On 6/27/2023, I contacted Child A's Mother. She reported that her son briefly attended the facility for summer care. Child A's Mother stated Child A went there for a total of six days before she pulled him for care. Child A's Mother reported that Child A came home one night and told her that program director, Alicia Harman yelled in his face. Child A's Mother did not know the date the incident occurred or why it occurred. Child A's Mother reported that she took him to child care the next day and when she dropped him off all the children were outside playing. Alicia Harman was there with an unknown staff member. Child A was left crying on the playground and no one was attempting to comfort him. Child A's Mother stated that she got out of her car and then Ms. Harman attended to Child A and held him. Child A's Mother stated that Child A told her after she drove away Alicia Harman set him down and said "na-na, tricked you, go away." Child A's Mother also reported another incident she witnessed during drop off when an unknown child was outdoors playing and went to the door and Alicia Harman 2 would not allow her to come into the facility. Alicia Harman stuck her finger in the child's face and said "no." Child A's Mother stated there was a staff member outside with the group of children. Child A's Mother was unsure of why the unknown child wanted to come into the facility. Child A's Mother could not recall the date when the incident occurred or who the other staff member was that was outside. On 6/29/20223, I completed an onsite inspection. During my onsite inspection, I interviewed Child Care Staff Member 1 (CCSM 1). She reported that she has worked at the facility for the past two years and works with Alicia Harman almost daily. CCSM 1 stated that she has never witnessed Ms. Harman yell at Child A. She added that she has never heard Alicia Harman yell at any child. The only time that Alicia Harman would raise her voice is to get the classroom's attention. CCSM 1 reported that Child A was always so quiet and shy that there would never be a reason to get upset with him to yell at him. CCSM 1 stated that Child A had separation anxiety and Child A's Mother also had a hard time leaving Child A. CCSM 1 stated that she was present on Child A's last day and during drop off. CCSM 1 stated that they were outside playing when Child A's Mother dropped off Child A. Child A's Mother and Child A had a routine where Child A would get dropped off and Child A would watch his mom get into the car and mom would wave goodbye. That day Child A's Mother dropped off Child A, got into the car and then got back out, Alicia Harman walked over to Child A and told Child A's Mother "it's ok, I've got him." CCSM 1 stated she saw Alicia Harman take Child A and comfort him, but never heard what she said to him because she was on the opposite side of the playground. CCSM 1 could not recall a time a child wanted to go inside, and Alicia Harman would not let them. CCSM 1 stated that if a child needs to go to the bathroom, they have to ask for permission first. CCSM 1 stated they allow one child at a time to go to the bathroom when they are outside on the playground. CCSM 1 stated that if the child is not back in a few minutes that they will go into the building to see why the child has not returned to the playground. During my onsite inspection, I interviewed CCSM 2. CCSM 2 reported that she never saw Alicia Harman yell in Child A's face. She stated that she has never seen Alicia Harman yell at any child. CCSM 2 reported the only time Ms. Harman would yell would be to get the groups attention. CCSM 2 stated that she is unaware of a time when a child was denied going inside to go to the bathroom or get a drink of water. She stated she never witnessed Alicia Harman deny a child to come inside and point in their face and tell them "no." CCSM 2 stated that children are allowed go inside to use the bathroom or get a drink of water when they are outside. The child has to

Child A's Mother reported that Child A told her that during lunch, Ms. Harman will make children throw their food away if a child says something she does not like. Child A's Mother did not know of any specific dates, children that were made to throw their food away, or if this was an isolated incident or occurred more than once. CCSM 1 stated a child has never had to throw their food as a form of discipline. She reported that after a child is done eating, they are asked to throw any uneaten food away, or if they drop food on the floor, they are asked to throw that food away. CCSM 2 reported that she's never witnessed a staff member telling a child to throw food away because they were misbehaving. CCSM2 stated the only time a child is asked to throw away food is when they are finished eating. Ms. Harman stated that she or staff would never make a child throw away their lunch as a form of discipline. If children aren't behaving during lunch, they are asked to move seats. Alicia Harman reported that children bring their lunches from home, 4 children are asked to throw away unsalvageable food but keep food they haven't opened.

Child A's Mother reported that children spend most of their day in the gym, however, have free range to go into the other classroom to get toys, unsupervised. Child A's Mother also reported that children are allowed walk to the bathroom unsupervised. CCSM 1 reported that if a child needs to go to the bathroom, they must ask for permission first. CCSM 1 stated they allow one child at a time to go to the bathroom when they are outside on the playground. CCSM 1 stated that if the child is not back in a few minutes that they will go into the building to see why the child has not returned to the playground. CCSM 2 reported that children are allowed go inside to use the bathroom or get a drink of water when they are outside. The child must ask for permission first, so staff knows where each child is at all times, they have to go alone, and if the child is not back in a few minutes staff will go into the building to check on them. Alicia Harman reported that if a child needed to use the bathroom or get a drink of water, she would have not denied them from coming inside to do so. Alicia Harman reported that children must ask to use the bathroom, so they know where children are at all times. She stated based on their age, they will allow the child to go to the bathroom by themselves, if the child is gone for more than a few minutes staff will check on the child to make sure they are ok.
