On 11/10/2022, child care licensing consultant Kathleen completed an on-site inspection with ning The following information is from approved report dated 12/14/2022. talked to Ms. Cunningham about a center's delay in reporting the mark CCSM 1 left on Child A's arm. Ms. Cunningham stated that the center completed their own investigation and suspended CCSM 1 for two days. Ms. Cunningham reported that CCSM 1 completed training on her two-day suspension and is now 2 back to work. Ms. Cunningham also reported that the center's cameras are not working in Child A's classroom, and she is not aware when they stopped working. The following information is from approved report. CCSM 1 stated she and CCSM 3 divided the children into two groups. She took her group to the "outdoor classroom" while CCSM 3 stayed in the classroom with her group. CCSM 1 stated that she gave the children multiple count down warnings to clean up on 11/03/2022, but Child A continued to play. Child A does not clean up or follow directions often and needs a lot of redirection and supervision. She*erxepsrsees-sde-dI concern regarding Child A's behaviors because he is ards the other children. She reported that Child A e floor, and e recalled an incident on the playground where Child A was knelt in fr her child, and it looked to her as if Child A was going to= the other She stated that the other child denied anything happening u reported that theMind ent made him uncomfortable. Child A was instructed to keep his hands to himself. CCSM 1 stated that she liked Child A a lot and she would never do anything to hurt him or any of the other children. CCSM 1 reported that all the other children were cleaning up. She calmly told Child A that it was time to clean up as she started to walk towards him. Child A started to turn and run from her as she bent down in front of him. CCSM 1 stated that her thumb nail made contact with Child A's arm when she reached out and grabbed his arm to stop him. Child A looked at her with a "shocked" face. She apologized to Child A multiple times and told him that it was an accident, and that she would never hurt him_ CCSM 1 reported that the red mark was not bleeding and did not break the skin. She asked Child A what he needed and provided him with an adhesive bandage. CCSM 1 reported that she checked in with Child A throughout the day and he told her that he was good. Child A took the bandage off in the afternoon and refused her offer of another one. CCSM 1 stated that she did not tell any coworkers about the incident, and she did not fill out any forms regarding the incident. She reported that she has never completed an incident report since she began at the center this past August. She stated that she was not asked about the mark by anyone on 11/03/2022. On 11/07/2022, she was called into the office by program director Karen Hayes regarding the mark on Child A's arm. Ms. Hayes advised her that Child A's Mother noticed the mark and wanted to know what happened. CCSM 1 told Ms. Hayes what had occurred on 11/03/2022 with Child A. She stated that she was suspended for two days, she had to complete additional training, and she had to complete an incident report. She has not talked to Child A's parents about the incident even though she has seen them at the center because she is afraid that they are mad at her. She stated that she feels really bad for what happened. According to report, a safety plan was established with Ms. Cunningham on 11110/2022 indicating that CCSM 1 will not be unsupervised with any of the children at the facility. 1 it from egarding the case. estigation was needed.
