On 0110812021, I received a voicemail from Ms. Quirk-Harmer informing the department that she received0all ationrs of ina ro riate contact involving a caregiver. It is alleged that the in the preschool classroom dragged Child A acro floor, resulting in an injury to Child A's back. Ms. Quirk-Harmer was informed of these allegations by Jessica Henderson, the center's program director on 01/0712021 at 7:30pm. Ms. Quirk-Harmer emailed a picture of the injury, which was taken by Child A's grandmother. The picture shows what appears to be an abrasion on the upper right side of Child A's back. The picture has been placed in the file. On 0111112021, 1 spoke with Child A's grandmother via telephone. Child A's grandmother indicated that she received an email on 01/06/2021 at around 12:30pm from the center's program director, Jessica Henderson. In the email, Ms. Henderson informed her that Child A was having a rough day and had numerous behavioral outbursts, one which involved throwing to the floor. Child A's grandmother stated that Child A has only been attending the center since and indicated that the center was aware of Child A's history of behavioral issues. Child A's grandmother stated that the email made no mention of any injury to Child A. Child A's grandmother arrived at the center around 5:30pm to pick up Child A. At that time, Child A informed her that hit another child in the classroom and that also had a "boo boo" on back. en she looked at Child A's back, she observed what appeared to be a large scrape or abrasion. When she asked Child A what happened to back, stated "Ms. ■ dragged ■ across the floor". Child A's grandmother immediately spoke to who denied e child and appeared unsure of how Child A obtained a injury to■ back. then mentioned two incidents, one where Child A jumped onto dragcgoOt aannd fell, and another where Child A was rolling around on the floor while tying to gets boots on. Child A's grandmother also spoke with Ms. Prophet, who was the other caregiver in the classroom that day. Ms. Prophet was unable to tell the Child A's grandmother how the injury occurred. Child A's grandmother later had an 3 email exchange with Ms. Henderson, who told her that she did not think Child A's injury occurred at the center. In the evening on 01/06/2021, Child A's grandmother saw a bruise on Child A's arm and was concerned if this bruise was also a result of this "dragging incident. Child A was in attendance at the center the next day (01/07/2021); however, was ■ - the following day due to Child A's grandmother's ongoing concerns that staff members were mishandling Child A and causing injuries. Child A's grandmother stated that she had past issues with Child A coming home from the center with scratches and marks on body that were not there when she dropped ■ off. She then stated that she has a cat in the home that could have also caused these marks, so in the end she was unsure of their origins. On 01/14/2021 1 conducted an unannounced on-site inspection. I interviewed Jessica Henderson (program director),Tina Quirk-Harmer (licensee designee), and Ms. Henderson stated that on 01106/2021, she received a call from around 6:30pm. informed her that Child A's grandmother saw an abrasion on Chi 's back and accused her of dragging Child A across the floor and causing the injury. Ms. Henderson stated that she sent an email to Child A's grandmother earlier in the day on 01/0612021, because she was in the classroom and observed numerous outbursts and behavioral issues that needed to be addressed. She indicated that she wanted to reach out to Child A's grandmother to inform her of the concerning observations and come up with a behavioral plan together. Ms. Henderson received an email from Child A's grandmother on 01/07/2021 at 9:04am, informed her of the alleged incident, which included pictures of Child A's injury. Ms. Henderson stated that she has since called and emailed Child A's grandmother numerous times but has not heard back from h

Ms. Henderson was made aware of the allegations of inappropriate contact involving Child A on 01/06/2021 but did not report the incident to the department within 24 hours.
