms. Morgan, Ms. McGovern and Ms. Mateo all reported having no concerns with the cleanliness of the facility. Ms. Hudson reported that the classrooms do their own cleaning, but she does not think it is thorough enough.

Ms. Cossar reported that the building has had cockroaches in the past. The center has a contract with Griffin Pest Control for treatment. She said that the center has a log book that staff are supposed to document any sighting of cockroaches or other pests in. No staff have reported seeing any recently. Ms. Swanson reported that she has seen cockroaches, but they have an exterminator that comes. She last saw one a couple months ago. She estimated that she has seen maybe three in total in the past two years. Ms. Haske reported that she has not seen a live cockroach in a long time. Ms. Mulero reported that she was told to log any cockroach sightings and she has. She has not seen a live cockroach in a few months. She has seen dead ones in the cupboards. Ms. Hudson reported that she has not seen any cockroaches at the center. Ms. VanStee reported she has not seen any cockroaches but has seen ants. The pest control company treated for the ants. Ms. Morgan has not seen any cockroaches but will report the sightings if she does. Ms. McGovern said that the center has had cockroaches in the past, but they have a pest control company that comes, and she has not seen any in over a year. She works in the 100 wing and the 200 wing was where she saw them. She explained that if you see a cockroach, you are supposed to write down what type of insect you saw in the log book and where you saw it for Griffin Pest Control. Ms. Mateo reported that she has not seen any cockroaches. I viewed the Griffin Pest Control log book and was able to see that the center has been logging pest sighting and that Griffin Pest Control does treat the facility. I also walked through the licensed portions of the building and did not see any cockroaches or any maintenance issues that would promote harborage. 5
