On 04/29/2021, I completed an unannounced on-site inspection with . Ms. were reviewed. Ms. Kwiatkowski explained that she did not notice any injuries on Child A on either occasion. Child A's Mother informed her of the 04/21/2021 injuries in a text message. Child A's Mother said something about the bruise on 04/27 when she picked Child A up from Ms. Kwiatkowski's care, but Ms. Kwiatkowski said she had not noticed it. Ms. Kwiatkowski could not think of anything that would have caused the injuries. She said that maybe they happened when Child A was sleeping from rolling and hitting head on the crib. On 04/21/2021, she was caring for four children and she did no see or know about any injury to Child A. She was caring for three children on 04127/2021 and did not see or know about any injury to Child A. Child A plays on the floor, in the exersaucer or sits in the high chair. Child A rolls back and forth but does not crawl or pull to standin . She denied that Child A is ever placed on the couch or chairs an no on carries around. Ms. Kwiatkowski W,J denied that an one else ever provides care for Child A. Ms. Kwiatkowski cares for Child C years old) and ChildD , year old) when Child A is in her care. Ch and Child D do not interact with hi d A. Child A did not cry or get upset on either day and is not able to get out of the exersaucer or high chair. There are no other children in the room with Child A when is nappin . Child A did not go outside either day. Ms. Kwiatkowski did not go anywhere with either. She described Child A as a "chill baby" who wants to be fed or change when crying. Ms. Kwiatkowski showed us the approved child care spaces and the crib that Child A sleeps in. I asked Ms. Kwiatkowski about the other children enrolled in her care and she was unable to remember their mother's name. Ms. Kwiatkowski said that she had a "bad memory." She offered no explanation about why her memory was bad other than "I had 5 kids." Ms. Kwiatkowski has who reside in the home. Ms. Kwiatkowski said a none of her were present on the days that Child A was injured. On 05/25/2021, l spoke with Child C and D's Mother who shared that she has no concerns about the care Ms. Kwiatkowski is providing for her children. Child C and 3 D's Mother said that only Ms. Kwiatkowski cares for the children when they are in her home. She has no concerns about Ms. Kwiatkowski's memory. Ms. Kwiatkowski has been kind, is excited to see the children and taught Child D to blow kisses. Her children have not had any unexplained injuries while in Ms. Kwiatkowski's care. She has seen Ms. Kwiatkowski's leaving the home in the morning when she is dropping her children off. I also spoke with Child C and D's Father who stated that he has no concerns about the care provided by Ms. Kwiatkowski. He said that no one besides Ms. Kwiatkowski has cared for the children while they are in her home. His children have not had any concerning injuries, only normal childhood ones. Overall, he has been happy with the care provided by Ms. Kwiatkowski. We discussed the investigation with Ms. Kwiatkowski. It was discussed that the bruise on Child A had a pattern to it and looked like it may have come from a Lego. Ms. Kwiatkowski explained that her spends the night, but he is not in the home when child care children are present and he does not provide care for the children. She does not consider him to be living with her but acknowledged that he spends the night. MHM1 and MHM3 reported that lives in the home; MHM2 reported that he sleeps over. On 06/08/2021, l spoke with Child A and B's Mother about the investigation. She explained that on 04121/2021, Child A and B's Father icked Child A up from Ms. Kwiatkowski's care and Child A had a blood blister in mouth when he got home. She sent Ms. Kwiatkowski a text message and picture o it and Ms. Kwiatkowski told her that she did not see it. Later she noticed that Child A had dried blood in .r nose. She thinks that maybe Child A fell face forward and th
