    On 05111/2023, 1 spoke with and coordinated time Weo compete an unannounced onsite inspection at the center. On 05/12/2023, and I completed an unannounced onsite inspection at the center fromW1Oam — 1:30 pm. Together we interviewed child care staff members (CCSM) 1-6, licensees Walishya and Dietrich White. Ms. W. White stated that Child A was only at the center for approximately 3 hours on 0510812023. She confirmed via the Brightwheel application that he arrived at 8:30 am and left at 12:45 pm. She added that she observed that Child A was cranky on 0510812023, however it may have been expected as it was his first day transitioning from the infant room into the toddler room. None of the CCSM's have ever been disciplined for inappropriate discipline toward the children nor has she ever had a concern for such. She has never received any parent concerns regarding inappropriate discipline use from the CCSM's. CCSM's complete a health check with all children at the time of arrival to the center. The CCSM meets the parent outside of the center and retrieves the child and brings into the center. The CCSM visually looks over the child, while the parent is on the other side of the glass outside of the center. If any marks are observed, they address it with the parent prior to accepting the child for the day. CCSM1 was the one that completed the health check for Child A on 0510812023. To her knowledge, no marks were identified with the parent. She added that Child A has been sent home for allergy-type rashes quite frequently in the past. Ms. W. White showed and me the entire day via video, with the exception of approximately 1 hour (10:40 am — 11:37 am) as during this time he was outside on the playground and there are no cameras. At this time, she observed that CCSM1 was with Child A and three other children from the infant room in the fenced portion of the playground. She observed that Child A was crying so she took him from CCSM1 and brought him into the center at 11:37 am. She did not notice any marks on him at this time. None of the video during the time that Child A was present at the center showed any concerning behavior that could have caused the bruising. She became aware of the marks, which appeared to be a rash, after CCSMS changed his diaper at approximately 11:59 am. She had a CCSM take a picture and text it to Child A's Father. She also had a CCSM call Child A's Father and notify him of the rash and that Child A needed to be picked up from the center. During the interview with CCSM1, she confirmed that she completed the health check in the morning with Child A and he did not have any marks. She became of the marks when CCSMS showed them to her after a diaper change. She stated that they looked like . She recalled changing his •c3 diaper two times that day and did not lift his shirt to see if there were any marks. He was very fussy that day, as it was his first day transitioning into the toddler room. After approximately 1 hours, Ms. W. White had him go back into the infant room. 1/2 During the interview with CCSM2, she confirmed that she was Child A's primary caregiver in the infant room. She was not working with him on this day as he was transitioning to the toddler room from the infant room. She recalled that she took Child A for a buggy ride on 05/08/2023 with she thinks 3 other children. She observed Ms. W. White take Child A into the center from the playground as he was crying a lot. Prior to going outside, she observed Child A playing with his shirt, and occasionally lifting it. When he did so, she noticed from across the room. She did not report it or bring it to the attention of any personnel as she thought it was a that CCSM1 would have noticed during the morning health check. She added that Child A often had _ in the past as he is sensitive to many things. During the interview with CCSM3, she confirmed that she worked in the infant room on 05/08/2023. She became aware of the - when CCSMS showed her after a diaper
