    On May 16, 2023, l was contacted by telephone by Pamela Blaine. She reported that on May 15, 2023, child care staff member 1 (CCSM 1) did not strap Child A into his high chair. As a result, when she removed the tray, Child A fell and hit the floor. Child A's parents were contacted, and they took Child A for medical treatment. Per the parents, they were told to keep an eye on Child A. CCSM1 was so upset after this occurred that she had to leave. On May 16, 2023, l interviewed Child A's father. He reported that Child A had Child A was seen at his pediatrician's office that day and were told that Child"i not have a concussion. Child A was also taken to for a second opinion, and they told him the same thing. Prior to this incident, he did not have any concerns regarding care or supervision of Child A at the center. Child A's father was upset as he believed that they waited approximately 10 to 15 minutes to contact him and did not immediately call 911. He and Child A's mother were able to view the video from that room. On May 18, 2023, I was able to view Child A at his home, along with . Child A appeared to have some We spoke with o , A's mother and father. According to s parents, he was eating well. Child A's father said that the incident occurred at 9:40am and that he was not contacted until 9:59am. On May 18, 2023, and I made an onsite inspection to the child care center !RPneorvieRwRameWa aine. Ms. Blaine described CCSM1 as a wonderful worker. The only issues in the past were that in July of 2022, CCSM1 tripped and fell on a child but that no injuries were reported. She did recall that approximately one year prior, she observed a child in a high chair with the tray attached but that the child did not have their seat belt engaged. She discussed the importance that children must be buckled at all times that they are in high chairs and put a note on the infant room refrigerator reminding caregivers of this. While at the center, we were able to view the video from the classroom on the date of the incident. The infant room is divided into two distinct spaces by a half wall. CCSM1 was on the younger infant side with four infants. CCSM2 was on the other side of the room with four infants. In the video, Child A is in the high chair. CCSM1 comes over and removes the tray from Child A's high chair. When she does this, she turns around to put it on the counter next to her. When she is turned, Child A is seen failing forward The floor in this area is a hard surface. This is at 9:40am. Child A is heard screaming and CCSM1 turns and sees him and immediately picks him up. CCSM1 is seen holding Child A and trying to calm him. CCSM2 is seen getting an ice pack and giving it to CCSM1 at 9:42am. CCSM1 is seen seated and holding Child A and singing to him. At 9:45am, Child A stops crying. CCSM2 called the office and spoke to assistant program director Tracy Ahola, as Ms. Blaine was not present at that time. We spoke to Ms. Ahola while viewing the video. Ms. Ahola acknowledged that she was contacted by CCSM2 but was not informed that it was an emergency. She arrived in the classroom at 9:51 am, per the video. Ms. Ahola told us that she arrived in the classroom, Child A was not crying but she observed a and contacted Child A's father. She then brought Child A to the office and wen Child A's parents arrived at 1 g:1 am, she took Child A to the foyer to meet them, and they requested to vier the video at this time. During the May 18, 2023, onsite inspection, we were able to interview CCSMI. She told us that she thought she had belted Child A in to his high chair but must have just slide him into the seat when the tray was attached. When the incident occurred, she had fed all of the children and was getting them ready for circle time She had placed the three other infants in her area in to bouncers or had them on the floor and Child A was the last child in a high chair.     On March 18, 2022, program director/licensee Pamela Blaine self- reported that Child A's mother informed her that Child A had marks on body and when asked what happened, Child A said that CCSM1 had grabbed She did not find out about it until the end of the day on March 16, 2022, when Chi A's mom contacted her to let her know what Child A said and to show her pictures of the marks on Child A's side that were taken on March 14 and March 15. On March 17, 2022, Ms. Blaine reviewed the video feed of the Preschool classroom in its entirety from March 14, 2022, to see if she could see what happened to Child A. There was only one point in the day that CCSM1 appeared to have been near Child A for around a minute, but because they were both in the house area, which is on the far end of the classroom from the camera, the view was partially blocked by a large doll house. Upon viewing the video, she did not see Child A getting injured in any way that CCSMI had worked at the center for almost 20 years, and she never would have thought CCSM1 would be capable of doing anything to a child. When she initially interviewed CCSM1 on March 17, 2022, she could not recall doing anything to Child A. Later, CCSM1 made the statement that she would never intentionally leave marks on a child. On March 18, 2022, I made an unannounced onsite in ction to the child care center with ameia tsiaine ana sne was eto show ust eonytimein the day on March 14, 2022, that CCSM1 and Child A were in close proximity and that CCSM1 could have grabbed Child A, consistent with the fingernail marks on Child A's torso. The time on the video was approximately 3:30pm. The entire classroom could be viewed from the camera. She identified Child A as the child playing in the house area of the classroom. Another child is seen leaving the area and no other children were present in that area at the time. CCSM1 came over to the house area and can be 3 seen leaning over Child A, who had just sat down in a chair. This lasted for approximately 30 seconds and the view was partially obscured by a large dollhouse. Because the house area of the classroom is far away from the camera, it did not pick up any audio of any conversation between Child A and CCSM1. Ms. Blaine said that she believes Child A was grabbed because Child A told her that was sitting in a chair in the house area when CCSM1 grabbed ■ because ■ was supposed to be cleaning up. She verified that CCSM1 has nails and that this was consistent with the marks on Child A's torso. Ms. Blaine was able to show us the picture of the marks on Child A that were taken the evening of March 14, 2022, when she disclosed to a family member bathing ■ that CCSM1 had grabbed ■ when asked how received the marks. In the picture of Child A's torso, there are five red marks. Four in a row at the bottom and one above it, which would be consistent with adult fingers grabbing this area on a child. Ms. Blaine did not receive this picture until March 17, 2022, and she was not able to interview CCSM1 until this day as well due to CCSM calling in sick the previous day. I was able to confirm through attendance records as well as viewing the video, that there were 10 children in the Preschool classroom on March 14, 2022, and that CCSM1 was the only caregiver in that classroom on that date. Ms. Blaine informed us that CCSM1 did not have any previous write ups and was hired in 2001. While at the center, PR I was able to participate in this as Child A's mother had given permission for -to be interviewed at the center. Child A was able to tell us that CCSM1 was her "teacher" but that she was' ". Child A lifted shirt to E show us a few spots on left that7appeare to a ruises. When as ed about how got the marks, told us that i lean up time and was in the house area. CSM1 put in a chair and told that she needed to e a better listener when she grabbed by the side.
