Child A (male, 3 years) and Child B (male, 3 years) bite other children and the center is not addressing the concern. Child C (female, 4 years) exposed herself to other children and the center did not address. On two separate occasions, children had accidents and fecal matter got onto the carpet in the rooms. On another occasion, a child vomited onto the carpet in a room. The center did not properly clean after these incidents. Nap/rest time often does not occur in the prekindergarten room. Lunch is made in the morning and sits out unrefrigerated. Garbage is not taken out daily. The center is very dirty and is not cleaned regularly.

On 08/04/2023, I interviewed the complaint source who stated that Child A and Child B constantly biting other children in their room. Ms. Williams doesn't do anything about the biting, as she is friends with their parents. On 08/07/2023, I completed an unannounced onsite inspection at the center from 10:50 am - 12:10 pm and interviewed program director/licensee designee Krystalle Williams. Ms. Williams stated confirmed that she is friends with Child A and Child B's parents, but this does not prevent the center from addressing necessary concerns, if she is made aware of such. No parents have brought up any concerns regarding Child A or Child B. She is aware that they are "best buds" and often injure one another. She has spoken to their parents, just as a precaution. On 08/08/2023, I completed an onsite inspection at the center to interview assistant program director Beth Holloway and CCSMs 1-8. Ms. Holloway stated that she was aware that Ms. Williams is friends with Child A and Child B's parents but confirmed that this does not prevent the center from addressing necessary concerns, if she is made aware of such. No parents have brought up any concerns regarding Child A or Child B. She is aware that they often injure one another and has spoken to their parents, just as a precaution. CCSM 1 stated Child A and Child B rarely bite, and it is not an ongoing concern. There have been a few instances between the two boys, but their parents have been spoken to and it is usually regarding scratching each other. No parents have brought up any concerns regarding Child A or Child B. CCSM 2 was aware that there some behavioral concerns with Child A and Child B, but nothing serious and not ongoing. CCSM 3 has cared for Child A and Child B and no parents have come to her, other than theirs, about any concerns. Their parents have been spoken to about how rough they can get with one another. CCSM 4 stated that Child A and Child B do not bite, they just get "rowdy" with one another. Occasionally another child is involved, and an accident report is developed. No parents have come to her, other than theirs, about any concerns. CCSM 5 confirmed that Child A does occasionally scratch another child, but not Child B. She has not received any parent concerns regarding these activities. 3 CCSM 6 does not work with Child A and Child B but is aware that they fight with one another. CCSM 7 was not aware of this concern. CCSM 8 works with Child A and Child B and added that their behavior "feed on one another." They are both fine when they are not together. She has never had a parent present a concern to her about their behavior. She does feel that Ms. Williams provides favoritism toward Child A as she is friends with his parents. On 09/01/2023, I left a voicemail message for CCSM 9. On 09/01/2023, I left a voicemail message for CCSM 10. Later she returned my call and stated that she is aware that Child A and Child B can get somewhat aggressive, as her son is in their room. She added that recently her son was mildly injured by one of them, but she does not feel it a concern. She feels that the center is addressing it appropriately.

The complaint source stated that Child C was sitting on the carpet in her room and lifted up her dress and moved her panties to show her private area to other nearby children. She was unsure when this occurred and the other children that were involved. She did not observe the incident, just was nearby. Child C's parents were notified, but none of the children who were exposed were notified. Ms. Williams and Ms. Holloway were both aware of the incident yet neither feel that it occurred as reported. Child D (female, 3 years) told her mother, Ms. Holloway, that Child C was playing with her dress, and it was lifted somewhat and only part of her panties were exposed, none of her private area. Previous CCSM 1 told them that she didn't see the incident but heard one of the children state they saw Child D's panties, so she went and specifically asked Child D if she showed her private area to 4 her friends and she replied "yes." That is why none of the other parents were notified, as an exposure did not occur. The incident was addressed with Child C's parents. CCSM 1, CCSM 2, and CCSM 3 all became aware of the incident with Child C as previous CCSM 1 told them that it occurred. They were not involved further. CCSM 4 and CCSM 5 were not involved with this incident but overheard that it occurred. CCSM 6 and CCSM 7 were not aware of this incident. CCSM 8 was not involved with this incident but was notified of it by previous CCSM 1. She is aware that Child C usually has shorts on over her panties under her dress so does not see how she could have exposed her private area. CCSM 10 heard about the incident from another CCSM and was not involved.

The complaint source reported that an infant in the Infant 2 room had a "blowout" and some of the fecal matter got on the carpet. Another time a child accidently got some fecal matter on the carpet in the prekindergarten room. Later that same day, a child vomited on the carpet in the prekindergarten room. She was not exact on the dates that these incidents occurred. She is aware that the carpet was not properly cleaned and sanitized afterwards as only glass/multi-surface cleaner and paper towels were used. Ms. Williams was not aware of the incident in the Infant 2 room or the fecal matter incident in the prekindergarten room. She was only aware of the vomit on the carpet in the prekindergarten room. She thought that the CCSM used soap and water, then water, and then the disinfectant. She provided me with a printout of the poster that is 5 in the rooms that shows the proper process to follow for cleanup of bodily fluids. She confirmed that the carpets were cleaned following this incident, as well. Ms. Holloway was not aware of the incident in the Infant 2 room. If it did occur, she stated that CCSMs should know the proper process to follow for cleanup of bodily fluids. She was told about the fecal matter in the prekindergarten room and that the disinfectant was not used. She was not aware of the vomit incident in the prekindergarten room. She feels that there is some confusion in the process and the CCSMs need to be further trained on the proper process. She recalls that the carpets were cleaned afterwards. CCSM 1 was aware of the incident in the Infant 2 room, as she was the one acting as the program director at the time. She did not supervise how the fecal matter was cleaned and assumed that the CCSM followed the posting in the room. She did have all of the children evacuate from the room for the rest of the day. She was not aware of the incidents in the prekindergarten room. CCSM 2 was told about the incident in Infant 2 but was not involved. She was not aware of either incident in the prekindergarten room. CCSM 3 was not aware of the incident in the Infant 2 room. She was told about the incidents in the prekindergarten room but was not involved. CCSM 4 was not aware of the incident in the Infant 2 room. She was not aware of the fecal matter in the prekindergarten room but was aware of the vomit, as one of the CCSMs told her. She was not involved. CCSM 5 was not aware of the incident in the Infant 2 room. She was the one that cleaned up the fecal matter and the vomit in the prekindergarten room. She did not do the disinfecting step. Soon after the incidents, the carpets were professionally cleaned. CCSM 6 and CCSM 10 were not aware of these incidents. CCSM 7 as the one that cleaned up the fecal matter in the Infant 2 room. She never did the 3-step process. She just used the sanitizing solution and then the disinfectant. She was not aware of the incidents in the prekindergarten room. CCSM 8 was not aware of the incident in the Infant 2 room. She was not involved with the incidents in the prekindergarten room but heard about them from previous CCSM 1. A

The complaint source stated that often nap/rest time does not occur in the prekindergarten room, as the get too busy and the time has passed. Ms. Williams, Ms. Holloway, and CCSM 1 confirmed that nap/rest time is always offered, but not all of the children actually fall asleep. They offer quiet activated after a period of quiet time. She is aware that previous CCSM 1 misunderstood this rule and felt that all children should sleep and remain on their cots the entire nap/rest time. CCSM 2, CCSM 6, and CCSM 7 never worked in the prekindergarten room and were not aware of the concern. CCSM 3, CCSM 4, CCSM 8, and CCSM 10 are aware that nap/rest time is offered daily and that not all of the children will sleep. They confirmed that they provide quiet activities for these children. CCSM 5 stated that previous CCSM 1 thought that all children had to sleep and remain on their cots for the entire nap/rest time. She always provides quiet activities for the children.

The complaint source has observed lunch prepared in the morning and sit out unrefrigerated until lunch time. This includes food that is perishable. Ms. Williams and Ms. Holloway both explained that often the cook may prepare lunch items in the morning, but the only items that are left unrefrigerated are non- perishables. On 08/07/2023 and 08/08/2023, I observed the leftover food from breakfast was thrown away and the lunch items that were prepared earlier, were non-perishable observed the cook prepare the meat for the day and serve it while it was hot. CCSM 1 and CCSM 3 both stated that they have prepared the meals and denies that lunch is made in the morning and sits out until lunch time. CCSM 2, CCSM 6, and CCSM 10 stated that they never prepare the food and denies that lunch is made in the morning and sits out until lunch time. CCSM 4 is the primary cook for the center. She clarified the detailed steps she completes daily for all meals. She confirmed that non-perishable food, such as fish sticks that were served recently, are cooked in the morning, and then remain unrefrigerated for hours until lunch time. CCSM 5, CCSM 7, and CCSM 8 stated that the food that is made ahead of time does not stay unrefrigerated until lunch time. If anything, their concern is sometimes they get their lunch, and it is too hot, and they have to give it time to cool down to the proper temperature.

The complaint source has observed that garbage has not been taken out at closing and when CCSM's open, they have to take out the garbage from the day prior. Ms. Williams and Ms. Holloway both stated that the garbage goes out daily to the dumpster. This activity is part of the closing CCSM duties. CCSM 1 denies that the garbage is not taken out daily. She lives very close to the center and if a CCSM closes and leaves the center realizing that they forgot to take it out, they will send a message through the group text message, and she will drive up to the center and take it out. CCSM 2 stated that garbage is take out daily and has never heard a concern otherwise. CCSM 3 reported that there are times that the garbage does not get taken out. She feels that this occurs less frequently. She is ware of such as she is an opener and has observed garbage in the center from the closers. CCSM 4 stated that sometimes the closers forget to take out the garbage. Sometimes they will remember and send a group text message and CCSM 1 will come and take it out. Other times they just forget, and the openers take the garbage out. CCSM 5 arrives at the center at approximately 7:15 am and opens her room. She often finds that garbage from the night prior and has to take it out. She does notify all via a group text message when this occurs. CCSM 6 and CCSM 7 has seen the group text messages from the openers in which they are notifying the closers when the garbage is not taken out the night prior. CCSM 6 often is the opener for her room and the garbage from the night prior is present and she has to take it out. CCSM 8 is a closer and has forgotten to take the garbage out. She has put it in a group text message and all of the CCSMs seem understanding. CCSM 10 stated that the garbage is often not taken out. She has forgotten herself and notified all via a group text message if she recalls.

On May 29, 2023, I interviewed Kelly Millar, who is the licensing consultant for the center. She recently completed an inspection at the facility on April 21, 2023. She did not recall the center being unkempt, dirty, or having mold anywhere in the facility. The facility had some violations, but they were paperwork related and not regarding the bathrooms or cleanliness of the facility. On May 29, 2023, I interviewed licensing consultant Juli Gohl, who had been to the facility on May 03, 2023, for a special investigation complaint (2023D0125004) for another unrelated issue to these allegations. She denied observing the center dirty or unkempt. She did not observe the toddler 2 bathrooms. On June 02, 2023, I interviewed the complainant. They stated that the center had mold under a shelf in the toddler 2 room (sometime in the early Spring 2023). A few of the staff also knew about it. The toddler 2/preschool bathroom had feces on the wall due to staff not cleaning. Some of the staff document the cleaning logs that they cleaned, but they didn't actually do it. Many of the staff are overwhelmed and not trained adequately. The same staff are always cleaning and not pulling their weight. The director doesn't check or make sure that its done or done correctly. In addition, there has been a lot of staff illnesses this last several months and the kids have been sick too. Some of the staff have been sick for weeks straight. This could be a result of the staff not washing/rinsing and sanitizing the equipment and rooms as needed. In particular, hand, foot and mouth went around a couple times and they couldn't get rid of it. Either because the director let sick kids come back or they weren't sanitizing things enough to kill the germs. On June 02, 2023, I completed an unannounced onsite inspection at the facility from 12:33pm to 2:30pm. I observed the entire facility. I did not observe the center to be dirty. I did not observe any feces on the bathroom walls. I did not observe any mold in the toddler 2 room. The bathroom in the toddler 2/preschool rooms had paper towels on the floor from the children dropping them. I observed a broken wrapped in plastic so the children wouldn't use it, but a child had peed on the top layer of plastic. There was cleaning procedures posted in the bathrooms with a cleaning schedule. It appeared that some of the staff had been signing the cleaning log, and some of the signature 3 spots were empty. The preschool bathroom cleaning log was dated February 06- February 10, 2023. The school age room actually smelled of bleach from recently being cleaned. I observed wash/rinse/sanitize solutions in all of the classrooms. I interviewed assistant program director Beth Holloway. The current director Krystalle Williams is out on leave right now. She denied that the center is dirty. She had no knowledge of feces ever being on a wall. Her own child is in the toddler 2 room and she is in the room a lot. She hasn't observed any issues of cleanliness in there. No parents have complained or even staff workers about the bathrooms or the center being unclean. She had no idea where this concern was coming from. She was aware that a while ago one of the staff had found mold under a piece of equipment in the Toddler 2 room after they had actually had the carpets cleaned and put the shelf back over the carpet. When the shelf was moved, they found a small amount of mold had started to develop since it was wet when they covered it. They had it cleaned and removed right away. None of the children were around it or harmed. On June 02, 2023, I interviewed CCSM 1. She denied that the center is dirty. She has never heard of mold in the facility. They did have hand, foot and mouth go around this winter/spring and she was sanitizing the equipment at nap time and at the end of the day. On June 02, 2023, the complainant stated that the children in the preschool room are dirty and not clean up when needed. The staff in that room are lazy and they do not wipe their noses when needed or wipe their faces after they eat. The complainant has seen this many times but doesn't have specific dates or which children were not clean. On June 02, 2023, and June 20, 2023, I observed the children in the preschool room. Both times they were finishing lunch and getting ready for naptime. I did not observe the children to be unkempt or excessively dirty. I observed children using the bathroom and washing their hands. I observed the stand assisting them with laying down and getting ready to sleep. On June 02, 2023, Ms. Holloway and CCSM1-7 denied that the preschool room children are not cleaned up as needed or are dirty. Ms. Holloway stated that none of the parents have complained that their children are dirty or not being cared for. She has been in the preschool room many times and seen the staff caring for the children. She had no idea why someone would allege that the kids aren't being cleaned or tended to. On June 15, 2023, I interviewed Child A's Mother, Child B's Father, and Child C's Mother. None of them had concerns about their child's hygiene or cleanliness at the center. They all felt that the children are adequately cared for and supervised. Child C's Mother stated that Child C has come home dirty many times, but usually because she plays outside in the afternoon and they pick her up right after they come in. She has never been unreasonably dirty though to where it is a concern. On June 15, 2023, CCSM8 denied that the preschool room children are dirty or not being tended to or cared for adequately. On June 20, 2023, CCSM9 denied that the children in the preschool room are not cleaned up when needed. They have seen the staff tending to them.

On May 29, 2023, I interviewed licensing consultant Juli Gohl, who had investigated an allegation regarding diapering procedures (2023D0125004). She completed an unannounced onsite inspection to the facility on December 16, 2022. She did not have any violations about this rule. On June 02, 2023, the complainant stated that some of the staff members do not wash their hands in between diapering's. They did not have specific dates/times or staff who do not do this, but they have observed this many times over the last few months. The staff are all trained in proper handwashing procedures, but some are lazy and they do not follow the requirements. Even if the staff wear gloves for diapering, they do not wash their hands between diapering or toileting a child. On June 02, 2023, I interviewed Ms. Holloway and CCSM1-7. They all denied not following proper handwashing procedures. They denied seeing a coworker not follow proper protocol. During the inspection, I observed the staff in the toddler room complete two diapering's. They followed proper handwashing protocol. I observed a staff assist a child with toileting in the Preschool room and they followed proper handwashing protocol. In the school age room, I observed a child come out of the bathroom and wash their hands. On June 15, 2023, I interviewed Child A's Mother. She has not observed a specific diapering procedure conducted by a staff member. She has overheard them telling children to go wash their hands after they use the toilet. One June 15, 2023, I interviewed Child B's Mother. She has not observed a staff complete a diapering. She is not there in the center when this would be done. She has no concerns that her child isn't being cleaned or isn't washing his hands when required. On June 15, 2023, I interviewed Child C's Mother. She has been to the center many times in the classroom. She has heard staff tell the children to clean up after eating or to wash their hands. On June 15, 2023, CCSM8 denied that the staff are not following handwashing protocols. On June 20, 2023, CCSM9 denied not following proper handwashing protocol or seeing a staff do this. 7

On May 26, 2023, the complainant stated the lead caregiver in the preschool room, is not in her classroom as required (at least 6 hours a day for a full-time program). She oftentimes leaves the classrooms and is gone longer than she says she will be. There is favoritism with the managers, so she is often chatting with them in the office. The complainant did not have specific dates or times when she is absent. On June 02, 2023, I interviewed CCSM1-7, and Ms. Holloway. None of the staff have observed the preschool lead caregiver out of the classroom for long periods of time. Ms. Holloway denied that the preschool lead is not in her room the required 6 hours of the day. Unfortunately, none of the classroom leads have time to leave the classroom for planning, which she would like to have them do. They currently have their great room/lobby set up with a desk. This is a space for staff to do their trainings at, or for leads to do their classroom planning at. Ms. Holloway indicated that it has been so busy lately that they barely have the one hour allotted time each week to do their planning. On June 15, 2023, CCSM8 denied that the preschool lead is not in her classroom 6 hours of the day, as required. On June 20, 2023, CCSM9 denied that the preschool room lead is out of the classroom.

On 04/27/2023, I interviewed the complaint source who stated that she walked by the Infant room 1 on the way to her pick up her child in the Infant 2 room and observed the unknown female child care staff member "scrolling" on her cellphone, appearing to be on social media. She was not watching the infants in the room. When she walked by approximately five minutes later, she observed the child care staff member was still on her cell phone in the same location of the room. She is unaware if the center has video that may be reviewed. On 05/03/2023, I completed an unannounced onsite inspection at the center and interviewed CCSM 1 and CCSM 2. CCSM 1 stated that she was acting as the program director in CCSM 3's absence, as she had left for the day. Program director/licensee designee Krystalle Williams was currently on leave and CCSM 3 is acting as the temporary program director in her absence. CCSM 1 confirmed that CCSM 3 was caring for four infants in the Infant 1 room at approximately 4:40 pm on 04/26/2023. The video that the center has is a live feed and does not tape. The 2 center notifies CCSM's that they are not use their cell phones unless they are contacting a parent, handling an emergency, or trying the reach the program director. I interviewed CCSM 2 who confirmed that she was working in the Infant 1 room at approximately 4:40 pm and caring for four infants. She admitted that she "was probably on my phone, as I often am." She stated that she more than likely was on social media. She confirmed that she was not using her cell phone to contact a parent, handle an emergency, or to reach the program director. She clarified that she understands that she should have not been on her cell phone on social media while she was caring for infants.

On March 15th, 2023, I completed an unannounced visit to Kinder Care Silver Parkway. I conducted an interview with program director Krystalle Williams. She was not present at the child care center on the day Child A was left unsupervised. The assistant program director Beth Holloway was not present either, the center was left in the care of the third in charge, CCSM 3. CCSM 3 was in a different room and was called by CCSM 4 who told her she had been notified by a child's pick-up person that Child A was alone in the lobby of the center. CCSM 3 is normally Child A's teacher, she wen to the lobby and got him and took him back to the classroom he was supposed to be in. CCSM 1 and CCSM 2 did not know he had been gone. When speaking with staff it appeared this happened near the end of the day, they were transitioning classrooms together due to number of children. CCSM 2 was attending to a different child on the opposite side of the classroom. CCSM 1 was signing in the children to the attendance who were transitioning. Within those few minutes Child A exited the room. They believe he exited through the shared bathroom space and out the door of the empty classroom, which had been propped open. They have had a staff meeting with a training already regarding the incident, appropriate name to face check in times of transition. They specifically spoke about Child A as he is non-verbal and would not necessarily answer them when his name was called, and they would need to look at him during the name to face checks. The room that Child A is normally in already has an alarm on it as he has a habit of leaving the room or trying to. They have already secured an alarm for the preschool classroom door as well to ensure that both doors he would possibly exit have alarms. CCSM 4 reported to her a child who was being picked up by their pick-up person noticed the child in the lobby and was concerned he was alone in the lobby. I conducted an interview with CCSM 3, who when the program director and assistant program director are not present is in charge of the center. She has been employed at the center since October 2021, she is the lead in Toddler 12. Child A is in her classroom routinely, he has autism or is being tested for autism. They have an alarm on the door of Toddler 2 due to his having left the room previously. On Wednesday March 8th, she was covering the office, and had transitioned to the office around 3:00 PM. She was in the Infant 2 classroom talking with a parent when CCSM 4 called her and said a pick-up person just told her that Child A was in the lobby unattended. 4 She went to the lobby and retrieved Child A and took him back to the preschool classroom. CCSM 1 and CCSM 2 did not know he had been gone. They were apologetic and reported they had just been doing their name to face checks. She reminded them they needed to watch Child A really well. She believes he left preschool room through the bathroom back to his regular room, and then exited the classroom through the open door, which normally has an alarm in place. She helped out in the room for a minute, and helped in Infant 2 room, and checked back in on them in preschool room. She did not notify Child A's mother about the incident. She notified Krystalle Williams the program director via telephone that day. She does not know if the teachers in the classroom notified Child A's mother at pick-up as she was not at the center at that time. CCSM 3 told Ms. Williams that another child's pick-up person found Child A unattended in the lobby. She has not ever dealt with a child unattended previously while left in charge. She has previously filled out incident reports but did not look at this as an incident which would require a form to be filed out. She has received the centers handbook for staff, she does not believe it gives specific detail on what to do with this situation. On March 15, 2023, I interviewed Krystalle Williams, program 8th director. She was absent from the center on March but returned March 9th. She did not report the unsupervised child to the department upon return to the center as she wanted to speak with the parties involved. She also wanted to include her assistant program director in those conversations, however she was not at work on the 9th, so she waited until the next day but then staff were not available. She did not get a chance to speak with those involved until the following Monday, which is when she called her district leader who advised her to report it to the department.

On February 22, 2023, 1 interviewed the complainant. She admitted that she had heard this being discussed at a party and that the person she heard it from someone else. She did not have any first-hand knowledge of the alleged incident and has never spoken to Child A or Child A's mother. On February 22, 2023, I spoke with . The complaint had been forwarded to his department Rue o the"na re of the allegations. He interviewed Child A's mother, and she denied that Child A had ever made these allegations. She said that between Christmas and New Year's, Child A did say that a male child care staff member by the name of took his blankets and his pants. _ On February 23, 2023, I interviewed Child A's mother. She denied that Child A had ever made the allegations and did not understand where this was coming from. The oft possible "incident" Child A has had at the center is that when Child A was he said a male staff member made him go outside and sit on a blanket and took ants. Child A's mother confirmed that he still attends the center and that he is not yet On February 24, 2023, I made an unannounced onsite inspection to the child care center. I interviewed program director Krystalle Williams and assistant director Beth Holloway. Neither had heard of any allegations made regarding Child A. They confirmed that Child A still attends the center. Child A's parents have never discussed any concerns regarding a child care staff member with Child A. There has only been one male child care staff member employed at the center, Child Care Staff Member 1 (CCSMI) but he no longer*ise em loyed at the center. There has never been any child care staff member named at the center. CCSM1 was let go in mid-December 2022 due to staff complain h Ms. Williams and Ms. Holloway said that no parent or child had complained about CCSM. They confirmed that the only time a child leaves the building, it is to go to the attached playground. I attempted to contact CCSM1 but as the date of this report. I have not heard back. 3

The complaint source stated that none of the CCSM's that work in the Infant 1 and Infant 2 rooms wash, rinse, and sanitize the diaper pad between changes. She notified both Ms. Williams and Ms. Holloway of this and it was not addressed prior to her last day of employment. During the onsite inspection at the center, I observed washing, rinsing, and sanitizing of the diaper pad after each use. Ms. Holloway was not aware that CCSM's were not washing, rinsing, and sanitizing the diaper pad between changes. She has observed all CCSM's follow this process when she is in the room. Ms. Voight, Ms. Bullen, Ms. Dunton, Ms. Kontnik, Ms. Holder, Ms. West, and Ms. Boldin stated that all CCSM's follow the washing, rinsing, and sanitizing process of the diaper pad between changes. All CCSM's are trained on it and Ms. Williams and Ms. Holloway observe that they are doing the process properly. Ms. Ireson stated that CCSM's in the past may not have followed this process, but they do now. They were reminded at multiple team meetings of the process and Ms. Williams and Ms. Holloway periodically observe them follow the process in the rooms. Ms. Reno admitted in the past that she was not always following this process. However, they have been trained at team meetings and she always follows this process now. She doesn't pay attention if others follow the procedure. Ms. Carter reported that it depended on which CCSM was working in the room at the time, as some CCSM's followed the process while others did not. Ms. Marshall and Ms. Nichols feel that when they worked in the room this process was always followed by all CCSM's. Ms. Williams was not aware that this was a concern and each time she is in the room she observes all of the CCSM's following the proper diapering procedures.

The complaint source stated that she was aware that Ms. Nichols was entering into the tablet that she completed diaper changes when she did not. Ms. Williams and Ms. Holloway were aware of this, and she did not feel that it was handled appropriately. All they did was remind everyone to change diapers. On a day she did not recall, Ms. Nichols cared for four children whose diapers were so full and heavy they were falling off. This was immediately brought to Ms. Williams's attention who stated that she would speak with Ms. Nichols, but never did. The complaint source reminded Ms. Williams to speak with Ms. Nichols multiple times that day and when Ms. Williams did so, nothing changed. All Ms. Williams did was remind Ms. Nichols to change diapers. Ms. Holloway was aware that Ms. Nichols was entering that she changed diapers when she did not. Ms. Nichols was coached on the subject, and it was corrected. Ms. Voight observed Ms. Nichols do this. She saw her enter that she changed a child's diaper but when she checked the child's diaper it was still wet. She notified Ms. Williams and Ms. Holloway. Ms. Bullen, Ms. Dunton, Ms. Kontnik, and Ms. Holder were not aware of this concern and are not aware of any CCSM doing such. Ms. Ireson and Ms. Boldin heard that Ms. Nichols did this often. They didn't notify Ms. Williams or Ms. Holloway as they never observed the concern. Ms. Reno stated that Ms. Nichols did this fairly often. She would enter into the tablet that she changed a diaper for a child, but the diapers were not changed and rj remained heavy and full. She recalls her changing a diaper at 10:00 am and not changing the diaper again until 2:00 pm. During this time frame she would enter into the table that she checked that the diapers were dry, but they weren't. She notified Ms. Williams and Ms. Holloway. Ms. West was not aware of this concern and never worked with Ms. Nichols. Ms. Marshall heard of the incident from another CCSM and told Ms. Williams and Ms. Holloway. Ms. Carter was not aware of this concern. Ms. Nichols admitted to entering that diapers were changed when she did not change them. She said she last worked at the center in July of 2022. She recalls that she was coached by Ms. Holloway on proper diapering procedures but could not recall when this occurred. Ms. Williams was aware that Ms. Nichols was doing this and added that she was coached on appropriate diaper checks and changing times. After this, she would do spot checks and each time a diaper was entered in the tablet. She then confirmed by checking the diaper to see if it was actually changed. Each time she confirmed accuracy in Ms. Nichols' entries into the tablet. The written discipline for Ms. Nichols could not be located. ...............

The complaint source stated that Ms. West changed Child A's diaper that had leaked onto his clothing. She did not change his clothes. Child A's Mother had to change his clothes when she arrived to pick him up. She believes that Ms. Williams and Ms. Holloway were made aware of this. Ms. Holloway was made aware of this situation by Ms. Reno. When she addressed it with Ms. West, who stated that she did not see the fecal matter on the clothes. The child's onesie was tie dyed and the fecal matter blended in with the pattern. Ms. Voight heard about the situation from Ms. Reno but did not observe. She feels that nothing like this has ever reoccurred, and they all do a good job at changing a child's clothes if they are soiled. Ms. Bullen, Ms. Dunton, and Ms. Holder were not aware of the concern, and they all do a good job at changing a child's clothes if they are soiled. Ms. Kontnik knew that Ms. West was the last CCSM to change Child A and fecal matter remained on his clothes. She believes that Ms. Reno brought it up to Ms. Williams and Ms. Holloway. Ms. Ireson was in the room with Ms. West when this occurred. Ms. West changed Child A's diaper and didn't realize that it leaked onto his outfit, as it was tie dyed and difficult to see. She even did a once over and did not see it. She feels it was an overlook and was not intentional. Ms. Reno is Child A's Mother and came into the room while Ms. Ireson had him on the changing table. He was fussy, so she was trying to soothe him. As Ms. Ireson pulled his pants down, she noticed dried fecal matter and said that it must have gotten on his clothes during the most recent diaper change that Ms. West completed. She changed his clothes then told Ms. Williams and Ms. Holloway that it occurred. Ms. West explained that this did occur, but Child A's onesie was tie dyed and she did not see the fecal matter. She did not leave him in his soiled clothes intentionally. Ms. Boldin heard of the incident from Ms. Reno but did not observe it. Ms. Marshall and Ms. Carter were not aware of the incident. Ms. Nichols never worked with Ms. West. Ms. Williams was not notified of the incident. 7

The complaint source observed Ms. Bullen eat too much of the children's food during meals, resulting in not enough food for the children who would complain of being hungry. Ms. Holloway was not aware of this concern. The CCSM's sit at the table with the children while they eat, and food is provided family style. She has observed meal time in all of the rooms, and it appears there is often leftovers. Ms. Voight explained that Ms. Bullen and her work together in the same room and she has never observed this. They eat family style, and the children always get enough food as they usually have leftovers. They can always get extra food from the kitchen as needed. Ms. Bullen, Ms. Dunton, Ms. Kontnik, Ms. Holder, Ms. Ireson, Ms. West, Ms. Boldin, and Ms. Nichols were not aware of this concern and never observed it occur. They clarified that CCSM's never eat too much of the children's food. Ms. Reno does not work with Ms. Bullen but had heard from another CCSM that she would make a large plate of food for herself and then serve very small portions of food to the children. Ms. Marshall primarily works in the kitchen and confirmed that there are usually leftovers. She is the one that serves the food, and it is portioned out for the children and the CCSM's. Ms. Carter only observed Ms. Bullen eat the leftover food, after the children were finished eating. Ms. Williams was not made aware of this concern and did not observe it. She feels that it is untrue as they have plenty of food for the children and CCSM's.

The complaint source observed Ms. Holder serve whole milk to a child who was only 11 months old. She could not recall the child's name. She thinks that the child had a sip of the milk prior to another CCSM telling Ms. Holder of the mistake and taking the milk away. She thinks that Ms. Williams and Ms. Holloway were notified but is unaware of how they handled it. Ms. Holloway, Ms. Voight, Ms. Bullen, Ms. Dunton, Ms. Kontnik, and Ms. Ireson were not aware of this concern. They feel that all CCSM's are aware of what beverages and milk shall be served to the appropriate ages. There is a chart in the room with the children's ages so they are certain what they can be served. Ms. Holder denies that this occurred. She recalled that she served milk to a child under the age of 1 year, but the child did not drink any of the milk. She could not recall the child's name. A CCSM reminded her of his age, and she was able to remove the milk prior to consumption. They now have a poster in the room with children's ages to prevent a mistake. Ms. Reno said that Ms. Holder served the child milk a few different times, but he did not drink it as another CCSM intervened and the milk was taken away. She could not recall the child's name. Ms. West and Ms. Boldin stated that another CCSM told them that this occurred, but they did not observe it. Ms. Marshall was made aware of the incident, but the child did not actually drink the milk. Ms. Carter was not aware of this concern. Ms. Nichols did not recall this incident occurring or observing such 12 Ms. Williams was aware that Ms. Holder served milk to a child under the age of one year, but it was noticed by another CCSM in the room prior to consumption. She believes Ms. Reno was the one that reported it to her. Since then, they have posted a chart in the rooms with the children's ages to prevent a child from being served and drinking milk when they are not old enough.
