On 8/8/22, I reviewed the incident report sent to the department. According to the incident report, on 7/29/22 around 4:00 p.m. while the children were engaged in outdoor play, Child A fell from the steps of the blue playground and got a gash on the middle area of her forehead. Ms. Amber Mueller and Ms. Bobbi Cebulski both whom are child care staff members were present outside and attended to Child A by comforting her, applying gauze and an ice pack to Child A's injury and notifying administration of the incident. Child A's Mother was notified of the incident and informed that Child A may need stitches. Child A's Mother picked up Child A from care and took her to the emergency room. Child A received 3 stitches to her forehead. On 8/8/22, I interviewed Child A's Mother via telephone. Child A's Mother confirmed the center notifying her of the incident on 7/29/22 shortly after the incident occurred. The center informed Child A's Mother that Child A fell from the steps of the blue playground and fell to the ground causing a gash on her forehead. The center said the injury looked as though Child A may need stitches. When Child A's Mother spoke with Child A of the incident, Child A told her that she fell trying to duck under the railing of the steps, hit her head and fell to the ground. Child A informed her mother that there were no child care staff members in the immediate vicinity where Child A was playing. Child A said her friend went and told Ms. Mueller, child care staff member what happened. Child A's Mother said she was unsure exactly how Child A fell rather if it was from climbing the steps of the blue climber or trying to duck under the handrailing of the climber. Child A's Mother immediately arrived at the center, picked up Child A and took her to the emergency room where she received three stitches in her forehead. Child A was kept home the following day for precautionary measures but returned back to care on 8/1/22. Child A has been enrolled in care at the center for approximately two months. Child A's Mother 3 expressed no concerns regarding the overall care and supervision they provide for Child A. She said the center communicates with her daily regarding Child A's experiences while in care. On the day of the incident, Child A's Mother said the center notified her immediately of the incident and the first aide, observation and comforting they provided until she arrived at the center. On 8/8/22, I conducted an unannounced onsite inspection to the center. Upon arrival at the center, I informed Mr. Trevor Dunn, assistant director of the allegation. Mr. Dunn informed me that he was not working at the time of the incident as he was on vacation; but was made aware of it upon his return. Mr. Dunn acknowledged the incident occurred on 7/29/22 regarding Child A falling as she was running at the steps of the blue playground. Child A fell from the steps and hit her forehead causing a gash toward the middle area of her forehead. Mr. Dunn said Child A's Mother was notified immediately of the incident as it was thought that Child A may need some stitches on the wound. While they were waiting, the child care staff members, Ms. Mueller and Ms. Bobbi Cebulski attended to Child A's injury by comforting her, applying gauze to the wound, and an ice pack as they comforted and observed her until Child A's Mother arrived at the center. Mr. Dunn said the center completed an accident/incident report and notified the department of the incident. Mr. Dunn said the child care staff members followed their injury protocol by notifying Child A's Mother of the injury immediately as they do with all head injuries. Regarding the supervision while children are engaged in outdoor play, Mr. Dunn informed me that the child care staff members position themselves around the play area to assure they have a global view of all the children playing. Mr. Dunn said the center always employees more than the recommended child care staff member to child ratio of 1:18.
