On 09/21/2022 I confirmed that Child A's - received a text message from licensee Nicole Proctor, indicating that on 09/20/2022, Child A had a around naptime. Mrs. Proctor observed on Child A's Mrs. Proctor Wimmediately messaged Child A's at 12:37pm including a photo of Child A's Mrs. Proctor confirmed that the children were inside unsupervised when the incident occurred at approximately 12:35pm and no one witnessed the incident. The mark was gone by 1:40pm. Mrs. Proctor reported that Child A and Child B were both in their Pack n Plays, right next to each other, drinking their _ They can stand up. Mrs. Proctor denied that she completed an incident report or sent any information to her child care licensing consultant, Cheree Riley. Mrs. Proctor was advised to complete and incident report and send to Mrs. Riley. Mrs. Proctor was also asked to provide me attendance records and child information cards for the day in question. Attendance records for staff were not available and children attendance records were incomplete. and I then interviewed confirmed she went outside to ask Mrs. Proctor something on the day in question and was not out there more than ■ minutes. When she went back inside she observed Child A to be crying and picked Child A up. She then observed Child A and Child B were each in their own Pack n Play next to each other. stated Child A and Child B did not have Child C im years) was also present that day and was awake when she went in to check on Child A.
