Ms. Lewis did not provide appropriate care and supervision on 09/08/2022 when she poured milk into Child A's mouth resulting in Child A coughing and or choking on the milk. In addition, Ms. Lewis did not provide appropriate care and supervision when she was rough while wiping Child A's face after the incident.

(2)A licensee, licensee designee, or program director shall report to the child's parent and the department, directly or via phone, fax, or email, within 24 hours of the occurrence of any of the following: b An incident involving inappropriate contact.

Ms. Greer failed to report the 09/08/2022 incident to Child A's Mother within 24 hours. Ms. Greer waited four days before reporting the incident to Child A's Mother. In addition, Ms. Greer failed to report the incident to the department within 24 hours of the incident. Ms. Greer waited four days and reported the incident on 09/12/2022.
