On 04/13/2022, I received a telephone call from program director/licensee designee Sarah Shaw who reported that on 04/12/2022 Child A ingested wood chips while on the playground and Child A's Mother notified her that he vomited them up at home. His parents took him for medical treatment at Hurley Medical Center where he received x-rays and no injuries were obtained. She called Child A's Mother on 04/13/2022 and was notified that once Child A was cleared by his pediatrician he would return to the center. Later that day, I received the written incident report via email from Ms. Shaw. On 04/13/2022, I interviewed Child A's Mother and Child A's Father who confirmed that he vomited up woodchips when he arrived home from the center on 04/12/2022. Child A's Mother called his pediatrician who advised that they seek medical treatment at the hospital. They took Child A to Hurley Medical Center, x-rays were completed, and they discharged him with no injuries or foreign bodies. They advised that they follow-up with his pediatrician. They confirmed that 04/12/2022 was Child A's first day at the center and he would not have been exposed to woodchips prior. They had concerns with the amount of time he was possibly left unsupervised to gnaw the woodchips long enough to break them down for swallowing. An unknown teacher told them that she saw Child A putting woodchips in his mouth three separate times and each time they approached him to remove the woodchips from his mouth. On 04/14/2022, I completed an unannounced onsite inspection at the center from 11:15 am — 1:00 pm and interviewed assistant program director Val Tolene and child care staff members (CCSM) Lorna Baird and Angel Dixon. Licensee designee Dawn Linsenman was present and confirmed that the cameras do not cover the playground. Ms. Linsenman stated that a work order would be completed to remove the woodchips from the playground. I observed that the playground did not have any equipment with elevated surfaces, therefore ground cover was not required. Ms. Tolene provided me with a copy of a written statement regarding her involvement with the incident. She explained that she received a telephone call from Child A's Mother at approximately 5:30 pm on 04/12/2022 notifying that he vomited up wood at home. She text messaged her a photograph of the vomit and stated that she was taking him to the hospital. Ms. Tolene then notified Ms. Shaw of the incident via a telephone call. She also called Ms. Baird, who explained the outdoor playtime for the day and then called Ms. Shaw and provided an update. Ms. Tolene clarified that Ms. Baird told her that she observed Child A putting woodchips in his mouth a few times but intervened each time and swept his mouth to get any remaining pieces of the woodchips out. Ms. Baird confirmed that she worked with Child A on 04/12/2022 and they went to the playground three separate times that day. She provided me with a copy of a 3 written statement regarding her involvement with the incident. She clarified that a lot of the children put woodchips in their mouths initially, yet once they intervene a few times and redirect them it is no longer an issue. She confirmed that Child A's first day in attendance at the center was that day. She is not aware of any other child ingesting the woodchips. She removed woodchips from Child A's mouth twice on that day and Ms. Dahlin did once. Each time she removed the woodchips, Child A reached and grabbed another handful and placed in his mouth. She even lifted him off of the woodchips to prevent this. If she were aware that Child A ingested the woodchips, she would have notified his parents. Ms. Dixon confirmed that she worked with Child A on 04/12/2022 and they went out to the playground three separate times that day. On 07/01/2021, program director/licensee designee Sarah Shaw left me a voicemail message stating that on 06/30/2021 two children pulled the pants down of another child and then touched the child's bare bottom. I was on annual leave and did not receive the voicemail message from Ms. Shaw until my return on 07/08/2021. At this time, I also received an email from Ms. Shaw with the completed written Incident Report. I called Ms. Shaw who clarified that Child A (male, 5 years) pulled Child B's (male, 5 years) pants and underwear down while they were on the playground. Child A and Child C (male, 5 years) then touched Child B's bare bottom. I notified Ms. Shaw that an investigation would be opened and that she needed to report the allegations to the Michigan Department of Health and Human Services. On 07/09/2021, I completed an unannounced onsite inspection at the center from 12:15 pm — 2:00 pm and interviewed Ms. Shaw, assistant program director Valerie 611 W. OTTAWA. P.O. BOX 30664. LANSING, MICHIGAN 48909 www.michigan.gov/lara . 517-335-1980 Tolene, and child care staff members Cathi David, Mikayla McAbee, and Emilee Vantol. Ms. McAbee and Ms. David were interviewed separately and confirmed that they worked with Child A, Child B, and Child C on 06/30/2021. They recalled Child D (female, 5 years) approaching both of them (separately) on the playground and telling them that Child A and Child C came to her and asked her if she would pull her pants down. Child D told Child A and Child C "no." Ms. David was notified first and had Child D then notify Ms. McAbee who addressed with Child A and Child C who both reported that they were "only joking." At this time, Ms. David, called Ms. McAbee over and told her that Child B just notified her that Child A and Child C had pulled his pants down and touched his bare bottom. Ms. David had been by the tree and left momentarily to assist another child with the application of a Band-Aid. Ms. McAbee and Ms. David recalled that Ms. Shaw responded quickly after being notified of what occurred and came to the playground and intervened by speaking with all of the children inside the center. Neither Ms. McAbee nor Ms. David spoke with any of the children's parents. Both clarified that this happened near the large tree on the playground and both were nearby. Ms. McAbee and Ms. David confirmed that they were within the appropriate caregiver to child ratio as the two of them were caring for 17 children, the youngest being 4 years of age. I then was notified that child care staff member Emilee Vantol was also on the playground, since it was her first day and she was being supervised by both Ms. McAbee and Ms. David. Ms. Vantol confirmed that she was never allowed unsupervised contact with the children on 06/30/2021. She was nearby Ms. David when Child D notified her that Child A and Child C asked her to pull down her pants and told them "No." She observed Child A and Child C hiding behind the large tree and Ms. McAbee went and called them to her. She was not involved in any other incident. Ms. Tolene became involved on 06/30/2021 when she was walking a parent back to the playground and Ms. David approached her and notified of what occurred and that Ms. Shas was also notified. She observed Ms. Shaw arrive and handle the incident. She did not speak to any of the parents of the children. Ms. Shaw explained on 06/30/2021, she had just returned to the center and was notified by Ms. Tolene to meet her on the playground. She noticed Ms. Tolene speaking with Ms. David who then reported the incident to her. Ms. Shaw confirmed that there are no cameras on the playground and that she has placed a work order to have them installed. I was shown the large tree on the playground and observed where both Ms. McAbee and Ms. David were during the incident.
