Tammy Collins did not ensure that Witness 1, Witness 2, and Witness 3 were determined to be eligible to serve as a child care staff member before she left them alone with child care children when she visited her mother.

R 400.1911(1) · R 400.1911(1) Care; supervision; children. A licensee shall ensure appropriate care and supervision of children at all times.

Tammy Collins did not ensure that appropriate care and supervision was provided for the children that she took to visit her mother on numerous occasions. Witness 2 and Witness 3 routinely smoked in the presence of the children. The children had access to a filled hot tub, an above-ground pool accessible by ladder, various medications, cleaning products, and a manual razor when Tammy Collins took the children to visit her mother. Additionally, appropriate care and supervision was not provided to Child A when Witness 2 used profanity in front of her on 04/11/2025.
