On 03/09/2022, I completed an unannounced inspection at It Takes A Village Academy. I spoke to the licensee and program director, Suzanne Collard, office administrator, Joanne Makki, and child care staff members, Karen Hudson and Tiffany Blackburn regarding the allegations. I observed the older toddler class and Child A was present. There were 10 children, ages 1 ': to 2 '1z years, with three child care staff members. The incident reports from 06/1112021, 02/2512022, family referrals, and child information cards were provided for review. According to Ms. Collard, Child A has been at the child care center since he was Child A is currently in the older toddler class. Child A does not talk and started waikinQ1"ve-,;,,~ review Child As file and she located two incident reports, dated 06/11/2021 and 02125/2022. According to Ms. Collard, the first incident occurred on 06/11/2021, when Child A was in the infant room. Ms. Collard stated that she was in the classroom when the incident occurred. She stated that Child A was crawling on the floor then he stopped. No one, including child care staff members or a child, touched him. When they went over to him and touched his arm he started to cry. Child A fell asleep and was placed in the crib. After he woke up is when they noticed something was wrong. Child A's parents were contacted and informed of their concerns. Child A was picked up. Child A returned to the center, and she was told that it was the r Ms. Collard stated that she was unsure if Child A was taken to the doctor. Documentation was never provided by Child A's parents. Ms. Collard said she notify the department about the incident via email. On 02/25/2022, Ms. Collards was told that Ms. Makki was reading a story on the carpet. Child A got up and his arm was hanging. Ms. Hudson came and said something to her, and an incident report was written. Ms. Collard contacted Child A's parents immediately. Child A's Father came to the child care center angry. Ms. Collard was able to talk to him and calm him down. Child A's Father took Child A. Child A returned three or four days later, and Ms. Collard stated that she did not know if Child A received any medical treatment. According to Ms. Makki, she is the office administrator. Ms. Makki stated that she was not aware of the June or October 2021 incident with Child A. On 02/25/2022, she was in the older toddler room providing care due to Ms. Collard meeting with the toddler room child care staff members. She was in the classroom for about an hour and Ms. Hudson was in the classroom with her. She started circle time (i.e., read a story, finger puppets, nursery rhymes, etc.) with the children. Child A just stood and watch and did not come over to the carpet. Child A seemed tired. She asked Ms. Hudson had he been like that all morning, and she said yes. Ms. Makki asked Ms. Hudson to bring Child A to the group. Ms. Hudson touched his hand and he seemed uncomfortable. She then ran her hand down his arm, and he started to cry. Ms. Hudson guided Child A over to the carpet. Ms. Makki asked Ms. Hudson to let the office know about Child A's arm. After circle time, the children went into the hallway to ride bikes and blow bubbles. Child A did not participate. He just stood and observed. Ms. Makki stated that Ms. Hudson said he came in like that. Child A was picked up and he returned to the center on 03/03/2022. Ms. Makki stated that the children are not disciplined at the center. They use redirection with the children and each classroom has a cool-down area if needed as a last resort. According to Ms. Hudson, she is a floater at the child care center. She stated that she was unaware of any incident in June or October 2021. She was working as a cook and not a childcare staff member at that time. Ms. Hudson stated that they separate the children if there is a concern and then talk to them about the issues or concerns as a discipline. She stated that she was in the older toddler classroom all day on 02/25
