Based on the interview of CCSM 1, CCSM 3, and the video footage viewed the center staff did not provide appropriate supervision of Child A when he walked out the building and was found and returned into the building by his mother. Child A was unsupervised for one minute and 59 seconds.

On 05/18/2022, I conducted a telephone interview with Child A's Mother. On 05/16/2022 Child A's Mother was contacted by CCSM Julia Blue and informed that Child A was throwing toys, when redirected he became angry and ran into the window. After running into the window, he had an open wound above his eye. Ms. Blue sent Child A's Mother a picture of the wound. Child A's Mother decided to come and pick him up to take him to urgent care. Child A's Mother took him to one urgent care that offered just a butterfly bandage. Child A's Mother was worried that it would come off and felt he needed something more stable. Therefore, she took him to another urgent care and was provided a liquid bandage to close the wound. Child A returned to the center the day after and continues to attend the center. Child A's Mother feels that the story does not explain how Child A would get an open wound. Additionally, Child A's Mother felt that the stories she received from the child care staff members varied in where the incident occurred. Ms. Blue informed her that the incident occurred in house area. However, Child A's Mother was contacted by CCSM Candasahia Reynolds who works as a family advocate. Ms. Reynolds called to ensure that all was well with Child A and told Child A's Mother the incident occurred in the wardrobe area. Ms. Reynolds also said that when Child A was redirected someone grabbed him and he snatched away and then he fell and hit his head. Child A's Mother also feels that there have been too many incidents or occurrences of injuries. In the past month, Child A has had several incidents with a particular child which have resulted in Child A having scratches or a knot on his head. On 05/20/2022, I completed an unannounced inspection at Wayne Metro Head Start Garden City. I interviewed program director Sigourney White, CCSM's Julia Blue, Rebecca Macinnes, and Heyam Alawamleh. Program director, Sigourney White stated she was informed that an incident had occurred through text by Ms. Blue. Ms. White was not in the building when the incident occurred. Ms. White called Ms. Blue to get clarification and she explained that during a transition time from work to clean up Child A began throwing toys in the house area instead of cleaning. Ms. Blue looked back to see if a toy Child A had thrown hit another child and when she looked back Child A was bleeding from an open wound. Ms. Blue provided first aid, then called and text Child A's Mother. Child A's Mother picked him up and took him to urgent care. Ms. White feels that the conflicting information regarding the incident is her fault. Ms. White informed Child A's Mother and Ms. Reynolds that the incident occurred in the wardrobe area. Ms. White stated that when she explained the incident, she mistakenly kept referring to the area as the wardrobe area. The house area and wardrobe areas are next to each other in Child A's room. Child A returned to the center the next day and continues to attend. There have been other incidents in which Child A and another child have been aggressive towards each other. Staff in Child A's room are 3 constantly redirecting children and discouraging aggressive behavior. Ms. White stated that Child A's room has children with challenging behaviors. Child A is in the Great Start to Readiness program (GSRP). The GSRP program is a federally funded program that does not allow for children to be expelled due to behavior. The center has one GSRP room making them unable to offer a different classroom to Child A. There are three staff members in the room and 15 children enrolled. The center does use various strategies to help children who are displaying challenging behavior. After this incident, the staff has begun to shadow children who are displaying challenging behavior in hopes of catching it before it occurs.
