Ms. Alsdurf stated that it was brought to her attention on 03101/2021 that Pu had pushed Child A to a sitting position in crib. a disciplined for the incident. On 03/24/2021, Witness came to Ms. AlsdurWf anstatsed that she was upset because had not been logging in Child B's information correctly on the KinderCare App(an app for parents to view their child's food intake developmental milestones etc.). Witness stated that she was appalled that was still employed at the center after what she had done to Child A on . Ms. Alsdurf asked Witness what she meant, and E Witness stated that had pushed Child A down in crib and that Child ■ A had hit head on the crib. Ms. Alsdurf stated that this was not how the incident owase o e to her. Ms. Alsdurf then spoke with about the incident. admitted that she had shoved Child ac down into the crib using her hand on the Child A's face and that Child A had hit■ head on the crib. Ms. Alsdurf provided documents of previous disciplinary actions against On 03/30/2021 l spoke with Witness, child care staff member, regarding the *aIlleation. She stated that she was initially excited to have Child B cared for by However on March 1, 2021 she was returning to the room and she d take her hand and Child A's head andsloved ■back down in the crib and Child A had hit head on the crib, did not comfort Child A at all and proceeded to ake Child B to the changing table to change the child's diaper. Witness stated that Child A did not suffer any injuries from this incident. Witness denied that she had ever witnessed abuse or handle any child inappropriately. On 04120/2021, was interviewed by at an off-site location regarding the allegation, stated a she did not put her hand on Child A's forehead. Child A was crying prior to pushing She stated that Child A was on knees in the crib when she pushed ■. ■ ■ 3 back. Child A then hit head on the clear plexi-glass of the crib. She denied that Child A cried harder a er was pushed. She told Child A that she was sorry and that she did not mean to do that. Child A did not have any injuries. reported to that this is the only time she ever did that to a child. On 05/25/2021 l spoke with Child As Mother regarding the ation. She stated that Child A did suffer from an occasional bump or bruise on forehead on various dates (unknown) that had given an explanationa*or, however, now Child A's Mother is concerned about -te reason for the injuries. She stated that Child A had not suffered any injuries from the incident in the allegation. Child A~' at the center.

On 05/17/2021 I spoke with Child A's Mother regarding the allegation. She stated that on several occasions Child A would be sent home with dirty clothes, face and/or food in his hair. Child A's Mother stated that she always sent extra clothes with Child A so that it would not occur, however it continued to happen. On 05/25/2021 I spoke with Child B and Child C's Mother regarding the allegation and both indicated that they have never had any concerns with Child B (1 year old, male) and Child C (15-month-old, female) being sent home dirty.

On 03/30/2021 Ms. Alsdurf stated that several of the infant parents had come to her with concerns about Ms. Abrams sending bottles home that should have been fed to their children. Ms. Abrams was either sending false information on the app (KinderCare App by Himama) or she was not sending any information at all. 4 On 03/30/2021 I spoke with Child D's Mother who stated that there was some discrepancy with Ms. Abrams not reporting accurately feedings to parents of the infants. Child D's Mother stated that she herself had noticed that on several occasions she had more bottles at pick up time than what Ms. Abrams was reporting that her child had had during the day. On 05/17/2021 I spoke with Child A's Mother regarding the allegations. She stated that Child A had only been in care at the center for a week and a half but that several days during that time full bottles were sent home that Child A should have consumed during the day. Child A's Mother stated that sometimes Ms. Abrams would share information on the KinderCare App regarding Child A taking bottles and Child A never took the bottle. Ms. Abrams never shared information consistently with Child A's Mother. When Child A informed Ms. Alsdurf about the issues she stated that Ms. Alsdurf had verified that there were other parents with infants at the center that brought concerns to her regarding Ms. Abrams. On 05/25/2021 I spoke with Child B's Mother who stated that Ms. Abrams did not always provide accurate information on the KinderCare App regarding Child B (1 year old, male). She stated that they would often have extra bottles sent home when Ms. Abrams had indicated on the app that Child B had taken the bottle. They would also have other children's information added to their app that did not pertain to Child B. On 5/25/2021 I spoke with Child C's Mother regarding the allegation. She stated that she never had any issues with Ms. Abrams. There were no concerns with the care and supervision that she provided to Child C (15-month-old, female). She stated that appropriate information was added to the KinderCare App for Child C.
