On 05/12/2021, I interviewed Child A's Mother. She is happy with the facility and believes they provide "good care" of the children. The center notified her immediately about the incident and was very apologetic. She states, "Accidents happen and they did nothing on purpose. He was just fine. The play area is fenced and he was a little scared, but I'm not worried about it." On 05/17/2021, I conducted an unannounced inspection at the facility and spoke to the director and inspected the outdoor play area. It's a fenced courtyard adjacent to the building and referred to as the Kiwanis Play Area. Program Director, Mrs. Susan Ford was not present when the incident took place, but she interviewed all staff involved and they provided her with details. A review of attendance records indicates there were nine children, four-years-old, and two staff in the classroom the day of the incident. A staff found him outside and another walked him to his classroom. Mrs. Ford fully expects staff to complete a face to name count when heading to the playground and when returning to the building. Children are expected to be supervised and never left outside. She also stated, both staff should be going outside with children and new procedures have been put in place to ensure it takes place. The assistant assigned to the room will not take her lunch until the children return from the outside play, thereby allowing her to go outside and help with supervisions. On 06/14/2021, I attempted to interview the building custodian. He has not returned my call but provided Mrs. Ford the following account of what he observed the day of the incident. He was walking down a hallway and noticed the door leading to the playground was propped open. He heard a child crying and walked outside toward the sound. He observed Child A inside the Kiwanis Town play area. He immediately alerted the teacher whose classroom is closest to the door about what he had seen. She asked him to watch her class and she ran outside to get Child A. On 06/14/2021, I interviewed by telephone. She was attending to her kindergarten class when came in and explained what he observed outside. She requested he watch the children and she ran outside, unlatched the ate to the play area and obtained Child A. He was crying and appeared scared. walked him back into the building and asked if anyone know where he belonged. She was told, "He's in Loritz room." As she walked toward the classroom, Mrs. Loritz walked out and asked her, "What happened." She informed her of Child A being found outside. She is not certain how long Child A was outside, but once she became aware of the situation, it was no more than a minute. Child Care Staff Member, Bethany Loritz is the lead teacher in Child A's room. She stated, "I made a lot of blunders that day and feel really bad. We're required to use 3 the face to name procedure when leaving the building, before returning and once we return." Mrs. Loritz did not count the children when they entered the building the day of the incident." She took the children out to play at approximately 01:30. At 02:00 p.m., she requested the children line up against the fence to begin entering the building. As the children lined up, she checked off the transition sheet and accounted for all nine children. As they begin to exit the play yard Mrs. Loritz became distracted by another child and was dealing with the situation. She last remembers Child A getting out of line to possibly collect a toy. She thought he had returned to the line. Once they reentered the building, she allowed children to obtain their "rest bag" and use the bathroom, which is not the typical routine. Routinely, children are made to stand in front of their locker and line up so they could use the bathroom before taking their nap. The assistance caregiver was at lunch at the time of the incident.
