On 12/9/2020, I interviewed complainant. Complainant stated that Child A (one-year-old male) was enrolled at the child care home in August, 2020. In the short period of being enrolled, Child A was taken to urgent care several times. The last incident occurred on 11/6/2020 around 10:30 a.m. when Child A fell into a wall that caused a huge bump on his forehead. Complainant stated that 2 licensee, Dawn Barnes notified Child A's Father as soon as the injury occurred. Child A was taken to urgent care regarding the injury. Ms. Barnes also sent a photo of the injury. Complainant informed me that Ms. Barnes stated that she reported the incident to her licensing consultant on an incident reporting form. Complainant stated that Ms. Barnes was not present when Child A was injured. Child A was in the care of child care staff member, Kendra Russek and she informed Ms. Barnes of the injury. Complainant stated it looked as if Child A was pushed instead of him falling on his own to sustain the type of injury. Complainant does not think Child A is being cared for properly due to the illnesses and injuries in the short period enrolled at the home. On 12/9/2020, I interviewed Ms. Barnes at the child care home. Ms. Barnes stated that she went upstairs to use the bathroom. The children were downstairs with Ms. Russek. Ms. Barnes stated that she hurried back downstairs. Ms. Russek informed Ms. Barnes Child A was walking and lost his balance hitting his head into the corner of the wall near the gate at the stairs. Ms. Barnes stated that she contacted Child's A Parents as soon as it happened. She stated that Child A is a new walker, and he falls a lot from losing his balance. Ms. Barnes stated that Child A was picked up and taken to urgent care. She stated that Child A's Father was upset about the accident with Child A's recent injury to his forehead. During my inspection, I noticed there was a dip underneath the carpet near the location where the injury occurred. Ms. Barnes stated that it was a covered drain. On 12/10/2020, I interviewed Child B's (two-year-old female) Mother. Child B's Mother stated Ms. Barnes is very good about letting her know if Child B has a scratch or has been injured while in her care. She stated Child B has been enrolled for over a year and Ms. Barnes has always been informative in keeping her aware of Child B's well-being. On 12/11/2020, I interviewed Child C's (two-year-old female) Mother. Child C's Mother stated that she does not have any concerns with Ms. Barnes. She stated Ms. Barnes is amazing with Child C. Child C's Mother stated that Ms. Barnes is very careful when children are ill and send them home.
Child A (one-year-old male) became sick quite often in a short period of time enrolled at the home. He fell into a wall and forehead received a large bump. There is a boat in the outdoor play area.

On 12/9/2020, I interviewed Complainant. Complainant stated that there is a boat in the outdoor play area of the child care home. Complainant is concerned of the safety of the children when playing outdoors. Complainant does not think that a boat should be kept in the area located where children are playing. On 12/9/2020, I interviewed Ms. Barnes. Ms. Barnes stated that the boat is on the opposite side of the outdoor play area. She stated that the children are supervised and do not play anywhere near the boat. I observed the boat in the backyard. It did not appear to be unsafe or accessible to the children based on the location and the location of the play area.
