On December 13, 2022 program director, Brianna Moynihan sent licensing consultant, Rachel Arens an incident report. The report outlined an incident that occurred on 12/13/2022 where Child A was left on a community playground unattended for approximately four minutes. The center was notified by a community member on the playground that the child was left when the rest of the class returned to the center. 2 On December 15, 2022, I completed an unannounced onsite inspection at the facility and spoke to Ms. Moynihan and child care staff member (CCSM) Karianne Turner. Ms. Moynihan explained the child care classroom utilizes the Imagination Station playground that is located across the parking lot from the facility. In order to get to the playground, the classroom needs to exit the front doors of the Grand Haven YMCA and walk along the parking lot approximately 1/8 of a mile to the playground. This playground is open to the public. Ms. Moynihan stated she was present at the center on December 13, 2022, standing at the front desk at the Grand Haven YMCA at around 11:00am. She observed the child care class re-enter the building, heading back to their classroom on the lower level of the YMCA. At about 11:02 am a member of the community entered the YMCA and notified the front desk that a child was still on the community playground crying and the community member believed she belonged with the class that just re-entered the building. Ms. Moynihan did not get the name of the community member or any contact information. Ms. Moynihan stated she ran out to the Imagination Station playground and stated she had her eyes on Child A around 11:04 am. When she found Child A, she was not crying, but began to cry when she tried to get her to come inside with her. Ms. Moynihan identified that Child A is not as familiar with her as she with her own caregivers. It took roughly 10 minutes to get Child A to go with her back into the YMCA. Once she got Child A into the building, she brought her down to her office to help her get comfortable and to calm down. Ms. Moynihan explained she returned Child A to her classroom around 11:35 am. She stated CCSM's in the child care classroom were not aware at this time that they were missing Child A. Ms. Moynihan acknowledged that the child care classroom was operating with two CCSM's when they left the building for the playground with seven children and were supposed to re-enter the building with eight children. While on the playground Child A was dropped off by her father at 10:35 am. There was also a staff change while on the playground at 10:50am. CCSM's brought the backpack with the attendance clipboard to the playground with them. Child A's Father documented Child A arriving on the clipboard and then it was placed back in the backpack. Ms. Moynihan stated CCSM's do a roll call/count at every threshold. They also let the front desk at the YMCA know when they leave the facility to go to the community playground. On December 13, 2022, staff told Ms. Moynihan that they completed a head count but did not do a roll call and did not look at the clipboard during the transitions back into the building. Ms. Moynihan stated training on proper transitions with all CCSM's have already taken place and will continue to take place regularly. I spoke to CCSM Karianne Turner who works as a staff member in the child care classroom. Ms. Turner stated she arrived for work on December 13, 2022, at 10:50 am. When she arrived, her classroom was playing on the Imagination Station playground. She entered the playground and the other CCSM left. Ms. Turner did not discuss the day with that CCSM but talked to the other CCSM Kristen Bremer who was still present on the playground. She asked Ms. Bremer how many kids 3 were present, and she stated seven. The attendance clipboard was not reviewed.
