On 06/16/2023 and 06/20/2023 I attempted calling the complaint source but was unable to leave voicemail messages. On 06/21/2023, I emailed the complaint source, providing my contact information, and requesting a telephone call. Later, I received a telephone call from the complaint source who stated that Child A's Grandmother notified her that Child A is in the highchair often to control his behavior. Child A's Grandmother also informed her that the center told her that if he is out of the highchair for more than 20 minutes, he destroys the entire center. She was at the center recently and observed Child A in an unknown CCSM's lap. Child A's Grandmother informed her that if she shows up at the center unexpected, he will be in a highchair. On 06/22/2023, I completed an unannounced onsite inspection at the center from 10:25 am - 11 am and interviewed Ms. Keels (via telephone) and CCSMs 1 and 2. Upon my arrival, I observed each room within the appropriate CCSM to child ratios. CCSM 1 stated that she was covering for Ms. Keels, as she was not present. I had her call Ms. Keels to see if she could come to the center. CCSM 1 had me speak with Ms. Keels, who stated that Child A has some special service providers that some to the center. She has never had a conversation with one of these service providers or Child A's Grandmother regarding them placing Child A in a highchair to control his behavior. She denied that they place in in a highchair, other than occasionally for eating. Usually, he will eat a meal at the table with the other children. She confirmed that CCSM 1 and 2 are the only ones that care for Child A. On 06/22/2023, I interviewed CCSM 1 and CCSM 2, separately. Both denied the allegations. They never observed Child A in a highchair other than for eating. Usually, he eats at the table. Neither had a conversation with a special service provider or Child A's Grandmother regarding them placing Child A in a highchair to control his behavior. No other CCSM cares for Child A. They did not have access to the videos. They will have Ms. Keels follow-up with me for viewing of such. On 08/14/2023, I called the center and left a message for Ms. Keels to return my call. Later, she returned my call and stated that her CCSMs misunderstood me, and she was not aware that she needed to follow-up with me. She confirmed that the videos are on a live feed and do not tape, so there is no footage for me to view. 3 On 08/14/2023, I left a voicemail message for Child A's Grandmother. Later, she returned my call and reported that she does not have any concerns regarding the center and Child A continues to attend. She has never picked Child A up from the center and observed him in a highchair. She understands that he occasionally eats in the highchair but is not placed there for any other purpose.

On 4/26/2022, 1 completed an unannounced onsite inspection and met with licensee designee LaMarcus Keels. LaMarcus Keels and Charlotte Keels have had this center for five years and have never had to register with MCIR. They did not know it was a requirement. They did receive a message from child care staff member Bryce Keels (CCSM 1) who was at the center when the Genesee County Health Department called. They also received the email which was sent to CCSM 1 indicating the steps they needed to follow to register with MCIR. They had not followed up as of yet but will do so immediately. They provided the name of the Genesee County Health Department employee who reached out, as Lori Popyck. The forms have been printed and will be sent tomorrow AM to complete the registration portion. On 4/27/2022, I attempted to call Genesee County Health Department, and left a voice mail requesting a call back. On 4/27/2022, I received a voicemail from Genesee County Health Department. On 4/27/2022, I called Genesee County Health Department, interview was conducted with Lori Popyck. There is a requirement for all centers to register with MCIR, and upload the immunization records into the portal, essentially building what they call a roster. They have been contacted on multiple occasions requesting they register on the site, so that the roster can be built. She offered to build the roster for them, however, they had to register for her to be able to do that. She is not sure why the other child care center has never had to complete this process. She is not sure why all centers do not do this automatically when they open, however they usually only find out randomly. There is not a system which sends them a listing of all child care centers who do not have a MCIR registration active. Sometimes they locate these centers who are not registered by happenchance. They might drive around and if the center name is not familiar, they check the registry to see if it's in there and if not, they make attempts to get the center to register. Sometimes they go through the LARA website checking that way, and on occasion they have received names from state representatives. They have reached out to the center since October of 2021, spoke with CCSM 1 and emailed the center the steps to register. 3 On 4/27/2022, I received an email from Mr. Keels the registration was completed. On 4/28/2022, I received an email from MDHHS the registration needed corrections. On 5/2/2022, I received an email from Mr. Keels the corrections had been completed. On 5/20/2022, I emailed Genesee County Health Department, inquiring if the registration was completed. On 5/20/2022, I received an email confirmation the center has completed the required registration with MCIR. On 5/20/2022, I emailed Mr. Keels regarding having an exit conference. On 5/26/2022, I conducted an exit conference with Mr. Keels.
