    On January 26, 2021, I interviewed Child A and B's mother. She stated that on the daily report she received for Child A, it listed bagel and peanut butter for AM snack on January 18, 2021. Child A's Mother stated that while Child A is not allergic to nuts, Child B (male, age 3) is. She was concerned that is Child A received this, Child B may have also. Per Child A and B's mother, the center states that it is peanut free in their handbook. When she spoke to licensee/program director Laura Breakie-Graybill, she said that Child A was fed Wow Butter, which is a nut-free substitute for peanut butter. On January 28, 2021, I made an unannounced onsite inspection to the child care center. I interviewed Laura Breakie-Graybill. She was aware that a child care staff member had noted on a daily report that a child had peanut butter but stated that the child care staff member put the incorrect information on the report. Ms. Breakie-Graybill stated that the center is peanut free and that they serve Wow Butter, which is a nut-free 2 alternative to peanut butter. To her knowledge, there is no peanut butter on the premises. While at the center, Ms. Breakie-Graybill showed me the church kitchen where meals are prepared. The child care has their own large refrigerator and freezer where their food is kept. I observed no peanut butter in the refrigerator and did not see any in the church kitchen area either. I did observe a large container of Wow Butter in the refrigerator. Wow Butter is a peanut free substitute that looks just like peanut butter. Ms. Breakie-Graybill provided me with a menu from the week of January 18-22, 2021 and it showed Wow Butter is listed as an AM snack option. No peanut butter was listed on the menu. While at the center, I spoke with child care staff member Candice Flatter. She stated that she had fed both Child A and Child B AM snack on 1/18/2021. She was aware that Child B is allergic to peanuts. She denied that peanut butter has been served to any children in care. She said that at the center they serve Wow Butter, which looks like peanut butter. This may be why she referred to it as peanut butter and how it got written down as peanut butter on the daily report. She believes that her assistant in the preschool classroom on 1/18/2021 was the one that wrote peanut butter in the daily report. Her assistant was Emily DeWilde. Ms. Flatter verified that she had been trained regarding allergic reactions and had completed health and safety training. I interviewed child care staff member Emily DeWilde during my onsite inspection. She confirmed that she had written peanut butter instead of the Wow butter that had been served to children on 1/18/2021. She denied that she had ever witnessed peanut butter being served at the center and she was aware of Child B's allergy. Ms. DeWilde also confirmed that she had been training on allergies through www.miregistry.com .
