On 02/02/2022, I received a copy of an incident report dated 01/26/2022. The incident report stated the following: "After lunch, the children were preparing to lay down for rest time. Child A was laying down on her cot. The teachers were working with other children helping them to get ready to lay down. Child A approached Ms. Potter and told her that another child had hit her in the eye. The other child was holding a plastic toy person. Child A's eyelid was bleeding from a small cut on her eyelid. She was cleaned up and ice was applied to the eyelid. Her mother was called and she was picked up about 20 minutes later. Child A's Mother said she would let the school know if she took her to the doctor. " On 02/03/2022, I spoke with Ms. Crystal Potter by telephone. Ms. Potter stated as the children were getting ready for nap, right after lunch. She was assisting a child it settling down for a nap. Child A walked up to her crying and told her she was hit by Child B. Child A was having difficulty in calming herself down. The lights were off so she viewed the injury with the flashlight from the phone. Child A had been injured on the left eyelid and was bleeding. Child A was immediately provided first aid to address the injury and Child A's Mother was contacted. When Child A's Mother arrived at the center the bleeding had stopped. Ms. Potter explained that Child B has a lot of behavior issues since he has been enrolled in the center since 2021. Child B was not where he was supposed to be by his cot during nap time and was on the other side of the room. Child B has a history of hitting, throwing items/toys, screams, flips furniture, and difficult to redirect. The center has been waiting since last year (October 2021) for someone to come out and assess him from Flint School District. Child B was out for a couple of months at towards the end of the year due to Covid-19 and came back a week before Christmas. It has only been since last week when he was assessed for speech but was still waiting for the assessment for his behaviors. Since this incident between Child A and Child B; Child B is more on a one to one with the child care staff members and if the need arises the classroom is evacuated if his behaviors is too extreme. Child A did return the very next day and observed that there was some bruising around her eye. On 02/09/2022, I made an onsite investigation at the center. I interviewed Ms. Kristen Dillard, program director, Ms. Crystal Potter, Nedia Triplett, and Anna Sheveleva, child care staff members. I interviewed Ms. Dillard and Ms. Potter. 3 expressed my concerns about the timeframe of how long Child B's behaviors have been going on in the room and the impact and now the injury occurring with Child A. Ms. Dillard stated there was a delay in getting Child B assess due to a shortage of staff in support services from the Flint School District. Child B's Mother is open for services and assistance. The social and emotional specialist for Child B was coming every other week but now is coming out every week since the incident occurred. Ms. Potter explained that they are now trying new ideas with Child B to negate some of the behaviors he usually has in the classroom. The child care staff members start Child B's Day with sensory table as he seems to really enjoy doing that, last week started a schedule with child care staff member to give Child B one on one blocks of time. It works well when there are three child care staff members in the room but can be difficult when there are just two in room. Transitions and not getting his way appear to be a trigger for Child B and this is when he will act out the most of throwing toys, hitting children, climbing on furniture, and running around the room and out the classroom.
