On 05/18/2023, I attempted to contact Child A's Mother. I left a message asking her to call me back. She called me back and then the call was disconnected. I called her again and the call went to voice mail. 2 On 05/18/2023, I attempted to contact Boulder Ridge Wild Animal Park, but they were not open. I sent an email to follow up. On 05/19/2023, I completed an unannounced on-site inspection and interviewed program director Kerry Robinson who reported that she was not on the field trip but CCSM 1, CCSM 2 and CCSM 3 were present. She explained that two classrooms were combined on the field trip. There were a lot of parents present with their children and the remaining children were separated into groups and assigned to CCSM 1, CCSM 2 and CCSM 3. She reported that she spoke with Boulder Ridge Wild Animal Park staff and was told that another school brought Child A to the office and said he lost his group. I interviewed CCSM 1 who said that they were on a field trip to Boulder Ridge Wild Animal Park. She was walking around with her group and took a playground break. After they played on the playground, they transitioned to the picnic tables for lunch. CCSM 3 counted the children and then noticed that Child A was not with them. She went back to the playground, and he was not there. They began looking nearby for him. While looking for him, another adult who was at the park with a different school told them that Child A was with an employee of Boulder Ridge Wild Animal Park in their office. CCSM 1 said that Child A was calm, and she then took him back to the rest of the group. CCSM 1 said that in hindsight, she should have counted, walked around and double checked to make sure she had all the children in her group. She stated, "we are always very good about counting, not sure why I didn't." CCSM 1 was supervising a group of five children and CCSM 2 and CCSM 3 were both in charge of six children each. CCSM 1 said that she thinks Child A may have been separated from the group for about 10 minutes. She said that they did a count of the children at 11:00 AM and Child A was found in the office at 11:10 AM. CCSM 1 said that Child A's parents are okay, and Child A is one that they need to keep an eye on because he "isn't always in tune with what the group is doing." I interviewed CCSM 2 who reported that she did not become aware that Child A was missing until the middle of the incident. She was with her assigned group at the picnic table. CCSM 2 explained that they had 28 children with 3 child care staff members. Seventeen children did not have a parent that attended with them, so they were split up between the three child care staff members. Parents were only supervising their own child. CCSM 2 said that they walked to the picnic tables, and it was very crowded so she could see it happening that Child A was separated from the group by the llamas and bird sanctuary. She thinks that Child A was missing for 10 minutes or less. On 05/19/2023, I spoke with Witness from Boulder Ridge Wild Animal Park. They explained that another school group parent brought Child A to their gift shop, and he told them what school group he was with. She thinks that he was in the gift shop for less than a minute. Child A said that he was going by the penguins. 3 On 07/05/2023, I attempted to contact CCSM 3 via telephone and sent a text message. On 07/10/2023, I received a phone call from CCSM 3 who explained that the field trip started with her splitting the children who did not have parents there into three groups. They planned to meet back up for lunch. When she was handing out lunches, she realized that Child A's lunch was still there. She asked CCSM 1 where Child A was. CCSM 1 told her that he was sitting by another child and the child's father. CCSM 3 looked over but could not find Child A there. She kept looking for him and told CCSM 1 and CCSM 2 that they all needed to go look for him. At that time, an adult from another group told them that Child A was with
