On 01/19/2023 I received an incident report from Gaylord ECC. On 01/19/2023 Child G told child care staff member Rebekah Kolka he needed to go to the bathroom. Ms. Kolka asked Child G if he went to the bathroom after gym class. Child G said he did. Ms. Kolka did not let Child G use the bathroom. Child care staff member Sara McVannel entered the classroom and smelled urine. Ms. McVannel noticed Child G had peed his pants. On 01/19/2023 I interviewed Child G's mother via telephone. She was notified that of the incident on 01/19/2023. There have been no previous issues with appropriate care and supervision at Gaylord ECC. If Child G says he needs to use the bathroom, then he needs to use the bathroom. 2 On 02/15/2023 I completed an on-site inspection at Gaylord ECC. I interviewed child care staff member Rebekah Kolka. On 01/19/2023 Child G came to her and asked to use the bathroom. It had been roughly 30 minutes since the children had used the bathroom. She said to Child G "we just went, are you sure?" Child G said "no actually I don't have to go". Ms. McVannel found Child G had peed his pants in the dramatic play area. From now on if a child asks to use the bathroom, she will take them to the bathroom. I interviewed child care area manager Brooke Donker. There have been no previous issues with Ms. Kolka. Ms. Kolka did receive disciplinary action from human resources as a result of the incident. From now on if children ask to use the bathroom children will be allowed to use the bathroom. I interviewed child care staff member Sara McVannel. On 01/19/2023 she entered the classroom and sat in the kitchen play area. She smelled urine. She asked Child G if he had an accident and took him to the bathroom to get changed. When she went back into the classroom child care staff member Suzanna Snyder approached her and told her that Child G had asked to use the bathroom and Ms. Kolka refused to allow him to go. She informed Ms. Donker of the incident. She has observed no previous issues with Ms. Kolka. I interviewed child care staff member Suzanna Snyder. On 01/19/2023 Child G asked Ms. Kolka if he could use the bathroom. Ms. Kolka told Child G no that he just used the bathroom and did not need to go again. It had been about 1 hour and 15 minutes since the children had used the bathroom. Ms. McVannel noticed Child G had peed his pants and took him to the bathroom to get changed. Ms. Kolka said she felt bad for not letting Child G go to the bathroom. She has no concerns with the care and supervision Ms. Kolka provides to children. I interviewed child care staff member Katie Milne. On 01/19/2023 she heard Ms. Kolka say that she felt bad for not letting Child G use the bathroom. Child G peed his pants about an hour after the children had used the bathroom. She has no concerns with the care and supervision Ms. Kolka provides to children. On 03/06/2023 I interviewed Child H and Child I's mother via telephone separately. There have been no issues with toileting, care and supervision or otherwise.

On 01/11/2023 I received an incident report from Gaylord ECC. Child A's mother contacted Gaylord ECC to report an incident with Child A. Child A reported that Child B was touching her private parts on the playground slide and on the bus. 2 On 01/18/2023 I interviewed Child A's mother via telephone. Child A came home and said that Child B touched her private area on the bus. Child A reported that Child B put his hand down her pants on the school bus and put his finger in her vagina. Child A's story has not changed, and she does not make things up. She took Child A to the doctor for a yeast infection; she told the doctor what Child A reported. The doctor found no evidence of abuse during his exam. There have been no previous issues and she has no concerns with the care and supervision Child A receives at Gaylord ECC. The child care staff have agreed to keep Child A and Child B separated. On 01/18/2023 I completed an on-site inspection at Gaylord ECC. I interviewed child care staff member Vicki Anthony. She is the bus aide on the bus Child A and Child B ride. The children are buckled into five-point harnesses on the bus. She observes the children from the front seat with her legs in the isle during the bus ride; she has never observed any issues between Child A and Child B. Child A and Child B never sit together and they only ride the bus together in the morning. She has no concerns with the care and supervision children receive on the bus. I interviewed child care staff member Nicole Barraw. She is the lead child care staff member in Child A and Child B's classroom. Child B is just starting to interact with other children and it's mostly boys. Child A and Child B sat at the same table until November; but have not sat together since then. She did observe Child B push Child A one time on the playground slide. Child A was standing and Child B was on his knees behind her; Child B became impatient and pushed Child A. She has observed no other issues between Child A and Child B. She has no concerns with the care and supervision children receive on the playground or in the classroom. I interviewed child care staff member Summer Lawes. Child B usually plays by himself, and Child A and B normally don't play together. She has observed no other issues between Child A and Child B. She has no concerns with the care and supervision children receive on the playground or in the classroom. I interviewed child care staff member Hannah Lochinski. Child A and B normally don't play together. She has observed no other issues between Child A and Child B. She has no concerns with the care and supervision children receive on the playground or in the classroom. On 02/07/2023 I interviewed child care staff member Jennifer Hope via telephone. She is the bus driver on the bus Child A and Child B ride. She reviewed one month of bus tapes and Child A and Child B did not sit together at all. The children are buckled into five-point harnesses on the bus. Ms. Anthony sits in the first bus seat and provides supervision to the children. She has no concerns with the care and supervision children receive on the bus. 3 On 02/07/2023 I interviewed Child C's mother via telephone. Child C (male, 3 years) had had no incidents or issues at Gaylord ECC. She has no concerns with the care and supervision Child C receives. On 02/08/2023 I interviewed Child D and E's mother via telephone. Child D (female, 4 years) and E (male, 4 years)'s ride the bus and there have been no issues. Gaylord ECC has been fantastic. She has no concerns with the care and supervision Child D and Child E receive. On 02/08/2023 I interviewed Child F's mother via telephone. Gaylord ECC has been great. She has no concerns with the care and supervision Child F(female, 3 years) receives. On 02/23/2023 I interviewed Child B's mother via telephone. Gaylord ECC did call and talk to her about the alleged incident between Child A and Child B. Child B has never reported any issues with children at school. She h

On 09/23/2021 an incident was reported via telephone and a written incident report was submitted. I completed an onsite inspection on 09/24/2021 with child care licensing consultant Mr. Josh Stein. There were no children in care at the time of the inspection. 2 We interviewed child care staff member Megan Cottrell from room 6 on 09/24/2021. Ms. Cottrell reported that on 09/22/2021 she and program director Holly St. Amour were taking children to the bathroom. Child B (male, 4 years 6 months) was having issues in the hall and refusing to go with the class. Ms. Cottrell stayed to get Child B under control so they could join the rest of the children. Ms. St. Amour took the remaining children to the bathroom herself. Child care staff member Nicole Barraw came out into the hall from her room to help Ms. Cottrell with Child B. Ms. Cottrell joined Ms. St. Amour at the bathroom and eventually Ms. Barraw and Child B joined them as well. Child B was still having a difficult time, so Ms. Barraw offered to take him back to class. Ms. Cottrell reported there were 12 children left at the bathroom with her and Ms. St. Amour. Ms. St. Amour told Ms. Cottrell to take the children that were finished back to class. Ms. Cottrell took 10 children back to class. When asked if she and Ms. St. Amour communicated with each other how many children each of them had when they split up the group Ms. Cottrell stated she didn't know. Ms. Cottrell returned to the class with the children and they sat down to snack. Ms. St. Amour came back to the class with one child after that. A few minutes later, maybe two minutes tops Heather Proctor head start area manager for rooms 1 through 5 brought Child A back to room 6. Ms. Cottrell confirmed Child A has very limited verbal skills. On 09/24/2021, Ashtyn Flinn, program director in room 3 was interviewed. Ms. Flinn stated she observed Ms. St. Amour on 09/22/2021 leaving the bathroom. Ms. Flinn sent children in to use specific bathroom stalls. The child she sent in to use the stall marked with a yellow circle came back out saying the stall was being used. Ms. Flinn went in to check. She could see feet in the stall, and she tried to talk to the child in the stall, but the child was not responding. She contacted Heather Proctor via telephone who came down to assist. Ms. Flinn had one of the children from her room crawl under the door to unlock it to be sure the child in the stall was ok since the child was not responding. They found Child A inside the stall. Child A had his pants down and was sitting on the toilet going to the bathroom. They shut the door to allow Child A time to finish going to the bathroom. After Child A finished and washed his hands Ms. Proctor took him back to room 6. They knew he belonged to room 6 because he was wearing his nametag on the back of his shirt. We interviewed Heather Proctor on 09/24/2021. Ms. Proctor is the direct supervisor for staff in rooms 1-5. Ms. Proctor stated on 09/22/2021 Child A was left in the restroom unattended. She was called to the bathroom by Ashton Flinn who discovered Child A was left unattended. Ms. Proctor reported Child A was not upset; he was finishing using the bathroom. Child A was not verbally responsive. Ms. Proctor waited for Child A to finish and wash his hands then she walked him back to room 6. When she took Child A into the room Ms. Amour looked and said, oh. The children and child care staff members in room 6 were all at the tables for snack at the time. Ms. Proctor advised Ms. St. Amour that this was a very serious situation. Ms. St. Amour replied, I can see you are disappointed in me and I am sorry. 3 Ms. Proctor returned to her office and emailed Ms. St. Amour's direct supervisor area manager Amanda Gascho and regional manager Paul Brown. I requested a copy of Ms. Proctor's email which she provided.
