    R 400.8380 · R 400.8380 Maintenance of premises. (2) The premises must be maintained so as to eliminate and prevent rodent and insect harborage.

    On October 26, 2022, I completed an unannounced onsite inspection at the child care facility. Due to the nature of the concerns in the allegations, upon arrival I contacted Tammy Compton, Education Coordinator, and requested that she come to the child care facility. Ms. Compton arrived within ten minutes of my telephone call. I explained the allegations to Ms. Compton. Ms. Compton acknowledged that the child care staff had seen evidence of mice in the classroom a few weeks ago. They are working with the building manager regarding their pest management plan. I contacted the building manager Heather Kale, who indicated that a pest management company is coming to the building twice a month and provided the documentation to me via email. Ms. Compton and I were in both the child care classrooms on October 26, 2022. I did not observe any mold, mice, or bats. Ms. Compton has not received any reports of mold or bats in the building. Ms. Compton is not aware of asbestos being a concern in the building at this time. The two early head start classrooms are currently only using the two approved classroom spaces and do not utilize any other space in the building. They are approved to use the gymnasium but reported they have not been in that space this current school year. I did not observe any mold, mice or bats on October 26, 2022, in the early head start classrooms. • The complaint that was received was anonymous and I was unable to obtain additional information regarding mice, bats, mold or asbestos in the building.

    On October 26, 2022, I completed an unannounced onsite inspection at the child care facility. Due to the nature of the concerns in the allegations, upon arrival I contacted Tammy Compton, Education Coordinator, and requested that she come to 3 the child care facility. Ms. Compton arrived within ten minutes of my telephone call. I explained the allegations to Ms. Compton. Ms. Compton and I walked to the front of the building to look for the hole. We walked around the back of the building and along the front of the building. Approximately halfway across the front of the building, Ms. Compton and I observed a hole along the roof line over the entrance doorway on the third floor. The hole was visible from the street and went into the building. I observed a support near the roof that appeared to be broken. I observed a dip in the front roof section, and I was able to see into the building. I contacted Heather Kale, building manager. Ms. Kale stated she was aware of the concern and a structural engineer is scheduled to come out to the building on November 2, 2022. At this time, I requested that Ms. Compton contact Kelli Webb, licensee designee and ask her to come to the facility. Ms. Webb arrived at the facility and observed the damage to the third floor on the front of the building. Ms. Webb and Ms. Compton decided to voluntarily relocate the two early head start classrooms to empty classrooms at other licensed sites in the county, until the building can be accessed. Photographs of the concerns were obtained during the onsite inspection. While at the onsite inspection, Ms. Compton, Ms. Webb and I walked the interior of the building. A large leak was observed in the gymnasium, which is separate from the hole in the front of the building There were approximately twelve containers on a large blue tarp collecting water leaking from the roof. The early head start classrooms have not utilized the gymnasium during this current school year. I observed damage on the third floor over the large windows. The material on the wall over the windows is cracking and not in sound condition. Early head start only utilizes the two classrooms on the main floor of the building. Photographs of the concerns were obtained during the onsite inspection. Ms. Compton and Ms. Webb were not aware of the building concerns and were not informed of the building issues by the building manager or any other building occupants. Both Ms. Compton and Ms. Webb stated children would not have been present in the building if they were aware of the building concerns. On October 26, 2022, all early head start children were sent home from the building during the onsite inspection. Ms. Webb wrote a plan indicating the children will not return to the classrooms until the building has been accessed and cleared by the engineer. Documentation will be provided to the department prior to their return. On November 3, 2022, I spoke with Jack Hippe, Building Inspector for the City of Flint. Mr. Hippe is aware of the building concerns and stated they are working with the building on corrections. The building does not have a certificate of occupancy. • The anonymous complaint alleged that children were using the gymnasium and "moved to the other side." Early head start has not utilized the gym during this current school year. Allegations were also made regarding there being no 4 plan for what happens when it snows. Last year everything was wet. I was unable to obtain additional information regarding these allegations because the complainant was anonymous. The building has several other programs, but they are not under child care licensing or head start.
