On 2/3/2026, Child A was burned by a hot glue gun that was plugged in by staff and placed on a high shelf; however, another child was able to pull the cord to the hot glue gun resulting in it being in access to children.

On 04/25/2023, I completed an unannounced onsite inspection at KinderCare Owen Road. I met with and conducted an interview with the program director Jaclyn Ettinger. She denies the center is propping bottles when feeding the infants. They have sufficient cribs in the center, they are not all in one room, but they have sufficient cribs for infants to be utilize. The center does allow infants to have blankets or shirts from home if they are awake, it helps them to have smells from home. On 04/25/2023, I interviewed child care staff member 1. Child care staff member 1 has worked at the center for approximately a year and has worked at other child care centers. She is in the Infant one classroom, and she is an assistant. Child care staff member 1 has completed trainings, videos, shadowing, safe sleep diaper changes, CPR/First Aid, and monthly trainings at the center. She is responsible for feeding infants. She did work last week in Infant one classroom. The lead for Infant one is child care staff member 2. CCSM 1 denies propping bottles. If she is feeding just one infant, then she holds the infant. In Infant one classroom, there are 12 3 infants some days, and other days they have fewer than 12. Cribs are moved around as needed, not all infants sleep at the same time. Some of the infants have shirts and blankets from home, and they do let the infants hold them when feeding or on the ground it soothes them. Child care staff member 1 has no concerns or issues with the center, staff, or management. On 04/25/2023, I interviewed child care staff member 2. Child care staff member 2 is the lead in the Infant one classroom. She has been at the center since November 2021. She has worked in other child care centers and was a nanny for 10 years. Child care staff member 2 became the lead in July or August of 2022. She did work last week. She denies propping bottles of the infants. Child care staff member 2 has not seen any of the assistants or anyone in the room propping bottles. Child care staff member 2 has no concerns with the staff she supervises or with management. On 04/25/2023, I interviewed child care staff member 3. Child care staff member 3 has worked at KinderCare for almost a month. She has worked in child cares before. She has worked in all three infant rooms. Child care staff member 3 was in infant one classroom last week. Child care staff member 2 stated she does not prop bottles when feeding infants. She holds the infants and feeds them. Child care staff member 3 has no concerns about any staff, the center, and she loves it there. On 04/25/2023, I interviewed child care staff member 4. Child care staff member 4 has worked at the child care center since March of 2023. She has never worked at a child care prior to this. Child care staff member 4 reported she normally bounces between the infant and toddler room. She believes she might have been in the Infant one classroom last week, but she is not sure. She feeds infants while sitting in a rocking chair. Child care staff member 4 denies seeing anyone prop bottles or that it is occurring. She has no concerns about the center, staff, or management. She feels like everyone is great and she likes working at the child care center. On 05/03/2023, I interviewed Child A's mother. Child A has been going to the center for three to four months. Child A's mother is happy with the center and has no issues. On 05/03/2023, I interviewed Child D's mother. Child D's mother is thrilled with the center and is very surprised to hear a complaint was filed. Child D's mother is not able to corroborate any part of the complaint. She reported the child care center as lovely, and feels her child is very safe there. Child A's mother believes the center is always making sure her child is safe, including telling them the binky they sent did not meet a safety standard so they could not use it, and they appreciated that. On 08/24/2022, I completed an unannounced onsite inspection at KinderCare Owen Road. I met with and conducted an interview with Program Director Jaclyn Ettinger and Assistant Director Tara Pitcock. This incident was regarding Child A, neither of them were present in the center at the time the center became aware there was an issue. Child care staff member 5, Sarah Bennett was in charge of the center and closing for the day. Child A's mother had come to pick her up and was seen walking down the hallway holding Child A, a garbage bag and was crying. Ms. Bennett stopped and asked Child A's mother what was wrong. She advised she sent Child A to daycare in Huggies, and they use pampers at day care. When she picked up Child A, she was extremely wet, pants were wet, diaper was wet, and Child A was still wearing a Huggies, so she had not been changed all day. Ms. Bennett apologized to Child A's mother and told her she would find out what had happened and notify the program director. Ms. Bennett called the program director and said the father had called and wanted to hear back from the program director. The worker whom they believe did not appropriately change diapers is child care staff member 1, Angelique Grayhek. It was the first day without a lead in the classroom as the lead had moved to the Pre-K classroom. It was the first day of new classrooms for a lot of classrooms. Ms. Grayhek started the day at 11:00 AM, and joined child care staff member 2, Hannah Provencola and child care staff member 3, Savannah Steward in the classroom. There has not been any issues with Ms. Provencola or Ms. Steward and so they felt comfortable allowing them to return to working with the children. They or other co-workers have spoken with Ms. Grayhek several times within the month she had been employed regarding needing to be more vigilant with diaper changes. Other co-workers had noticed children who needed diaper changes with Ms. Grayhek being responsible for those said children. Ms. Bennett and Ms. Pitcock have spoken with Ms. Grayhek; however, Ms. Grayhek denies she has been talked to about diaper changes. They asked her to stay home the next day and they would call her regarding a return-to-work date. Ms. Bennett has had to go in and change a diaper with Ms. Grayhek. Child A arrived at the child care center around 7:30 AM, and when Ms. Grayhek arrived at 11:00 there had been no documented diaper changes completed on Child A. They provided coaching to Ms. Provencola and Ms. Steward, who reported they both thought the other child care staff member had completed the diaper changes. The center utilizes the KinderCare application, for information provided to parents about the child care center while there. At 12:02 PM there was a documented wet diaper in the tablet, it is not known who entered the wet diaper notification, but it is clear nobody changed the wet diaper when they saw it at 12:02 PM. They have brought back primary care assignments, entering initials of who checked/changed the diaper in tablet, and they now have white boards in the classroom/bathroom areas to document who is wearing diapers and pull-ups. They have verbally spoke with Ms. Grayhek, but not provided any previous written write up's regarding diapering. When they spoke with staff Ms. Provencola was very upset, and tearful about the incident. Ms. Grayhek just smiled and said okay, she did not seem upset or worried about the situation. Ms. Steward when spoken to was also very upset and felt badly. Child A returned the next day, they spoke with her father and provided an update as to the steps they have taken already and to the additional steps they will be undertaking to ensure this does not happen again. On May 26, 2022, I interviewed the complainant via telephone. The complainant stated that they worked with child care staff member Kim Brennan for 2 days and observed safe sleep violations everywhere. The complainant was being trained by Ms. Brennan and other staff, so they had just recently been through the safe sleep training protocol. Ms. Brennan would place infants into the cribs face down (which is not allowed until 2 they can roll over on their own) and placing blankets over their faces and entire bodies (which is also not allowed). There are cameras in the center but no one is checking them. The program director, Jaci Ettinger, has the cameras right by her office. The complainant did not say anything to Ms. Brennan because they were new, every other staff who came into the room seen the safe sleep violations and no one said or did anything. As a result, the complainant did not report this to Ms. Ettinger or the assistant director Tara Pitcock. The complainant further indicated that Ms. Brennan usually worked alone, since there are only 4 infants in that room right now and the ratio would only be 1 staff to 4 infants. The complainant observed Child A (female, age 6 months) being swaddled inappropriately. They observed Child B (male, age 5 months) being covered with a blanket over his head one time while he was very congested. The complainant removed the blanket after he was laid down with it. They observed Child C (male, age 7 months) and Child D (male, age 7 months) with blankets draped over them as well. On June 01, 2022, I completed an unannounced onsite inspection at the center at approximately 1:20pm. Program director Jaci Ettinger and assistant director Tara Pitcock were present and working. I immediately went to Infant Room 1 (Ms. Brennan's room) to observe the children. Child care staff worker Mary Jane Ockert was working, as Ms. Brennan had just left for the day. I observed Child A being diapered by Ms. Jane. I observed Child B sleeping on his back in a crib, with a blanket lying around his neck and wearing a bib. Ms. Ettinger immediately picked him up. She acknowledged that a blanket should not have been in the crib with him. I observed Child D laying on the floor propped up on a soft cushion, and he was awake. I observed Child E (male, age 10 months) sleeping in a crib on his stomach, completely covered in a blanket from head to toe. There was another unused blanket wadded up in the corner of the crib. Ms. Ettinger indicated that Child E is able to roll back and forth from his stomach to his back. She acknowledged that the blankets should not have been in the crib and they were removed. I interviewed Ms. Ettinger and Ms. Pitcock afterwards. They both agreed that the blankets are against the company's policy for safe sleep for infants sleeping in a crib. They were not sure why Ms. Brennan would place the blankets in a crib with an infant, or why the blankets remained on the children after she left. According to the daily sheets, Ms. Brennan documented that Child B had been laid down at 12:04pm and Child Eat 12:15pm. Ms. Brennan is the substitute lead caregiver for Infant Room 1. She has worked there since 2018 and received safe sleep training on June 20, 2018. The company also retrains staff every year on safe sleep policy. Recently, a new child care staff worker, Brianna Hernandez, had quit working there after only 2 days. She was training in Ms. Brennan's room most of the time. She informed Ms. Ettinger that she "saw things that she did not like." Both Ms. Ettinger and Ms. Pitcock tried to reach her via phone calls and emails to ask her what she was referring to but they never heard back from her. They do have cameras in the center for each room, but unfortunately Infant Room 1's camera has not been working lately. I was able to view the cameras and confirmed that Infant Room 1's camera was not working. They have already put in a request to fix the camera a few weeks ago. The cameras are live and 3 do not rec

On May 26, 2022, the complainant stated that Ms. Brennan frequently curses at the babies while changing their diapers. She will say they things like "we're going to change your shitty fucking diaper right now" even though the babies can't understand 4 her. The complainant would witness her saying things like this and she didn't even try to hide it. Ms. Brennan was on her phone a lot and would hardly engage with the babies at all. On June 01, 2022, Ms. Brennan stated that she would never curse at a child. She might say something like "oh shit" around them but not because of them, but because she dropped something etc. She was very upset about the allegations and denied that she would ever say bad things around the children. She works very hard and does a good job every day. She has no time to play on her phone. She is busy all the time. She only uses it to play music or enter their daily sheet information on the center's app. She has never had a complaint made against her and the parents love her. She feels like one bad person has misunderstood her and it isn't fair to portray her in this way. On June 01, 2022, Ms. Ettinger and Ms. Pitcock denied ever hearing Ms. Brennan curse or use bad language towards or around the children. No one has ever complained to them about her using bad language while working. They know that she is very popular with the children and parents and even babysits for some of them in their homes. Oftentimes Ms. Brennan works alone so there are not too many other staff workers who are in the infant room to observe her daily interactions with the children. On June 01, 2022, Ms. Nordstrom stated that she has heard Ms. Brennan use a mild swear word around the children before. It is never directed towards the children though and she could not recall a specific date or time. She is very engaging towards the children and is nice to them. She has observed her on her phone a lot, most likely because they use the center's online application for the children's daily sheets (which is on their phones). She has seen her on her phone more than some of the other staff.
