On 04/13/2021 I completed an on-site inspection to the center from 12:00pm to 1:30pm. I observed that all the classrooms were in ratio. The Owl room had 2 staff members to 6 infants. The Fox room had 2 staff members to 7 toddlers (1 to 2 1/2 years old). The Raccoon room had 2 staff members to 7 preschoolers (2 to 3 1/2 years old). The Moose room had 1 staff member to 8 preschoolers (3 to 4 years old). The Bear room had 1 staff member to 11 preschoolers (4 to 5 years old). I interviewed staff member Hayley Traczuk in the Moose room. Ms. Traczuk stated she has worked at the center for the past year and works on Mondays from 9:00am to 4:30pm; Tuesday, Wednesday, and Fridays from 8:00am to 6:00pm; and Thursdays from 9:30am to 5:30pm. Ms. Traczuk stated her room is always in ratio. She stated there are 11 children enrolled in the Moose room, and when all 11 children are present, staff member Emmallee (Hubbard) works in the room with her. Ms. Traczuk stated there has never been 1 staff member to 19 children in the Moose room or any of the rooms at the center. I interviewed staff member Danielle Jaworski in the Bear room. Ms. Jaworski stated she has worked at the center for the past 3 years and works Monday through Friday from 7:00am to 5:00pm. Ms. Jaworski stated the Bear room, and the center are always in ratio. Ms. Jaworski stated there are usually 10 to 12 children present in the Bear room, and if there are 13 children, the director Jillian (Hukill) has another staff member Emmallee or Sydney helping her. Ms. Jaworski stated they do not combine the Bear and Moose rooms until the end of the day when there are only 4 or 5 children still present. I interviewed program director Jillian Hukill. She was shocked someone made this allegation. She stated she recently had a staff member Samantha Kaczor, leave because she got a new job. Ms. Hukill stated she is in the process of hiring more staff. Ms. Hukill stated she has limited the enrollment since the Covid 19 pandemic started and does not have 19 children enrolled in any of the preschool classrooms. Ms. Hukill stated because of Covid 19 and staffing needs, she keeps enrollment in all the rooms low. According to Ms. Hukill there are 6 children enrolled in the Owl infant room, 10 children enrolled in the Fox toddler room, 8 children enrolled in the 3 Raccoon preschool room, 11 children enrolled in the Moose preschool room, and 13 children enrolled in the Bear preschool room. Ms. Hukill stated if there are more than 10 children in the Moose room or more than 12 children in the Bear room, there is a 2nd staff member working in the room. Ms. Hukill stated they do not combine the preschool rooms until the end of the day when there are just a few children still at the center. During the on-site inspection, I took a copy of the staffing schedule for 04/13/2021 and copies of the children's daily attendance records for 03/29/2021, 03/30/2021, 03/31/2021, 04/01/2021, 04/02/2021, 04/05/2021, 04/06/2021, and 04/13/2021. I asked Ms. Hukill for copies of the staffing schedule and attendance for the dates 03/29/2021 through 04/06/2021. I also requested to see the child information records for the children enrolled in the Moose and Bear rooms. On 04/14/2021 I interviewed previous staff member Samantha Kaczor. She stated when she worked at the center she usually worked in the Moose or Bear room. Ms. Kaczor stated she never saw any of the rooms at the center out of ratio and they only combined the Moose and Bear rooms at the end of the day when there were less than 10 children present. On 04/14/2021 I interviewed staff Member Emmallee Hubbard. She stated she works in all the rooms, but she is usually in the Moose or Bear room. Ms. Hubbard stated she has never seen any of the rooms at the center out of ratio, and there has never been 19 children in any room. Ms. Hubbard stated if there are 12 or 13 children in the Moose or Bear room, there are always 2 staff members present. On 05/07/2017 I interviewed pr

Between 7:39 and 7:53am there were 2 staff members Ms. Hukill and Ms. Jaworski present. At 7:39 there were 9 children including 2 infants, 1 toddler, 2 preschoolers (2 years old), 1 1/2 preschooler (3 years old), and 3 preschoolers (4 years old) at 10th the center. At 7:46am a child, an infant, arrived. At 7:51a m 11th 12th a and child, a toddler (1 years old) and a preschooler (3 years old), arrived. At 7:53am 4 staff members were required when a 13th child, a preschooler (3 years old), arrived. At 7:55am a 14th child, a preschooler (4 years old) arrived. Also, between 7:53am and 8:00am the maximum group size of 12 infants and toddlers was not maintained. On 05/11/2021 between 7:45am and 7:56am, the required ratio of 1 staff member to 4 infants and toddlers was not maintained. At 7:45am the children were in the Main room and there were 2 staff members, Ms. Hukill and Ms. Jaworski, present with 9 children including 2 infants, 1 preschooler (2 years old), 2 1/2 preschoolers (3 years old) and 4 preschoolers (4 years old). At 7:55am a 10th child arrived; a preschooler (4 years old).
