On 12/09/2021 I completed an unannounced visit to David D Hunting Child Development Center. I interviewed Program Director Stacy Shotko. Ms. Shotko stated Child A was in the classroom with the rest of the students. The class was ready to go outside, and the children had put on all their snow gear. When the class got outside, staff only counted 13. The classroom had 14 children. A staff 2 member went back into the classroom and found Child A sitting on the cot alone. Child A was alone for approximately 4 minutes. I interviewed child care staff member Kelsey Kolk. Ms. Kolk stated she was in the room with 14 children, and they were getting ready to go outside. Ms. Kolk was the assistant in the class to her co-worker Mary Johnson. Ms. Johnson stated all children were counted and they began to walk down the hall to the playground. When the children got to the playground, they were counted again. They only counted 13 children and noticed Child A was not there. Ms. Kolk went back to the classroom and noticed that Child A was in the room alone. Ms. Kolk retrieved Child A and brought her back to the playground. Ms. Kolk stated Child A was left alone for approximately 4 minutes. On 12/09/2021 I contacted child care staff member Mary Johnson. Ms. Johnson stated she was working in the classroom on 12/06/2021 with her co-worker Kelsey Kolk. The children were getting ready to go outside on the playground. Ms. Johnson counted all the children and believed she counted the full 14 children in the class. When the classroom got to the playground area, the children were counted again. It was then that Ms. Johnson noticed that Child A was not in the group. Ms. Johnson asked Ms. Kolk to go get Child A from the room and bring her back. Ms. Johnson stated she felt she counted correctly but must have missed Child A when they were lining up. Ms. Johnson stated she is taking some time off from work and realized that she made a mistake. On 01/05/2022 I contacted Child A's Mother. Child A's Mother stated she was notified of the incident, but not until the next day. Child A's Mother stated she does not have any concerns about the center or the incident. She stated she knows that people can make mistakes. She stated her child still attends and enjoys the center. I attempted to contact Child B's Mother and Child C's Mother. I left a messages for a return call; however, as of the date of this report, I have not received a return telephone call.
