None of the staff member saw the injury on Child A's forehead and the knot developed later. The injuries on the thumb and the forefinger were taken care with a band aid from the staff members.

On 02/13/2023 I spoke to the complainant. The complainant stated that Child A has been bitten on several occasions. The center is aware of the issue, but the biting persists. The complainant is concerned with the supervision of children at the center. On 02/13/2023 I spoke to Child A's Father. He stated that Child A had been bitten six times by Child B at Farmington Hills Nursery School. Child A told his parents that it was Child B that bit him. He said the bites occurred in the toddler 2 classroom between 07/07/2022 and 02/09/2023. Child A's Father provided me with photographs and interactions with the center for the following dates: 07/07/2022- The photograph shows a circular mark about the size of a half dollar on Child A's left forearm. It is red and purple and slightly raised. There are blue and purple indents in the skin in some areas around the circle. Child A's Father stated that an incident report was not received and that he was told it may be a bug bite. Child A's Father was not upset about this incident but was not sure it was a bug bite either. On 11/11/2022- The photograph shows a circular red mark about the size of a half dollar and two abrasions on Child A's right cheek. The mark is between his nose and ear, near the center of his cheek. The abrasions are roughly an inch long, inch wide 1/2 and two inches apart. There is inflammation about the abrasions. Child A's Father stated that he received an incident report for this injury. The incident report stated that Child A played in the house area with another child when he was bit on the cheek. On 11/23/2022, Child A's Father met with Ms. Brown. During this meeting he expressed concern about the safety and supervision of Child A in the toddler 2 classroom. Ms. Brown reassured him that Child A and Child B would be separated from one another throughout the day to prevent further biting incidents. Ms. Brown told him that if Child B bites Child A again, he would be disenrolled from the facility. On 11/23/2023 Child A was bitten again. This time he was bitten on the right forearm. A photograph shows several red small square marks outlining a circular red mark. An incident report was not received. On 12/19/2022 a fourth bite occurred. This bite occurred on Child A's finger. Child A's Father could not recall which finger was bitten. A photograph was not taken. An incident report was received. The incident report stated that Child A tried to grab a toy from another child who hit him because he could not have the toy. The other child bit Child A. Child A's Father said he was concerned for Child A's safety. On 1/19/23 a fifth bite occurred to Child A's right arm. An incident report was completed on ProCare and staff members called Child A's Mother to inform her of the incident. The 3 incident report stated, Child A was bitten on the arm by another child during play time over a toy car. On 1/20/23 (while on the school field trip), Ms. Brown told Child A's Father that this wouldn't happen again because Child B was asked to leave the school, allowing for one more week of attendance so the family could find other childcare arrangements. On 02/09/2023, Child A was bit a sixth time. This time he was bit on his left arm. Ms. Brown told the parents that the center board office has allowed Child B to remain at the center for the time being, while they review the case. A written incident report was not provided to the family. On 02/12/2023, Child A's Mother sent an email withdrawing Child A from the center due to safety concerns. Child A's Father stated that Ms. Brown told her that the children would be separated. The children remained together even after the biting incidents. He was disappointed to leave the center. He said Child A loved going to the center and he felt Child A learned a lot while in attendance. On 02/24/2023 I completed an unannounced, onsite inspection at Farmington Hills Nursery School. I interviewed Program Director, Krystal Brown, and Child Care Staff Member (CC

On 10/12/2022, I interviewed Staff Member A via telephone. Staff Member A stated that on 10/07/2022 from 12:30pm-1:00pm, she was the only staff member caring for five or possibly six children in the toddler 2 classroom. Staff Member A indicated that she could not remember the exact number of children because she was so busy that day. Staff Member A stated that Crystal Brown (program director) was aware that the classroom was out of ratio for those 30 minutes but did not immediately place another staff member in the classroom. Staff Member A stated that she believes that other classrooms have been out of ratio in the past but was unable to recall the dates and which classrooms. On 10/18/2022, I conducted an unannounced on-site inspection. I interviewed Crystal Brown (program director), Staff Member B, Staff Member C, and Staff Member D. 2 Ms. Brown stated that on 10/07/2022, Staff Member A and Staff Member B were working in the toddler 2 classroom with seven children in attendance. Ms. Brown covered all the breaks for the day and stated that none of the classrooms were out of ratio. Ms. Brown stated that she covered the toddler 2 classrooms lunch breaks on 10/07/2022. Ms. Brown worked with Staff Member A from 12:30-1:00pm, when Staff Member B went to lunch. Ms. Brown stated that she is always stepping into classrooms to give her staff members breaks and indicated that the center often has extra staff who also help keep the classrooms in ratio. Staff Member B stated that she and Staff Member A worked in the toddler 2 classroom on 10/07/2022, caring for a total of seven children that day. Staff Member B said that Ms. Brown came into the toddler 2 classroom numerous times that day and ensured that both she and Staff Member A were able to take their lunch breaks. Staff Member B denied that the toddler 2 classroom was out of ratio on 10/07/2022 and further attested that she has never seen any of the classrooms at the center out of ratio. Staff Member B indicated that Ms. Brown is always checking up on staff to make sure they are getting their breaks and helping to cover classrooms in order to maintain ratio. Staff Member C stated that she works in the toddler 1 classroom. Staff Member C denied that the toddler 1 classroom has ever been out of ratio while she was working and stated that she has never seen any of the classrooms at the center out of ratio. Staff Member C said that the staff members are all very supportive of each other, and that Ms. Brown steps in when needed to make sure that classrooms are properly staffed, and all employees get their breaks throughout the day. Staff Member D stated that she works in the multiage 2 classroom and that she has never observed any classroom to be out of ratio. She added that Ms. Brown is always checking on the classrooms and covers staff members so that they can take their breaks. While on-site I observed the four classrooms that were open. The child care staff member-to-child ratios were as follows: Infant- 1:3 Toddler (combined)- 2:6 Multiage 1 (preschool)- 2:9 Multiage 2 (preschool)- 1:8 Ms. Brown also provided me with the attendance records for the toddler 2 classroom for the following days which included the child care staff members-to-child ratio: 10/03/2022- 2:8 10/04/2022- 2:7 10/05/2022- 2:7 3 10/06/2022- 2:7 10/07/2022- 2:7 On 10/27/2022, I conducted a 2nd unannounced on-site inspection. While on-site I observed the five open classrooms. The child care staff member-to-child ratios were as follows. Infant- 1:4 Toddler 1- 1:4 Toddler 1- 2:8 Multiage 1 (preschool)- 2:6 Multiage 2 (preschool)- 1:10
