On 0211712021 I com leted an on-site inspection at the Hutchinson child care home with e interviewed icensee Monica Hutthiason. She as been caring for Child or about On 01119/2021 Child A did climb out of pack and play in the living room and fell. Child A cried a little bit and Child A's guar ians were notified ri ht away. Last week Child A got a bruise above knee when ■ was getting out of the high chair. Child A's guardians were no ified right away. Other than ose two incidents there was nothing else significant that happened to Child A recently in child care. She does not know how Child A's injuries occurred. Physical discipline is never used. Redirection is used with Child A currently, as ■ is too = for time out. I On 02119/2021 and I completed a face-to-face interview with Child A's father.Woes"nouse any form of discipline with Child A because of he only sees six hours per week. Child A's maternal grandmother is always present at visitaion but is never left alone with Child A. Child A has not fell during visitation and he does not now how Child A's injuries occurred. He was informed that the bruise on Child A's leg occurred at child care from the high chair. and I completed a face-to-face interview with Child A's Pernagrandmother. She does not know how Child A's injuries occurred. She has been present for every visit with Child A. She has observed no physical abuse of Child A during parenting time. She does not discipline Child A and neither do Child A's mother and father. RREoes On 02/24/2021 and I completed a face-to-face interview with Child A's mother. On 03/05/2021 I reviewed the for Child A's on =,1 and I interviewed Child C years}'s guardian via WeepM one.Wi Masa nded child care with Ms. Hutchiinnssoonn since She has no concerns with the care Child C receives. Child C has come home with a bruise on ear; Ms. Hutchinson told her the screen door hit ■ on the ear. ■ 3 F and I interviewed Child D ,' years)'s mother via telephone. Child D has attended child care with Ms. Hutchinson since was old. She has no concerns with the care Child D receives. Child has had no unexplained injuries while in Ms. Hutchinson's care. ,1 On 03/15/2021 and 03/11/2021 I attempted to contact Child E year) and ,1 Child F years)'s mother via telephone. As of the date of this report I have been unable to reach Child E and F's mother for an interview. I", ' On 03/19/2021 I interviewed Child A and B years)'s guardians. They have no concerns with the care Child A and receive with Ms. Hutchinson. They have had no issues with unexplained injuries to Child A and B in Ms. Hutchinson's care. They do not now how Child A's injuries occurred.
