Appropriate care and supervision were provided to Child F on 9/24/2025. Child F was not injured during the interaction on 9/24/2025. Teacher 1 denies any ill intent on her behalf. She stated Child F slipped and she was trying to readjust her for the feeding. She did not realize Child F's arm was behind her 2 LANDMARK BUILDING • 105 W. ALLEGAN STREET • LANSING, MICHIGAN 48933 Michigan.gov/MiLEAP back. Teacher 2 stated Child F began to cry when she stopped the feeding to hand her to Teacher 2.

Based on our findings, investigation, and review - which includes considering the investigation and findings of MDHHS -CCLB determines Teacher 1 is conducive to the welfare of children and is eligible to be associated with a child care organization.

(1) At the time of a child's initial attendance, a center shall obtain a child information card, using a form provided by the department or a comparable substitute, that is completed, signed and dated by the child's parent and updated as changes occur. The center shall keep current hard copies of the c hild information cards at the center.

Program Administrator was able to provide me with six of the nine child information cards requested. There was no child information card on file for Child F. Most of the cards were missing required information.

Licensee Designee, Melisa Ricketson Bowman and Program Administration did not report to the department within 24 hours of knowledge of an incident involving an allegation of inappropriate contact.

On 9/30/2025 there were numerous boxes stacked along the hallway from the front door into the back of the building where the classrooms are located. This is a main means of egress in the building. On 10/15/2025 the boxes were no longer stored in the entryway path.
