On 11/05/2020, I spoke to the complainant, who stated they had concerns with the center due to the infant classroom not providing parents with daily sheets to let them know how much food the babies are eating. The complainant stated that the food provided to the children is food parents bring in, but it is not getting fed to the children; and if it is, it is not written on the daily sheets. The complainant did not have any further information. On 11/05/2020, I spoke to Child A's mother via telephone, and she stated that Child A is nine months old and used to attend Learn & Grow; however, Child A does not attend anymore and her last day was 10/30/2020. Child A's mother stated that the child care staff members in the infant classroom would say Child A slept a lot during the times she was there and would only have on the daily sheet one diaper change and that Child A drank one bottle for a six to seven hour day. Child A's mother stated she used to be employed at the center while Child A attended and that she never received a daily sheet during the times she was there. She said she has spoken to the lead caregiver Alex Provost about the concerns and was told that Child A does eat at each mealtime. However, Child A's mother still does not believe that Child A eats because she will bring three bottles every day and is supposed to be given to Child A every four hours. Child A's mother stated that it would vary how many bottles would be left when she picked her up. She stated that when she would arrive at 7:30 AM the staff members would try to feed Child A at 9:00 AM and she would not be hungry yet and they should not have fed her. When asked if she talked to anyone about this concern, she stated she had not. I asked Child A's mother how she knew Child A was fed at 9:00 AM if it was not written on the sheet, and she stated she did not know this answer but knew that they had tried feeding her. She then stated she knew because staff members would "probably have told her". Child A's mother stated she never asked for a daily sheet or talked about the concerns with Ms. Braidwood or the staff members in the infant room. On 11/10/2020, I made an unannounced inspection at Learn & Grow Daycare and interviewed program director Holly Braidwood and staff members Alex Provost and Kaila Jones. First, I interviewed Ms. Braidwood regarding the allegations, and she stated that daily sheets are completed each day in the infant classroom and the staff members in the infant room are "anal about this". Ms. Braidwood stated that Child A was a very picky eater and would not eat the food that Child A's mother brought. Ms. Braidwood and staff members would talk to Child A's mother about this all the time and she never would change the food she brought. Ms. Braidwood stated the infant classroom staff members were worried that Child A's mother would call in a complaint due to the issues they had with her as an employee and would always make sure that Child A had a daily sheet and everything filled out right. 3 Next, I spoke to Alex Provost and Kaila Jones while observing them in the classroom. Once I walked into the room, mounted on the wall were the children's daily sheets filled out for the morning. There were seven children, and all the children had a form on the wall and were filled out for the morning hours. Ms. Provost stated that the sheets get filled out as they go throughout the day and then they go straight to the children's bags before the staff walk the child to their parent at the door. All children get a daily sheet each day and they make sure they are completely filled out. Ms. Provost stated that some of the children are picky eaters or will not finish a bottle and they will still write on the sheet how much of the food or bottle they ate. On 11/05/2020, I spoke to the complainant via telephone and she stated that she has seen children propped with a bottle in a swing. The complainant did not have any names of children for which this was happening. On 11/05/2020, I spoke to Child A's mother via telephone and she stated Child A is put in a swing for nap and the child care staff members will prop her bottle with a blanket. Child A's mother stated Child A is nine months old and can hold her bottle, but this still happens. Child A's mother stated she has not seen this happen she just knows this is how she sleeps. She did not have any further information on this and did not know which child care staff members put Child A in a swing and propped the bottle. On 11/10/2020, I made an unannounced onsite inspection at Learn & Grow Daycare and spoke to Ms. Braidwood, who stated that all of the children but one in the infant room do not hold their own bottle or drink their bottles at the table, and that is Child B. Child B is always held for all feedings and with bottles. She stated she makes sure no child is ever in the swing with a bottle and the staff are very good at not allowing children to even sleep in the swing. Next, I spoke to Ms. Provost and she denied ever putting a baby in a swing with a bottle propped. She stated that the only baby who is that young is Child B and Child B is always held for each feeding. Ms. Provost stated that the other children sit at the table in the seats for each meal and do not need help holding a bottle or even sit in the swing. 5 I then spoke to Ms. Jones who stated the same as Ms. Provost and denied ever propping a child with their bottle. She stated Child A would never have allowed her bottle to be propped and in the swing. Child A spit up a lot and needed to be held at all times. Ms. Jones stated that Child A was in her primary care group and she always held her for each feeding. During my observation, I did not see any child go into the swing or have a bottle propped. Child B was fed and held the whole time during the feeding. No concerns were presented. On 11/17/2020, I spoke to Child C's mother via telephone and she stated that her child does not go in a swing and can hold her own bottle, so she does not believe they would ever prop Child C's bottle. She stated that she is not allowed in the building due to Covid-19, but prior to Covid-19, she never walked in and saw any child in a swing with a bottle. On 11/17/2020, I spoke to Child B's mother via telephone and she stated that she has never seen or heard of any child care staff members prop a bottle in a swing with any of the children. She stated she feels if Child B had a bottle propped, she would spit up a lot and she does not do this. Child B can go all day in the same clothes, and they are not wet. Child B's mother stated child care staff members send pictures of Child B throughout the day, and she has never been in the swing or seen other children in the swing. She stated she does not have any concerns with this.

On 09/10/2020, I made an unannounced onsite inspection at Learn & Grow Daycare and interviewed program director Holly Braidwood. I informed her of the allegations, and she stated that Hunter Lako works Monday through Friday from 12:00 PM- 6:00 PM, but the children sleep until about 3:00 PM, so Ms. Lako is only working with the children for about three hours in the evening. Ms. Lako takes over for Ms. Braidwood in the preschool classroom each day. Ms. Braidwood stated that it is Ms. Lako's job to talk to the parents in the evening and let them know how their child's day went. Ms. Braidwood stated she has a notebook in her classroom to exchange notes for the staff, so they the information about each child's day. Ms. Lako will go over it when she arrives, and anything that is needed to be passed on to a parent, she takes care of it. Ms. Braidwood stated that Child A has some behavioral issues in the classroom, and it is important that each day we let his mother know how his day has been. Child A's mother spoke to Ms. Braidwood last week about Ms. Lako always having something negative to say about Child A; however, they all met together and decided on a way for navigating Child A's behavior and everyone is "good now." They discussed that Ms. Braidwood works with Child A in the morning and Ms. Lako will be the staff member to work with Child A in the evening and discuss everything with Child A's mother at pick up. Child A's mother never complained of Ms. Lako harming her child or being "mean", she was just struggling with picking Child A up every day and hearing that Child A had a bad day. Ms. Lako has only been working in this room for about four months and is still learning each day how to connect with all the children in the classroom. Ms. Braidwood stated she has never had any concerns with Ms. Lako and her behavior with the children. She stated she is soft spoken and has had to talk to her about being sterner sometimes with children that are strong-willed. Ms. Lako is 21 years old and does not have children of her own, so she is just learning how to handle children with behavioral issues. Ms. Braidwood stated she has been working with her a lot on how to react to certain situations and how to handle children that need a little more work. Child A runs out of the classroom a lot and has "melt downs" when he gets upset. Child A does not have a written behavioral plan at this time because all staff members and Child A's mother have been working together on making sure they all stay consistent with him. Ms. Braidwood stated that last week there was an incident with Child A hitting other children with his hat and hurting them. Ms. Braidwood stated she took the hat away from Child A and put it in her office and then talked to Child A's mother about what happened. Child A's mother was not concerned about this and Child A got his hat 3 back at the end of the day. Child A is at the center every day until about 5:30 PM. There have been only two times Child A was picked up early and it was due to Child A's mother getting out of work early and not due to being concerned with Child A's well-being at the center. Ms. Braidwood stated she has never seen Ms. Lako put any child on the wall for a timeout. She stated the only time the children sit on the wall is when she takes them to the bathroom or to get ready to go outside. It is not a disciplinary issue. Ms. Braidwood then stated that she thinks the allegations against Ms. Lako, unfortunately, is coming from some staff altercations that have been going on. She stated that Beth Woolworth works with the school agers in the room next to Ms. Lako and for some reason she does not think they get along very well. She stated she thinks it's more of just an age difference and they do not relate. Ms. Braidwood stated she spoke to both staff about their differences and has reminded Ms. Woolworth she needs to help Ms. Lako and work together.
