Licensee designees and program directors Ms. Vance and Ms. Doss did not provide a written information packet to each parent enrolling a child that included a notification plan for accidents and injuries. A review of the written parent information packet, including the parents handbook, found that the center failed to provide information to enrolling parents about how and when notification will occur if a child is injured while in care.

He fell from the bench onto the asphalt resulting in injuries to the back of his head. irst attended Forest Area Early Learning Center fora on 09/07/2021. n , Child A's Mother dropped Child A off at the center around 10:00 AM. Around 4:30 PM, she noticed a missed call from the center. When she called back, she spoke with child care staff member (CCSM) 1 and was informed that Child A had a fever of 101 degrees Fahrenheit (F). She was concerned someone from the center was calling to tell her that Child A his head on the cement outside because there is "cement everywhere" at the center. When she voiced this concern to CCSM1 she said, ''Dope, he just has a fever.'' By the time Child A was picked up around 4:45 PM that day, his temperature was 97 degrees F. His temperature was normal on the morning of 09/0912021, so Child A's Mother took him back to the center. CCSM 1 was present when she dropped Child A off. When Child A's Mother picked Child A up around 5:00 PM on 09/09/2021; CCSM 3 told her he had an accident from falling off the picnic table and had a "little red mark" on the back of his head. Child A's Mother looked at Child A's head and saw three red marks and bruising from the top of his head to the bottom, along with scraped skin. CCSM 3 acted as if Child A's injuries were "no big deal," saying that they put all of the babies at the picnic table for snack outside. CCSM 3 gave her an incident report and told her that CCSM 1 said Child A also fell from the table the day before. Child A's Mother asked to see the picnic table he fell from. CCSM 3 showed her an adult-sized picnic table just outside the toddler classroom door that was placed over asphalt. CCSM 3 reported that she was outside when Child A fell, and she had just walked away for a moment. Child A's Mother was extremely upset when she heard what happened; especially because she did not receive a phone call to inform her of his injury when she had just been called the day before about Child A's elevated temperature. She questioned CCSM 3 as to why she did not receive an incident report for Child A's fall from the table on 08/08/2021. CCSM 3 told her that because Child A did not have C] any marks, they did not write a report. She did not know what the center's policy was about notifying parents when a child has an accident or is injured. When she went back to look at her enrollment information and the parent handbook, she did not see any information about how parents are notified if a child is hurt while in care. Child A's Mother took Child A to the doctor on 09/1012021 for an examination. The doctor told her it did not appear that Child A had The incident report provided by Child A's Mother states the following regarding Child A's injury: "Date of accident: 9/9/21.. Time of accident: 2:40PM. Nature of injury: Fall. Location of incident: Outside. What was the child doing: Sitting at the picnic table. Caregiver response and first aid: TLC (CCSM 2). Parent contacted? Yes. Name of parent contacted: [Child A's Mother]. How parent was contacted: Other Pick up. Other contacts or actions: Small bump on head. Date: 9/9/21." On 09/1312021, 1 reviewed the child care licensing incident report provided by child care licensing consultant Ms. Amanda Wendell via email that same day. The original email with attached incident report was sent to Ms. Wendell by program director and licensee designee Ms. Pamela Vance on 09/1012021 at 6:03PM. In the email, Ms. Vance stated she was informed by the center's head start liaison that Child A was seen by a doctor for his injury. The incident report that was signed and dated by Ms. Vance on 09/10/2021, states that at approximately 2:40PM on 09/09/2021, three caregivers and seven children were outside for snack/play time.
