    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On October 23, 2020, I interviewed Child A's Mother who indicated that on October 22, 2020, Child A placed a piece of straw up his nostril. The item was not visible without looking up his nostril. Child A's Mother was contacted by the facility and asked to come and pick up Child A. She stated that she believes an ambulance should have been called by the facility because the item in his nostril could have 2 gone from his nasal cavity down in to this throat and become lodged in his throat, obstructing his airway. She also stated that the facility failed to provide appropriate first aid to Child A during the situation. Child A was walked out of the facility by child care staff members. He should have had his head held still until an ambulance arrived to treat Child A. Child A's Mother took Child A out to the car when she arrived at the facility. Child A was having respiratory difficulties, and Child A's Mother called 911 to request an ambulance. An ambulance arrived and accessed Child A. The ambulance staff did not have tweezers so they were unable to remove the straw. The facility had tweezers which were supplied to Child A's Mother at which time she removed the straw piece from Child A's nose. Child A's Mother reported that Child A did not go to the hospital and did not require additional medical follow- up. Child A's Mother supplied a photograph of the straw piece located in Child A's nasal passage on October 23, 2020. The object was not visible without looking up Child A's nasal passage. On October 23, 2020, Child A's Mother also indicated that the facility previously allowed Child A to sleep with his nose and mouth against plexiglass. A photograph was supplied on October 23, 2020 but a date that his occurred was not indicated. Child A's Mother stated the facility needs to be more aware of protecting a child's airways. On October 23, 2020, I interviewed Kelli Webb, licensee designee. Ms. Webb was made aware of the situation at the facility but was not present at the time of the incident. Jennifer Kitchen, Coordinator, reported the incident to Ms. Webb on October 22, 2020. It is her understanding that Child A was participating in a group activity which involved lacing straw pieces through a pipe cleaner. Ms. Buffey and Ms. Hess were supervising children during the activity. The straws were accidently knocked over on the floor and everyone was cleaning up the pieces. Child care staff members did not observe Child A place the straw up his nasal passage but they indicated that Child A was not having difficulty breathing during the incident. Child A was monitored by Ms. Kitchen, Karrie Buffey, Child Care Staff, and Eula Hess, Child Care Staff Member. When the ambulance was called by Child A's Mother, Child A's respiratory rate was 98%. This was observed by Ms. Kitchen who was outside standing by the ambulance while Child A was being treated. If Child A were having difficulty breathing an ambulance would have been called by the facility. Ms. Webb does not have concerns with the care and supervision provided to Child A on October 22, 2020. On October 27, 2020, I completed an unannounced onsite inspection at the facility. I interviewed Ms. Kitchen who was present at the facility on October 22, 2020 but was not in the classroom at the time of the incident. The classroom had two child care staff members Ms. Buffey, Ms. Hess and eight preschool age children. Child A was participating in a free choice activity which involved the use of straws to make bracelets with pipe cleaners. The straws were cut into various lengths. During the activity, the straws were knocked off the table and everyone assisted with picking up the straws. Once Child A told Ms. Hess and Ms. Buffey that he had something up his 3 nostril, he was taken to Ms. Kitchen for assessment. Child A's Mother was then called to come and pick up Child A. A child care staff member was always present with Child A until his mother picked him up at the facility. Ms. Kitchen did not o
