On 05/11/2023, I received an incident report submitted to child care licensing by the center indicating that Child A was running on the playground and fell going up the stairs of the playground structure cutting his left cheek on 05/03/2023. On 05/16/2023, I completed an onsite inspection. I interviewed program director Cheryl Jurek, Child Care Staff Member 1 (CCSM 1), Child Care Staff Member 2 (CCSM 2), and Child Care Staff Member 3 (CCSM 3). Cheryl Jurek stated that Child A's Mother did not voice any concerns about the center not calling for emergency 2 medical treatment. She stated that Child A's Mother was called immediately and arrived at the center within 10 minutes. Child A's Mother transported Child A to the hospital. CCSM 2 accompanied Child A and Child A's Mother to the hospital. CCSM 1 stated that she immediately called the office when Child A fell and cut his cheek on the stairs of the playground structure. She promptly took Child A to Cheryl Jurek in the office. She described the injury to Child A's face as being a "deep cut." CCSM 2 stated that she was in a classroom near the office when Cheryl Jurek called to her for assistance when Child A fell and cut his cheek on the playground. She witnessed Cheryl Jurek immediately call Child A's Mother. She accompanied Child A and Child A's Mother to the hospital. She described Child A as calm and referred to him as a "rock star" regarding his response to the situation. Child A's Mother did not mention any concern about the center not calling for emergency medical treatment. CCSM 3 stated that she was in the grassy area of the playground when Child A fell and cut his cheek. She and CCSM 1 immediately looked at Child A's face. CCSM 1 immediately applied pressure to the wound and rushed Child A inside to the office. I inspected the area and playground structure Child A fell on. I did not notice any tripping hazards or defects to the playground structure. I reviewed the center's parent notification policy for injuries. The policy states that a telephone call will be made to the parents if an injury is of a serious nature. On 07/26/2023, I interviewed Child A's Mother by telephone. Child A's Mother statec that she had no concerns with the way the center handled the incident. She was able to get to the center in less than four minutes. She is confident that the center would have called for emergency medical treatment had they not been able to reach her or if she was unable to get there so quickly. She loves the center, and she trusts them. Child A has recovered from his injury and is doing well.

On 03/15/2023, I received a written complaint alleging that Child Care Staff Member 3 (CCSM 3) hit Child A female) with a shoe. A loud smack was heard. Two days later, CCSM 3 tied Meijer bags on Child A's feet. Child A had to play on the playground with the bags on her feet. CCSM 3 refused to take the bags off Child A's feet. The complaint also alleged that Child Care Staff Member 4 (CCSM 4) witnessed CCSM 3 hit Child A with a shoe. Also, program director Cheryl Jurek was made aware of the shoe incident and the Meijer bags incident. 3 Additionally, CCSM 3 hit Child C male) in the summer of 2021. Child C's parents disenrolled him after CCSM squeezed his arm and left marks. The complaint alleged that the abuse of Child C was reported to the center, but it was swept under the rug. Cheryl Jurek advised that the center does not have cameras in the classrooms. stated that he spoke to Child B's Mother on 03/16/2023. Child Bs Mother told him that she has no concerns with the center. Child B's Mother denied seeing any concerning marks or bruises on Child B ,female) that may have been received at the center. _ stated that he spoke to Child A's Mother on 0311612023. Child A's Mother told him that Child A no longer attends the center. She told him that she had concerns with the center and that she shared her concerns with program director Cheryl Jurek and licensee designee Kellie Olson, but they failed to address her concerns. Wo stated that he spoke with Child C's Mother on 03/19/2023. Child C's er told him that Child C left the center in July 2022 and is doing well at his new center. Child are Staff Member 12 (CCSM 12) and licensee designee Kellie Olson. CCSM 12 stated that CCC 3's employment ended due to her tone and not being a good fit for the center. and I also interviewed Child Care Staff Member 1 (CCSM 1), Child Care Pa ember 11 (CCSM 11), Child Care Staff Member 10 (CCSM 10), Child Care Staff Member 8 (CCSM 8), and Child Care Staff Member 13 (CCSM 13). CCSM 8 stated that she has not had an issue working at the center. She described her coworkers as "sweet" and her supervisors as "friendly." She began working at the center August 2022. She denied having knowledge of the allegation against CCSM 3. CCSM 10 denied having knowledge of the allegation against CCSM 3. CCSM 11 stated that CCSM 3 frequently raised her voice with the children. She reported her concerns to the administrators. She was told by the administrators that they have been made aware and they were talking to CCSM 3 about it. CCSM 13 stated that CCSM 3 elevated her voice with the children, but she had no knowledge of CCSM 3 handling children inappropriately. She called the office to request additional help for CCSM 3 on a few occasions, particularly at rest time. CCSM 1 had no knowledge of the allegation against CCSM 3. She never worked with CCSM 3. On 03/22/2023, and I completed an on-site inspection. CCSM 12 and CCSM 13 were in the office and greeted us upon our arrival. Wa and I also interviewed Child Care Staff Member 9 (CCSM 9), Child Care ember 7 (CCSM 7), and Child Care Staff Member 6 (CCSM 6). CCSM & denied knowing CCSM 3. CCSM 9 stated that she heard that CCSM 3 yelled at the children, but she never heard CCSM 3 yell firsthand. It was reported to her by other staff members that CCSM 3 used cuss words in front of the children. Some parents told her that their children were happier once she became their "teacher." On 03/15/2023, I received a written complaint alleging that Child Care Staff Member 5 (CCSM 5) had vodka in her backpack accessible to children at the center on 02/01/2023. It is suspected that CCSM 5 has worked with children on two occasions while being under the influence of alcohol. Additionally, Child Care Staff Member 1 (CCSM 1) and Child Care Staff Member 7 (CCSM 7) smoke "weed", use "edibles", and smoke a vape pen in their vehicles during their breaks. Also, Child Care Staff Member 6 (CCSM 6) takes Adderall that is not prescribed to her while caring for children at the center. Wi s On 03/20/2023, I spoke to by telephone. He stated that he spoke to Child B's Mother on other told him that she has no concerns with the center. He stated that he spoke to Child A's Mother on 03/16/2023. Child A's Mother told him that Child A no longer attends the center. She told him that she had concerns with the center and that she shared her concerns with program director Cheryl Jurek and licensee designee Kellie Olson, but they failed to address her concerns. Child C's Mother was not interviewed regarding these allegations because Child C stopped attending the center in July 2022. 7 On 03/20/2023, 1 spoke to by telephone. Cheryl Jurek advised that the center does not have cameras in the classrooms. present for interviews with assistant director Child Care Staff Member 12 (CCSM 12) and licensee designee Kellie Olson. CCSM 12 stated that CCSM 5 was employed with the center from 11/29/2022 until 02/01/2023. Kelly Olson noted that CCSM 5's employment ended when she was asked to leave after a vodka bottle was discovered in her possession on that day. Kellie Olson explained that CCSM 5 offered to get a breathalyzer, so they initially asked her to leave to get the test. CCSM 12 stated that CCSM 5 did not act like she was intoxicated and denied smelling alcohol on CCSM 5 on 0210112023. CCSM 5 told them she had spent the night at a friend's house the night before and was dropped off at work in the morning, so she had no choice but to bring her two duffle bags inside the center. Kellie Olsen stated that CCSM 5 failed to provide sufficient documentation that she was not under the influence when she was working on 02/01/2023, so the decision was made to terminate her employment. CCSM 12 and Kellie Olson stated that no one brought any concerns to them about CCSM 5 smelling like alcohol or acting like she was intoxicated while working at the center before 02/01/2023. CCSM 12 stated that she escorted CCSM 5 down to the classroom where she had stored her duffle bags so that she could gather her belongings. CCSM 12 recalled CCSM 5 bending down to pick up her 2"1' bag when something fell out from the bag. CCSM 12 denied seeing what it was that had fallen. She noted that there was nearby on a cot, but she could not recall which child it was. CCSM 12 and Kellie Olson stated that they have no knowledge of CCSM 1, CCSM 7, or CCSM 6 working while under the influence of any substance. They have no concerns with CCSM 1, CCSM 7, or CCSM 6 ability to care for the children. and I also interviewed Child Care Staff Member 1 (CCSM 1), Child Care PaStaff Member 11 (CCSM 11), Child Care Staff Member 10 (CCSM 10), Child Care Staff Member 8 (CCSM 8); and Child Care Staff Member 13 (CCSM 13). CCSM 8 stated that CCSM 5 did what was easiest for herself. She described her as less professional, such as leaving her food around the classroom, taking her shoes off, and allowing children to sit on her lap at rest time. She thought CCSM 5 dozed off one day during rest time. She saw CCSM 5 close her eyes for 10 - 15 seconds and then jump with a start when a child got up to wash their hands. She noted that CCSM 5 seemed confused in that moment. She told a coworker, but she did not report it to any administrators. She described CCSM 5 as seeming "frazzled" at times, not overwhelmed, but she was getting there. She stated that Child A's Mother told her several times that CCSM 5 smelled o

The complaint states that CCSM 2 squeezed pressure points on Child A's arms and legs when Child A was having a temper tantrum. Child A asked CCSM 2 to stop touching her because it hurt, but CCSM 2 did not stop. According to the complaint, this was "caught" on camera. Child care licensing consultant Stephanie Shell investigated a complaint alleging that Child Care Staff Member 2 (CCSM 2) grabbed Child B by the arm. Stephanie Shell concluded that CCSM 2 provided positive methods of discipline when she redirected Child B after he took a toy from another child. present for interviews with assistant director Mild Care Staff Member 12 (CCSM 12) 2=s and licensee designee Kellie Olson. CCSM 12 stated that CCSM employment was terminated due to lack of professionalism. She explained that CCSM 2 threatened to assault Child Care Staff Member 10 (CCSM 10). id I also interviewed Child Care Staff Member 1 (CCSM 1), Child Care Staff ember 11 (CCSM 11), CCSM 10, Child Care Staff Member 8 (CCSM 8), and Child Care Staff Member 13 (CCSM 13). CCSM 8 and CCSM 11 stated denied having knowledge of the allegations against CCSM 2. CCSM 10 stated that CCSM 2 would physically guide children to a place to calm down rather than talk them through the issue. She recalled a time on the playground when CCSM 2 picked Child C up under his armpits and placed him on her lap and held him for discipline purposes. CCSM 10 thinks Child C hit another child, but she is not certain. Child C sat on CCSM 2's lap sideways while CCSM 2 sat on the ground. CCSM 2 was trying to keep Child C from hitting her. She had a hand on each of his wrists. Child C was screaming and trying to swing his arms to hit her. CCSM 2 was not being rough with Child C. From a distance, it looked like Child C was physically restrained in a tight hold, but the video footage revealed that her hands were loose. CCSM 10 recalled another incident between CCSIVI 2 and Child A. She noted that it was common for Child A to run to another classroom when transitioning from the playground to the classroom or to just run in general. The children were lining up to go inside when Child A ran towards the back fence, which was a common occurrence for Child A. CCSM 2 picked Child A up and put her over her shoulder. Child A was screaming and CCSM 2 told her to calm down in a harsh tone.. CCSM 10 stated that it did not look playful, but perhaps it was. She explained that CCSM 2 also babysat Child' A at her home, so she wondered if this was typical banter between CCSM 2 and Child A. She described Child A as being very rude to her. On 7/26/2022, I spoke to Child A's Mother. She stated that Child A (male, 3 years old) has attended care at the facility since he was 12 weeks old. Child A transitioned into the 3-year-old classroom the second week of June 2022. Since his transition she has noticed behavior changes where he is aggressive towards others. Child A has been seeing a child psychologist due to his behaviors. Child A's Mother reported, Child A was picked up from the facility early on Friday, 7/22/2022. When asked about his day, Child A responded "Skie broke my arm." Child A's Mother reported that Child A told her that Skie clinched his arm so hard he felt like it was breaking. Child A's Mother reported there were no marks on Child A's arm. She e informed the program director, Cheryl Jurek of the incident on Monday 7/25/2022. Child A's Mother stated that she has had no concerns with the facility until this incident and stated that her child has received "spectacular care." On 7/26/2022, I completed an unannounced inspection at the facility. During my onsite inspection, I interviewed licensee designee, Kellie Olsen. She stated that she 2 was not present in the classroom when the alleged incident occurred, however has made aware of the allegations. Ms. Olsen reported that she does not believe that child care staff member, Skie Roberts would ever hurt a child. Child A started having behavioral issues in the toddler classroom when he started in the 3A classroom the staff to child ratio and class size increased. The expectations for children increase and so does their independence. Ms. Olsen stated that she has observed the classroom and has seen Child A become aggressive towards staff members in the classroom. Child A will become upset, and he will attack staff members and scratch them and say things like "I want to cut off you're your head and nipples." Ms. Olsen reported that program director, Cherly Jurek, requested a meeting with the parents to discuss Child A's behaviors on 7/19/2022. On 7/26/2022, I interviewed lead caregiver, Skie Roberts. Ms. Roberts reported on Friday, 7/22/2022, she noticed staff member, Annabel Childers, working with Child A's emotions. Ms. Roberts went over to intervene and found out that Child A had taken a toy from another child. Ms. Childers tried redirecting and explaining to Child A that he couldn't do that, and Child A got upset. Ms. Roberts started talking to Child A about his emotions and Child A said, "I want to cut off your head and nipples." Ms. Roberts told Ms. Childers to call the office to inform them of what is happening and to contact Child A's Mother. During the incident Child A was next to the sensory table and rubbing his arm on it saying, "I want to cut my arm, I want to bleed." Ms. Roberts stated that she started documenting the incident and removed his arm from the sensory table, so he didn't cut his arm. She added that there were no marks on his arms from this. Child A then put his fingers in his ears and said, "I have to get the smoke and blood out." Ms. Roberts stated that she tried talking him through the situation and he eventually calmed down and said he wanted a "huggle" which is a hug. Around 11:30am Child A's Father picked up unexpectedly and Child A started hiding behind staff members and saying he did not want to go home. Child A's Father had to coax him saying he was not in trouble and his mom was going to take him to the park. Child A eventually went home with is father. Ms. Roberts denied ever hurting or grabbing Child A. She added that she provided Child A's Father a copy of an incident report and documentation that he could take to the child psychologist on 7/25/2022, their previously scheduled meeting. During my onsite inspection, I interviewed staff member Annabel Childers. Ms. Childers reported on 7/22/22, Child A took a toy from another child. She intervened and explained to Child A that he cannot take toys from other children. Child A got upset and Ms. Robert came over and took the le
