On 1017/2021, l received a complaint stating that on 9/2/2021 Child A was injured with a he provider reported she got up off e couch and trippedover the dog, landing on another couch on her elbow, while holding Child A in her arms. On 10/13/2021, I spoke to Child A's Mother. She stated that on 9/2/2021, she picked Child A up from Ms. Jacobs' home and that she was asleep in her pack and play 4 upon her arrival at approximately 4:20 PM. She had to wake Child A up to leave the child care home. Ms. Jacobs did not report an injury to herself or to Child A at that time. Child A's Mother noted that Child A seemed "really happy" when she woke up and that she was also happy when she got home. That evening Child A's Mother was pedaling Child A's legs and that she began screaming. She realized that Child A contact Ms. Jacobs' to see if anything had happened at her home that day. She explained that Ms. Jacobs was attending a rehearsal dinner that evening so she was not able to speak on the phone, but she was able to send a text message to Child A's Father when he asked about any possible injuries. Child A's Mother noted that Ms. Jacobs' reported to Child A's Father via text message that she tripped over the dog while holding Child A, but that she caught herself and that she didn't think it caused an iniury to Child A. A's Parents Mopped by Nis. J to get more details on the fall. Child A's cabs"home Mother reported that Ms. Jacobs demonstrated how she fell and explained that she was sitting down on the couch, she had fed Child A a bottle, and then she fell asleep in her arms. Ms. Jacobs demonstrated to Child A's Parents standing up, tripping over the dog, and falling onto the seat of another couch when she tripped, falling onto the child, who reportedly had her foot turned "funny" into Ms. Jacobs' stomach; and taking the brunt of the fall on her foot/ankle, Child A's Mother indicated that Ms. Jacobs' mentioned that prior to falling asleep, Child A had been crying and was having a hard time taking the bottle. Child A's Mother explained that Child A had only been at the child care for two or three weeks at that time and that she had been having issues transitioning. She explained that Ms. Jacobs' told them she 'didn't think anything of it' at the time Child A didn't seem to be injured after the fall. Child A's Mother noted that after seeing the demonstration, she "completely understands how it could have happened" and feels it was a "freak accident". Child A's Mother stated that for the first week after the incident, her and Child A's Father kept both of their children home from Ms. Jacobs' child care because they didn't know if they should They did end i1 up sending both children back as she noted they had to go back to work and it On 1011312021, l completed an unannounced on-site inspection to the child care home. I interviewed Dorothy Jacobs. She stated that it was one of the first weeks that she had Child A in her care. Around 2 PM on 91212021, she was holding Child A while sitting on one of her couches. She reported she was rocking her to sleep and that she had not taken the bottle well and was a little fussy. Child A was reported to be "half asleep" as Ms. Jacobs stood up and continued rocking her for another second or so. She stated she didn't that her dog was laying on the floor near realize her feet and as she stood up, she tripped over the dog, stepping on him, and fell into another couch that was next to the couch she was sitting on. The couches were next to each other in an "V shape, and there was about a foot or more space in between them. There was also a large glass, round, coffee table in between the couches, leaving approximately a foot of space between the coffee table and each of the couches. Ms. Jacobs demonstrated the fall for me in the setting where the fall took place. Ms. Jacobs stated she thinks Child A's foot must have twisted when she landed between the couch and Ms. Jacobs.
