On 02/13/2023, I completed an on-site inspection. I interviewed program director Rachel Price, licensee designee Emily Vander Galien, and CCSM 1. Rachel Price stated that Emily Vander Galien was on the way to the center with the video to review. I reviewed the video with Emily Vander Galien. She identified CCSM 1 as the staff member caring for the children in the video. The video shows CCSM 1 buckle Child A (17-month-old, male) into a chair at 1:00pm and sit Child B (16-month-old, male) in a chair at the same table when they woke up from nap. Child B was not buckled in the chair and got up at 1:27pm on his own. She sat Child B back down at 1:28pm but is unclear if Child B was buckled in the chair. CCSM 1 picked Child B up from the chair at 1:46pm and sits him back in the chair, unbuckled, at 1:48pm. Child B got back up from the chair at 1:49pm. CCSM 1 sat Child C (17- month-old, male) at the table at 1:40pm he got up from his nap. Child C got up out of the chair on his own at 1:49pm. CCSM 1 unbuckled Child A at 1:50pm. CCSM 1 buckled Child A in a chair again at 2:01p m for snack. She also buckled Child B in a chair at 2:02pm and Child C at 2:04pm for snack. Child D (17-month-old, male) joined the children at the table, unbuckled. CCSM 1 served snack at 2:17pm. CCSM 1 unbuckled Child B and Child C at 2:33pm. She unbuckled Child A at 2:36pm. Based on this information, Child A was buckled in a chair for 50 minutes and again for 35 minutes just 11 minutes later. Child B sat at the table for 27 minutes and was redirected back to the chair for an additional 22 minutes. Child B was buckled in a chair 12 minutes later for an additional 31 minutes. Child C was buckled in a chair 2 for 29 minutes. It is important to note that CCSM 1 was not engaged with the children during this time. CCSM 1 stated that Child A, Child B, and Child C did not nap as long as they usually do, so she tried to keep them busy with books while the other children finished their naps. She did not intentionally leave the children sitting for so long. She stated that the children typically are seated for that length of time for snack. We discussed options for lesson plans for better engagement and transitions. On 02/16/2023, I received an internal corrective action for CCSM 1 from Emily Vander Galien via email. The document indicates that the corrective action includes a written warning for CCSM 1. The document also notes that CCSM 1 appeared to be unengaged and lacked positive interactions between herself and the children and she failed to provide the children with appropriate activities while the other children slept. It also notes that CCSM 1 will receive additional coaching and training with the center's curriculum specialist on 02/22/2023. The training will include curriculum expectations, age-appropriate activities, ideas on how to extend play, sensory, art, music, movement, and real-life items. CCSM 1 complete weekly lesson plan each Friday for the following week. CCSM 1 will be required to document five items on the lesson plan each day. The document is signed by CCSM 1 and Emily Vander Galien.

On 12/22/2022, 1 spoke with center director Sara Schutte who On 12/2712022, l spoke with Child A's Mother who reported that Child A was admitted into the hospital overnight. Child A's heart rate was up to 190+ and her pressure was high. The hospital completed a and Child A She met with Ms. Schutte and CCSM 10 today. Oe "eo ow where the pills came from. She was told that the staff would review the footage of the camera system, but the system is new. Child A's Mother reported that the center notified her at 8:37 AM and Child A's Father at 8:39 AM. 1 Child A was dropped off at 7:40 AM and CCSM 2 and CCSM 5 were in the classroom. Child A's Mother reported that she only stepped into the room when she dropped off. On 12/27/2022, and I completed an unannounced on-site inspection. We interviewed center director Sara Schutte who reported that she has been the director since February. When she arrived at the center on 12/21/2022 just before 9:00 AM, the ambulance was there, and the center staff had contacted the Poison Control hotline. Ms. Schutte reported that she was told that Child A was in the Infant 1 classroom when she got a hold of a The staff saw blue drool on her and then found pieces of a ~ and an intact ~ on the floor. A third was found on the counter in the Infant 2 classroom. he Poison Control hotline was called by CCSM 1, and they told her it was The staff then vacuumed the classroom. The night before, the center had a parent's night out until 8:30 PM. CCSM 3 was in the Infant 1 classroom with two infants, and she reported that she did not see any at that time. The Infant 2 classroom was not used that night. On the 21st, CCSM 2 who is the lead caregiver in the Infant 2 classroom came in at 6:45 AM, CCSM 1 and CCSM 4 came in at 7:30 AM. Right before breakfast, around 8:00 AM, the Infant 2 children were moving from the Infant 1 classroom to their own adjacent classroom. Ms. Schutte has heard from staff that they think the pills belonged to either CCSM 11 because she has not been to work since the incident occurred or CCSM 9 because "her boyfriend smells like weed and she fell asleep in the classroom." CCSM 3 and CCSM 6 were supposed to clean the classrooms on 12/20/2022. The classrooms have cameras, but they only show the center of the classroom and the front area where the high chairs are is not visible in the footage. and I interviewed CCSM 1 who is the lead caregiver in the Infant 1 Ras=sroom. She reported that on 12/21/22, she was working at the front desk at 7:00 AM. CCSM 2 came in to work at 7:30 AM and later called her when she was in the kitchen getting breakfast ready to ask if she had opened up the classroom because she had found a CCSM 1 had not opened the classroom, but she went to the room to speak with CCSM 2. CCSM 1 said that two pills were vacuumed up from behind the high chairs that were against the wall. A third pill was found on the counter of the Infant 2 classroom. CCSM 1 showed us photos that she had of the pills; one of the paper towel used to wipe Child A's face, one of a crushed pill on the floor and one of the intact pill on the counter. CCSM 1 said that she and CCSM 4 found the pill on the counter, looked it up on the internet and then called 911. CCSM 1 said that the 911 operator told them they could call Poison Control, which she did. It took 20 minutes for the ambulance to come to the center. CCSM 1 said that she thinks CCSM 9 had the pills because CCSM 9 was in both infant classrooms the previous day and had been feeding a child in a high chair. CCSM 9 was not there the morning when the pills were found. CCSM 1 said that Ms. Schutte and Ms. VanderGalien told them that they can't ask CCSM 9 if they were her pills. CCSM 1 said that Ms. Schutte went to all the CCSMs and told them that they needed to be diligent about what's in their pockets. CCSM 9 made a comment about who would do that and then changed her shirt into one that did not have pockets. CCSM 1 also said that CCSM 9 is dropped off at wo
