On 1/16/2026, a dead bat was found in room #103. An inspection showed that there was not an infestation and that this was an isolated incident. The premises are currently clear of bat activity.

On 11/17/2022, I spoke to Child Care Licensing Consultant Jennifer Koluch via phone, and she stated that she had not received any incident reports regarding St. Fabian Catholic School regarding any issues concerns. On 11/17/2022, I made an unannounced onsite inspection at St. Fabian Catholic School from 9:57 AM — 11:00 AM and interviewed the licensee designee, program director and staff members regarding the incident with Child A. First, I interviewed Licensee Designee Courtney Nixon and Program Director Samantha Mann together. They stated that an incident did occur at the facility regarding Child A being left in the classroom. Ms. Mann stated she could not remember the exact date, but it was assumed that it occurred on 11/07/2022. The two staff members in the classroom were lead caregiver Lida Kasyouhannan and child care staff member Ms. Raya Kajy with 17 children. Ms. Mann stated that Child A is in the three-year-old preschool classroom, and after nap time they were getting ready to go to the playroom which is located down the hall from the classroom. When Ms. Kajy arrived at the playroom where Ms. Lida had the children already, she noticed that Child A was missing. Ms. Kajy went back to the classroom, and Child A was sitting in a chair crying. Ms. Mann nor Ms. Nixon knew how long Child A had been left in the classroom alone. Both stated that this has never happened before. Ms. Nixon stated when she spoke to Ms. Lida about the incident, Ms. Lida felt awful and knew that it was a huge mistake. Ms. Nixon stated no disciplinary action took place regarding the incident with either staff member. Ms. Nixon and Ms. Mann both stated they did not contact Child A's parents after the incident. Child A is still enrolled, and they are unsure if Ms. Lida or Ms. Kajy contacted Child A's parents. Next, I walked the hallway where Child A's classroom is compared to the playroom. The classroom is at the beginning of the hallway and the playroom is almost at the end of the hallway and on the other side of the hall. There are about four classrooms in between the two rooms. The mat room is one room past the playroom on the opposite side of the hallway. I then interviewed child care staff member Ms. Raya Kajy, who works as the aide in the three-year-old classroom. Ms. Kajy stated that one day last week, she could not recall the exact date, but it was around 2:30 PM when all the children were up from their nap and getting ready to go to the playroom; she took the mats to the mat room down the hall. Ms. Lida was taking the children to the playroom which is located by the mat room. Ms. Kajy stated she did not know why Ms. Lida did not wait for her to return to the classroom prior to walking to the playroom. She stated most of the time Ms. Lida does wait for her, but this day she did not. When Ms. Kajy entered the 3 playroom, she stated she instantly asked Ms. Lida if she had all the children and that was when Ms. Lida noticed that Child A was not in the room. Ms. Kajy stated she then walked to their classroom and found Child A standing at the door crying. She asked Child A what she was doing, and Child A told her that she did not see a staff member in the classroom. Ms. Kajy stated that after she had Child A with her, she saw Ms. Mann in the hallway and told her what had happened. Ms. Mann took Child A with her, and Ms. Kajy returned to the playroom classroom with Ms. Lida. When she arrived back in the classroom, Ms. Lida asked her where Child A was, and Ms. Ms. Kajy stated she told her that Ms. Mann had taken her. Ms. Lida then left the room to go get Child A. Ms. Lida did return a couple minutes later without Child A and stated that she did not see them, so she came back to the classroom. Ms. Kajy stated she asked Ms. Lida how this happened, and in perspective, Ms. Lida became very nervous and said she did not know how Child A got left behind. Ms. Mann did bring Child A back to the classroom a few minutes later. She stated Child A was left in the c

On 11/30/2022, I received an incident report from St. Fabian Catholic School stating there was an incident involving the same classroom and staff members as the previous allegation. Ms. Mann stated she contacted her licensing consultant and sent her the same report on 11/28/2022. The incident report was filled out by Program Director Samantha Mann and states on 11/28/2022 around 11:00 AM, Ms. Lida left a child unattended in the hallway. Her class had come in from playing outside and Ms. Lida had taken the children's coats off except for the half day children, who kept their coats on. The full day students walked into the classroom, and the half day children remained in the hallway. Ms. Lida was in the classroom when Child B wandered out of the classroom and down the hallway unattended. Ms. Mann stated that Ms. Kajy was out in the hallway helping full day children get their coats off. Ms. Poota is a child care staff member (aide) for the four-year-old classroom, and she found Child B in the hall unattended. Ms. Poota said to Ms. Kajy "You have a child down here alone". Ms. Ms. Kajy then told Ms. Lida that Child B was down the hall unsupervised. On this day, Ms. Mann stated that Ms. Lida had been terminated from her position and no longer works at St. Fabian Catholic School. On 12/06/2022, I interviewed Child B's mother via phone, and she stated she was contacted on 11/28/2022 by Ms. Lida and was informed that when the classroom came in from recess, Child B was told to leave her coat on due to going home soon. Child B is a half day student, and after recess she gets picked up shortly after. At some point after being told to keep her coat on, Child B wandered off and went down to the next classroom. Another staff member in the hallway brought Child B back to her classroom. Child B's mother stated that was all the information she was given, and she stated it was presented that it was not a big deal, and she did not think much about it. She stated Child B attends Monday, Wednesday, and Friday half days (mornings). She stated this is Child B's first year at the school. Child B's mother stated that Ms. Lida does not work there anymore, which did cause her to go speak to Ms. Nixon because she was concerned that Ms. Lida was fired due to this incident- which she was told was not a big deal. Child B's mother stated Ms. Nixon told her that Ms. Lida no longer works there, but it was a mutual decision between the school and Ms. Lida. Child B's mother stated nothing like this has happened before and she did not have many concerns about it due to so many other staff members in the hallway. Also, she thinks Child B just got confused and thought she was to walk down to where she is picked up at the end her day. On 12/09/2022, I spoke to Ms. Lida Kasyouhannan via phone, and she stated on 11/28/2022, she had 18 children total, and five of the children were half day students. She stated after recess time in the morning, they come back into the building, and the full day students take their coats off while the five other students remain in their coats as they are getting ready to be walked down to the doors where they are picked up by their parents. Ms. Lida stated that all the children were bundled up because it was so cold outside. When they got back to the classroom, Ms. Kajy assisted the children with taking their coats off in the hallway while she went back in the room to get the five children's backpacks and folders ready for the children to take home. Ms. Lida stated while she was completing folders, Ms. Kajy ran in the classroom and yelled at her that she had lost another child. Ms. Lida stated she went into the hallway and was given Child B who was found down the hall, not with the class. Child B did not have her coat on anymore- which she should have since she was a half day student. Ms. Lida stated someone must have accidently mistaken her for a full-time student and not a half day.
