On 10/08/2021 Stefania Myers, lead caregiver, submitted an incident report to Stephanie Shell, licensing consultant, informing her that Child A, female age 4, was found wandering in the hallway. On 10/12/2021 I left a telephone message for Child A's Mother requesting contact with her. On 10/14/2021 I conducted an unannounced on-site inspection to the licensed facility. I met with Missy Buurstra who is the Great Start Readiness Program (GSRP) administrator. She was aware of the incident and how it had been handled, however she was not present when the incident took place. I met face to face with Sam Smeege to inform her of the investigation. 2 I met face to face with Stefania Myers, lead caregiver. She explained to me on the day of the incident the classroom was very warm. She had propped open the entrance door to allow for cooler air to enter the classroom. She and her assistant were instructing different groups of children when Rachel Horaney, the office administrative assistant, came into her room with Child A. She was not aware she had left the classroom. She called Child A's Mother and informed her of the incident. I then met face to face with Julie Watts, who is another GSRP teacher/program director, who found Child A in the hallway. She explained on the day of the incident she was in the hallway with one of her students when she noticed a child walking back and forth. She asked Child A if she needed help and brought her to the front office as she did not know Child A. She recalled Child A was in the hallway from 1-2 minutes by herself. I then met face to face with Rachyl Horaney, administrative assistant. She recalled the incident with Child A. She said Ms. Watts brought Child A to the front office stating she was found in the hallway. She knows all the children and brought Child A back to her classroom. Upon arriving in the room both teachers were surprised to see her with Child A as they were not aware she had left the classroom. On 10/22/2021 I spoke with Child A's Mother by telephone. She said she had spoken to Child A's teacher, and she apologized. She felt it was an accident and did not have any concerns about Child A's care or supervision at the licensed program. On 11/16/2021 I spoke with Julie Guenther, designee, for an exit conference and recommendation of a second provisional license.

On 03/03/2021 I completed an unannounced visit to Kentwood ISO Breton location. I met with Program Director Melissa Buurstra. Mrs. Buurstra stated Child A (Female, 4-year-old) was left in the hallway unattended for a couple minutes on 02/25/2021. Mrs. Buurstra stated staff member Julie Guenther notified the office of the incident. Then the incident was reported to Licensing Consultant Stephanie Shell on 02/26/2021. Mrs. Buurstra stated she was not in the classroom or near when the incident happened. She stated staff member Noemi Murillo was with Child A and the classroom getting them ready to go outside for recess. Ms. Murillo was then assisting another child in the hallway and Child A slipped behind her back. I interviewed staff member Noemi Murillo. Ms. Murillo stated she was getting her eight children ready to go outside for recess. While getting the children ready, one child began misbehaving and laid on the floor. Ms. Murillo was getting this child up to 2 continue walking to the door, and Child A slipped behind her back and went towards another hallway and the lockers. Ms. Murillo had counted the children while getting ready and in line and had accounted for them all. She forgot to count the children again as they were all outside on the playground. Child A was then brought outside approximately two minutes later by teacher Kari Johnson. Ms. Johnson stated Child A was in the hallway alone near the lockers. Child A was not upset and ran onto the playground with her other friends. Ms. Murillo stated she was upset the incident happened and would be taking time to make sure she counts children at all transition times. I spoke with staff member Julie Lutz. Ms. Lutz stated she was outside on the playground with her eight children. While she was outside on the playground, teacher Kari Johnson came out with Child A. She stated Child A was in the hallway near the lockers unattended. It appeared Child A was unattended for approximately two minutes. Child A went outside and joined her friends on the playground. Ms. Lutz stated she talked to Ms. Murillo who stated Child A must have slipped behind her while she was assisting the child who was laying on the ground. Ms. Lutz stated she spoke with Ms. Murillo about the importance of counting children at all transitions. She stated Ms. Murillo was upset at herself for the incident. Ms. Lutz stated Child A's Father picked Child A up and was notified of the incident. Child A's Father acknowledged the incident happened but did not have any other input. I spoke with teacher Kari Johnson. Ms. Johnson stated she works at the school but is not an ISD employee for the GSRP program. Ms. Johnson stated she was in the hallway and noticed Child A sitting by the lockers alone. Ms. Johnson talked with Child A and noticed that children were on the playground. She took Child A on the playground to join her classmates. Ms. Johnson stated she did not have any idea how long Child A was in the hallway as she did not see when the classrooms went outside. She stated it did not appear long as the other children had not gotten very far on the playground. I completed an exit conference with Program Director Melissa Buurstra. I explained that Child A was left alone, and the center would be cited for appropriate care and supervision. Ms. Buurstra stated she understood and indicated she would be talking to staff about counting and making sure to always provide direct supervision to children. On 03/16/2021 I contacted Child A's Father. Child A's Father stated he was aware of the situation and did not have any concerns. He stated the school acted appropriately. Child A still attends and enjoys her school. On 03/24/2021 I held an exit conference with Licensee Designee Ashley Karsten. I 1st notified Mrs. Karsten of the provisional status. Mrs. Karsten stated she understood the decision. 3

R 400.8365 · R 400.8365 Heating; temperature. (1) The temperature in child use areas must be maintained at a safe and comfortable level so that children in care do not become overheated or chilled.
On 01/14/2021 I made an unannounced visit to The Kent ISD- Kentwood GSRO Breton Ave. location. I met with Program Director Melissa Buurstra and went over the allegations. Mrs. Buurstra stated there has been an issue with a radiator in classroom A. She stated the classroom radiator does need to be repaired and a part has been ordered. Mrs. Buurstra stated the room has never fallen below the allowed temperature of 65 degrees Fahrenheit. She stated they have a digital thermometer in the room that always shows the temperature. Mrs. Buurstra indicated she does get in the school early and if the room feels cold, she turns on a space heater and warms the room up. She stated the space heater is on when the classroom is not open to children. She heats the room up and then turns and unplugs the heaters right before the children come in. Mrs. Buurstra stated she has only had to use the heaters one day because the temperature has not been that 2 cold. The classroom doors are opened and the heat from the rest of the building also circulates in the classroom. Mrs. Buurstra stated they were having issues with the internet and phone as well, but that every teacher in the building has a working cell phone. In the event of an emergency, any teacher in any room would be able to call out. She stated the phones and internet have been fixed. She stated parents were notified the day the phones were out. I walked down to classroom A with Mrs. Buurstra. I observed the classroom to be 68 degrees Fahrenheit. The room temperature was comfortable. The temperature reading was being taken approximately 2 feet from the ground. The phones were also in working order. Mrs. Buurstra stated the part that is being ordered for the radiator might take some time to get in. She stated if the room falls below 65 degrees Fahrenheit and they are not able to keep it warm, they will move into room 4 and use that approved room. I was able to view and verify room 4 was ok to use. Mrs. Buurstra stated she will let me, and her Licensing Consultant Stephanie Shell know when the part arrives, and things are back in order. If there are any issues that she needs help with, she will contact Mrs. Shell directly. On 02/02/2021 I contacted staff member Jessica Isler-DeBoer. Mrs. Isler-DeBoer stated the classroom was out of heat, but it was comfortable. The staff opened the door, and the hallway heat was able to come into the room. Staff were also able to use heaters to warm the room up before the children were in class. The only day that it was a little chilly was on a Friday, but there are no children in care on Friday's. Mrs. Isler-DeBoer stated the facility has done everything they needed to keep staff and children warm and she has no concerns. She also stated every staff member has a cell phone to use. On 02/03/2021 I spoke with staff member Kris Lodewyk. Mrs. Lodewyk stated the classroom was out of heat, but it was always comfortable. She stated they opened the door for the heat in the hallway. They also used heaters in the classroom before students came in. Mrs. Lodewyk stated the room was never below a temperature of 65 degrees Fahrenheit. She stated the boiler was being fixed today. She also stated every staff member has a cell phone to use. 3

The temperature in the room was maintained properly. It was 68 degrees Fahrenheit at my visit. If the temperature falls below 65 degrees Fahrenheit, the classroom will be moved to room 4.
