On 05/23/2023, allegations were received that the home was infested with mice. On 05/26/2023, allegations were received that the house was unsanitary, and all the children had pink eye, strep, and hand foot and mouth 4 disease. On 05/27/2023, allegations were received that the house is "overall disgusting." On 6/20/2023, allegations were received that the home is infested with mice, roaches, and bed bugs. On 05/25/2023, I completed an unannounced on-site inspection and interviewed licensee Datasha Chapman. Ms. Chapman was informed of the allegations. She stated that the home was "infested with bed bugs, roaches, rats, mice and people" when she moved into the home in 2019. She has cleaned the house up and the only thing remaining is mice. She caught a mouse a few days ago and has traps set around the house. She has a relative who works for Orkin who comes every two weeks to address the issue. She said that she has mice in the attic of the home. Ms. Chapman walked me through the home so I could observe the conditions. In the basement level of the home, the playroom was not maintained in a clean condition. The playroom floor had a lot of debris on it and the flooring was dirty, cracked and parts of it were missing. Adult Household Member 4 (AHM 4) swept up a pile of the debris while I was speaking with Ms. Chapman about the condition. The stairs and the floor in the kitchen were dirty. There was clutter piled up on the front porch, in the playroom, living room, kitchen and dining room. The clutter was stacks of totes, plastic bins, toys, plastic bags, empty boxes, and clothing. The walls, door and stairway to the basement were dirty. The walls in the playroom were dirty. The door frame on the porch needed repair. The entry door into the home was dirty. There was a hole in the ceiling of the living room. Ms. Chapman reported it was due to a water leak that was repaired but that she had not gotten around repairing the ceiling yet. I took photos of the home to document the condition. I did not see any infestations during my walk through of the home. Additionally, I did not see any mouse traps; therefore, there were not any within children's reach. Ms. Chapman reported that she will clean up the clutter and she does not plan to enroll any children until after her home has been cleaned and decluttered. Adult Household Member 3 (AHM 3) and AHM 4 denied that there is any infestation. On 5/26/2023, I completed an unannounced on-site inspection and interviewed CCSM who stated she had no concerns. I interviewed AHM 4 who said that she has seen mice in the kitchen but there is poison set out. On 6/21/2023, I spoke with Witness who confirmed that he works for Orkin and has treated the home. He said that he had treated the home for roaches and bed bugs in the past. He continues to treat the home for mice. He said that there was not currently an infestation. He was last there two weeks ago. He has helped out by sealing gaps and has recommended that new insulation be added. 5

On 5/23/2023, allegations were received that drug dealers and sex offenders live in the home. On 5/27/2023, allegations were received that Ms. Chapman lies to child care licensing about who lives in the home. On 5/28/2023, allegations were received that Adult Household Member 5 (AHM 5) lives in the home. On 6/20/2023, allegations were received that Adult Household Member 6 (AHM 6) resides in the home and is making and selling switch guns out of the home. On 05/25/2023, Ms. Chapman reported that Adult Household Member 1 (AHM 1), Adult Household Member 2 (AHM 2) and Adult Household Member 7 (AHM 7) were the only people who resided in the home with her. AHM 1 and AHM 2 are the only ones who have been fingerprinted and were known to child care licensing to reside in the home. Ms. Chapman reported that AHM 7 is disabled and is unable to straighten his fingers, so she was not sure if he was able to be fingerprinted. I told Ms. Chapman that alternative arrangements could be made and that she needed to contact the fingerprint department regarding any accommodations. Ms. Chapman agreed to contact the Child Care Background Check department that day to get AHM 7 scheduled for an appointment. Ms. Chapman reported that AHM 3, AHM 4, and Child A and Child B were currently in the home and were visiting from out of state. She reported that AHM 3 does have a criminal sexual conduct charge and is a registered sex offender. She said that he is homeless and goes between her house and AHM 4's mother's house in Illinois. She stated that he did not live at the home. Ms. Chapman reported that the home belonged to AHM 1, and the family considers it their home, so she struggles with keeping family out of the house. During the inspection when Ms. Chapman was walking me through the house, I saw three adults. Ms. Chapman reported that one was CCSM, one was her Aunt, and one was AHM 2. I observed AHM 2 in an upstairs bedroom laying in a bed with a child. Ms. Chapman reported that AHM 2 was taking care of his nephew. I spoke with AHM 3 who reported that he lives at a different address in Grand Rapids with his aunt. AHM 4 is not allowed to be at his residence, and AHM 3 has to leave the residence during the day when his aunt goes to work, so he comes to Ms. Chapman's house to see AHM 4, Child A and Child B. He said that he stays at a friend's in Pontiac, AHM 4's mother's house and Ms. Chapman's. He denied that he lives at Ms. Chapman's residence. On 05/25/2023, I spoke with who all reported that AHM 3 was at Ms. Chapman's home in Ma On 05/26/2023, I asked Ms. Chapman about how long AHM 3 has been staying at the home. She reported that he has been there since 1989. He had a house fire in April 2022 and has been staying with his aunt, but she does not let his family stay there so he moved around. She said that he, AHM 4, Child A and Child B have been at her home for the past four days after they arrived Monday morning. Ms. Chapman said, "I can't help that everyone comes here, its granny house." She went on to explain that she has put $50,000 into getting the home cleaned up and repaired and her family cannot accept that it is her house now. Ms. Chapman reported that there was previously an injunctive order for the home due to police involvement. She said that AHM 7 has been residing in the home since April 2023. She did not make a fingerprinting appointment for him yesterday. Ms. Chapman said that her aunt who visits the home is "on drugs and a thief." I interviewed CCSM who reported that AHM 3 is at the house for a few days and then goes out of town for a few weeks and then comes back. He has currently been at the house for the past three or four days. I interviewed AHM 4 who reported that she has been at the house on and off since her house fire in 2022. She stays with her mom in St. Louis because she cannot be with AHM 3 at his aunt's house. She said that she had been at the home for "maybe a week" after arriving there last week. On 5/26/2023, allegations were received that due to Ms. Chapman's medical and life circumstances, she is not physically and emotionally capable of caring for children. On 06/01/2023, I observed Ms. Chapman moving around her home. I did not observe that she had any physical impairment. I asked Ms. Chapman about the allegation, and she denied that it was true. She has health conditions, but they do not impair her physically or emotionally. Review of the licensing file showed that Ms. Chapman provided a medical clearance request dated 08/05/2021 that indicated that "No physical/mental condition or health problem exists that would limit the ability to provide independent care of children (birth to age 17) in a child care setting."

I observed that there was an adequate supply of food in the home. The complaint was anonymous, so I was unable to obtain any additional information. On 05/28/2023 allegations were received that people fight, smoke, and drink in front of the child care children. The police have come to the house. On 05/25/2023, Ms. Chapman reported that she has a restraining order against two relatives that do not currently live at the home and that one of them assaulted someone at her home a few weeks ago. Ms. Chapman reported that she smokes marijuana but does not smoke during child care hours or when children are present. I asked her where she was storing it, she said in a closet, but she has a safe she can put it in. She agreed that she will keep it in the safe. Ms. Chapman did not have a no smoking sign posted. She explained that she had one up on the door and her relatives will take it down. On 05/26/2023, I observed a large jar of marijuana in the living room. I pointed it out to Ms. Chapman and reminded her that she had agreed to store her marijuana in her safe. AHM 4 reported that she has never seen anyone use drugs other than marijuana and that only happens when there are no children there. On 06/01/2023, Ms. Chapman denied the allegation that people drink, smoke, and fight in front of the children. She said that the police have been to the home three times since her license was issued on 02/01/2023. She said that the first two times were for AHM 5, and the third time was when a relative who does not live there threatened her. Ms. Chapman reported that AHM 3 "snorts cocaine" and AHM 4 "does too." She said that AHM 3 and AHM 4 are "very violent" with each other, and 16 she has called the police on them before too. She said that AHM 4 cut the tires of the car in the driveway a couple weeks ago on a Sunday. Ms. Chapman also reported that another relative, who was in the home during my inspection on 05/25/2023 "does crack." On 06/06/2023, l reviewed the court files for the orders dated 10/18/2021. The petition detailed that Ms. Chapman was assaulted by her relatives and was hit in the head multiple times and maced with pepper spray. Ms. Chapman wrote in the petition, `°I am a child care provider and this incident took place in front of children I was in care of." I reviewed the bureau's records and found that Ms. Chapman did not have a child care license in 2021. She applied for a license in October 2020; but her enrollment was closed by the bureau in 2021 due to lack of response. I reviewed the injunctive order issued for 1015 Benjamin Ave in 2012. The petition details that AMH 3 and AHM 7 and 2 other relatives were prohibited from the residence and premises. The State of Michigan and the were the plaintiffs who were seeking abate a public nuisance due to 1015 Benjamin Ave having a "general reputation as a place where people sell, purchase, and/or use controlled substances. It details that AHM 3 was arrested for possession of marijuana, maintaining a drug house, and being a second felony offender. The permanent injunction was dissolved on 09/23/2019. On 06/20/2023, AHM 4 reported that the police came to the house because Ms. Chapman called to report on her. She said that there were billing records on the computer, and she agreed to send them to me that day. Ms. Chapman said that she had child information cards but could not locate them at that moment. On 05/26/2023, Ms. Chapman stated "I never hit children." I asked to see a copy of her discipline policy. She said that she sent her discipline policy to her licensing consultant when her license was opened. She uses PBIS (Positive Behavior Interventions and Supports), restorative practices and redirection as discipline. CCSM reported that no physical discipline is used. She takes toys away and separates children as discipline. AHM 4 reported she has never seen Ms. Chapman hit any children. She said that time out is used by Ms. Chapman and her children "love" Ms. Chapman. On 06/01/2023, and I spoke with Ms. Chapman about the allegation of children being burned with cigarettes. Ms. Chapman reported that she does not smoke cigarettes, she smokes marijuana - pre-rolls or blunts. She said that AHM 7 and AHM 5 are the only ones that smoke cigarettes. They do not smoke in front of the children. Ms. Chapman said that she did not recall any child getting burned by a cigarette. CCSM stated that Child E spilled hot water from her noodles on her a few weeks ago. They put burn cream on it, but she did not have a mark, Child E just said that it stung. On 06/02/2023, I received attendance records and child information cards from Ms. Chapman via email. On 06/20/2022, AHM 4 reported that Ms. Chapman cared for Child A and Child B the previous week while she and AHM 3 were at work. She said that her children told her that she made them sit on the couch all day. AHM 3 was asked about the burn allegation. He reported that their family makes false allegations against each other and he thinks that was made up. He said that when they "get mad at each other, we do dirty things to each other." On 06/21/2023, Ms. Chapman denied the allegations that she is mean, "whoops" the children, and makes them sit on the couch. 13
