On 4/10/2025, I interviewed Ms. Wade. She stated that she has no other individuals other than CCSM and Supervised Volunteer interacting with children. CCSM stated that she and Ms. Wade are the only individuals that provide care. I reviewed staff attendance records. There records indicated that CCSM, Supervised Volunteer, and Ms. Wade work in the child care home. Child A’s Mother and Child B's Mother both stated that they have not observed any individuals inside the child care home other than Ms. Wade, CCSM, and Supervised Volunteer.

On 4/10/2025, I interviewed Ms. Wade and CCSM. Both indicated that all children's diapers are checked frequently and changed when wet or soiled. Child A’s Mother and Child B's Mother did not have any concerns with their children being changed.

I interviewed Ms. Wade, CCSM, Child A’s Mother and Child B’s Mother. Ms. Wade and CCSM denied knowing about scratches on the children and Child A’s Mother and Child B’s Mother did not have concerns regarding the care and supervision of children. This allegation has been previously investigated as noted in SI-00126748.

On 4/10/2025, I reviewed attendance records. Of two attendance records reviewed, staff signed in each time, however, they did not sign out.

She did not have any specific details. I requested to have the individual with the firsthand information to contact me and I did not receive a phone call. On 07/12/2023 Child A, B, C, D, E, F, G and H's Mothers stated that their children have not been hit or whipped with a belt by Ms. Wade.

On 06/08/2023 Ms. Wade stated that she has never given Melatonin to the children. She would not administer any medication without written permission provided by the parents. Ms. Wade currently does not have any children on medication. On 07/12/2023, the complainant stated that she did not have firsthand information, but she was told this information. She did not have any specific details. I requested to have the individual with the firsthand information to contact me and I did not receive a phone call. 07/12/2023 Child A, B, C, D, E, F, G and H's Mothers stated that their children have not received any medication from Ms. Wade.
