On March 9, 2023, I completed an on-site inspection. I spoke with program director Bridget Riis. She stated Child A is in the Adabelle classroom and Child Care Staff Member 2 and 3 are the "teachers" in that classroom. The classroom had just come in from outside and Child Care Staff Member 4 who was working in the Pratt classroom was getting ready to go outside with her class; she looked up at her door that exits to the playground and saw Child A standing there. Child Care Staff Member 4 brought Child A inside and called Child Care Staff Member 5. Child Care Staff Member 5 went to the Pratt classroom, got information on what happened, and returned Child A to the Adabelle classroom. On March 9, 2023, I spoke with Child Care Staff Member 2. She stated on March 7, 2023, Child A was left outside alone. She and Child Care Staff Member 3 were calling the children to come inside. Child B was trying to get off a bike and she got stuck. Child Care Staff member 2 went over and got Child B off the bike and they went into the classroom. They did not do a name to face when they transitioned from outside play to the classroom. At least, she did not. She is not sure who came in the classroom first, she thinks everyone just came in about the same time. Once inside, she took off her coat, she unzipped two children's coats who asked for help, then she rearranged chairs for snack. She was doing this because one of the children in the classroom has an allergy and they were having yogurt and they moved chairs to keep the allergen away from the child with the allergy. Child Care Staff Member 2 stated after she moved a few chairs, Child Care Staff Member 5 arrived at the classroom with Child A. She believes between the time they returned to the classroom from outside until Child A was returned to the classroom was approximately 2 to 2 minutes. Child Care Staff Member 2 stated during 1/2 transitions from outdoor play to the classroom they typically do one of two things, they call the children to line up at the door and they will do a head count before they open the door, or they will touch the top of the child's head as they are counting, and the children enter the classroom. I explained that I was under the impression they do a name to face and what she just described is not a name to face; I explained name 3 to face is verifying each child by name/face as part of the count. This is normally done with a list that can be checked as making face/name recognition. Child Care Staff Member 2 stated this was something they used to do with their child care management application as they could check a photo of each child when they were counting them. However, the center changed applications and the new application does not have that option. She stated before they went inside, Child A was running up and down the sidewalks between the Adabelle and Pratt classrooms. Child A is new to the classroom and this playground is much larger than the one he used to play on. On March 9, 2023, and March 10, 2023, I spoke with Child A's Mother. Child A's Mother stated someone from CT DBA Gilden Woods Grand Ledge called her right when Child A was found outside without supervision. The person who called told her the child care staff members in Child A's classroom must have counted incorrectly. Where they found Child A was further down the building from his classroom in the backyard. Child A went to the door of another classroom and that child care staff member found him. She stated Child A probably loved being outside longer. Child A is shy and quiet, and he probably was off playing alone, he is also very independent. Child A has been attending the center for two and half years and she has no concerns with the care he is provided. She has no concerns with the incident of him being left unsupervised as she knows how much he plays alone, he was found, and he was fine.
